We report a 31-year-old woman in her 4th month of pregnancy who presented with a sudden decrease in her right visual acuity. The right ocular fundus showed marked retinal edema and a cherry-red spot. A cranial computerized tomographic scan showed cerebral infarction. Cerebral angiography disclosed moyamoya disease. Ischemia of the ophthalmic artery due to constriction of the origin of the internal carotid artery and hypercoagulability due to pregnancy have presumably caused visual impairment in this patient.

The visual function in 54 eyes of 27 patients with Best’s disease was analyzed by retrospectively examining the Snellen visual acuities and the electrooculogram (EOG). Visual acuities, generally well maintained in most patients, were 20/40 (0.5) or better in 41 of 54 (75.9%) eyes, and showed no correlation with age. Those in the cicatricial stage had worse visual acuities than those in the previtelliform stage. The EOG was abnormal in 47 eyes (87.0%) with a subnormal light peak/dark trough (Lp/Dt) ratio less than 1.85. Seven eyes (13.0%) with an Lp/Dt ratio 1.85 or higher had significantly smaller Dt values compared with those with a low Lp/Dt ratio. The Lp/Dt ratio did not correlate with patient age or disease stage. No correlation existed between visual acuities and Lp/Dt ratios. The Lp/Dt ratio did not reflect the severity of the macular lesions. The results indicate that not only a low Lp/Dt ratio but also a low dark trough value in cases with a normal Lp/Dt ratio are helpful in diagnosing Best’s disease.

Six patients with the acquired immunodeficiency syndrome had frosted periphlebitis associated with large areas of cytomegalovirus retinitis. Following the administration of ganciclovir, periphlebitis resolved within 2–6 weeks concomitant with quieting of cytomegalovirus retinitis. Frosted retinal periphlebitis in the acquired immunodeficiency syndrome is associated with cytomegalovirus retinitis in North America. Medical treatment of cytomegalovirus retinitis leads to resolution of the periphlebitis without the need for systemic corticosteroids.

A 3-year following-up study of the pattern electroretinogram (PERG) was performed in 15 eyes of 8 ocular hypertensive (OHT) patients. At first PERG recording, the amplitude of the first positive wave P1 was statistically significantly reduced in the OHT patients compared with age-matched controls (p < 0.05). There was no difference in the amplitude of the second negative wave N2 (p > 0.05). At 40 months, the amplitude of both P1 and N2 decreased and the latency of P1 was elongated. Among the changes, the decrease in N2 amplitude was the most drastic (p = 0.0001 compared with the control, p < 0.05, compared with the first recording). Glaucomatous visual field defects developed 5 years after the first PERG recording and 2 years after the reduction in N2 amplitude in 1 patient. Decreased N2 amplitude of the PERG may be an important warning sign predicting the development of glaucoma in OHT patients.

To determine the current place of argon laser trabeculoplasty (ALT) in the therapeutic armamentarium of primary open-angle glaucoma (POAG), 211 photocoagulated phakic eyes were analyzed. 159 eyes had a 5-year follow-up, 147 eyes had an 8-year follow-up and 127 eyes had a minimal 10-year follow-up. These eyes came from an initial group of 151 patients, 76 women and 75 men, with a mean age of 67 years. We decided, to judge trabeculoplasty results according to three combined criteria of failure –insufficient or transient intraocular pressure drop (≤3 mm Hg), progression in visual field loss and need for filtrating surgery – and according to various expected goals: prevent an increase of drugs with their subsequent side effects, taper a nontolerated medical treatment, and try to cease all medical treatment in noncompliant patients. According to these criteria, during the first 5 years, the cumulative failure rate in the 159 eyes rose steadily from 19% (31 eyes) in the 1st year to 52% (82 eyes) in the 5th year. In the 147 eyes with an 8-year follow-up, 25 eyes (17%) were succesful and in the 127 eyes with a 10-year follow-up 14 eyes (11%) were still controlled. At the 10-year follow-up the surgical failure rate (35%; 40 eyes in 113) was similar to the rate at 5 years (33%; 27 in 82). 30 eyes, which were excluded from the study, despite being still controlled, belonged to patients who had died during the follow-up. Their mean age at the time of ALT was 76 years. The low success rate at 10 years was predictable from the 10% new failure cases per year encountered in the 5-year data. Despite this important failure rate in long-term treatment, making laser efficiency comparable with drugs, trabeculoplasty is still useful in the current therapeutic armamentarium of POAG; not as a rival procedure of invasive filtrating operation reserved to presurgical glaucomas, but as a complementary treatment of the intermediate stages of the disease for which efficacy was found to be better in the current study and for older patients with a short life expectancy.

130 single piece hydrogel logel intraocular lenses (IOL) were implanted in the capsular bag after planned extracapsular cataract extraction. Anterior capsulotomy was performed by capsulorhexis in 59 patients while the envelope technique was used in 71 cases. The mean follow-up was 22 months and the average visual acuity was more than 8/10 in 78.5% of cases. In the capsulorhexis group a higher percentage of IOL was placed right into the capsular bag. No significant inflammatory and noninflammatory postoperative complications occurred. Lens decentration, observed in 5 cases, was due to asymmetric lens placement with one flange in the bag and the other in the ciliary sulcus. In both groups endothelial cell loss was about 7%. Results of the study indicate the safety and the efficacy of the logel lens as an intraocular implant. To prevent postoperative decentration and deformation of the IOL unnecessary intraocular manipulation and ‘in-out’ positioning should be avoided.

In a polycentric study, the influence of some of the most common variables capable of influencing the reliability of prekeratotomy pachymetry has been evaluated. The fluctuation in a sequence of measurements from the same point on the cornea with the patient in different positions (orthostatic and clinostatic), during progressive drying of the lacrimal film and before and after corneal marking have been evaluated. Three operators, working randomized, used the same computerized pachymeter (DGH1000) to make the measurements. The data obtained in this way were statistically analyzed by the Statgraphics personal computer program. Some interesting and useful considerations about the more correct methods to run the test and its real accuracy have emerged.
