18 patients with metaherpetic corneal lesions were treated after perforating keratoplasty additionally with the virostatic trifluorothymidine to prevent a herpetic recurrence. During a follow-up period of at least 1 year there was a recurrence in 11 % of the cases. This result is – compared with 53% recurrences of previous own cases before the use of a specific virostatic – considerably better.

In 6 cases with poor prognosis who had undergone perforating keratoplasty, the local steroid therapy was combined, for an average duration of 15 months, with a systemic treatment of cyclophosphamide and procarbazine. During a postoperative following up duration between 19 and 32 months five grafts remained clear, however, in 1 case this therapy could not stop an allograft reaction.

The clinical and anatomopathological features of angioma of the iris are described and the relevant literature is briefly reviewed. A personal case in a 47-year-old woman is presented. Reference is made to the fact that, unlike many of the cases in the literature, histological confirmation of the diagnosis was obtained. Mention is also made of the importance of fluorescein angiography in the differential diagnosis of other forms, particularly melanoma. Fluorescein angiography, for the first time applied to angioma of the iris in the present case, enabled a satisfactory result (follow-up of 8 months) to be obtained by conservative iridectomy, in line with the recent approach to the management of this form.

After a short historical survey the indication and technique of the intravitreal injection of SF6-gas within the limits of the actual retinal surgery are discussed and demonstrated by means of 10 cases. The main indications are (1) tamponade of oral and preoral giant tears, (2) tamponade of central retinal foramina, (3) tamponade of equatorial giant holes of 1–2 quadrants as well as tear groups, and (4) SF6-gas as substitute of the vitreous body.

Observation with long follow-up of the first 200 cases of intraocular injection of silicone combined with vitrectomy led us to discover three major complications: cataract, keratopathy and hypertonia, the last two found only in aphakic eyes. No toxical action of silicone could be proved to explain these complications. They seem to be related to physical properties of the silicone which we use, and in particular its gravity lower than one. This density is responsible for noxious mechanical phenomena. They can be partially kept back by preserving the lens in the first stages, avoiding segmentary iridectomy in aphakic eyes, nocturnal procubitus position and silicone ablation in failed cases.

2 cases of unilateral traumatic Purtscher’s retinopathy are reported. The pos-traumatic increase of the cephalic venous pressure may cause subconjunctival hemorrhages and retinal venous stasis followed by hemorrhages and ischaemic changes, like capillary non-perfusion and cotton-wool spots. Correlation of the fundus picture with fluorescein angio-grams suggests that the cotton-wool spots found in association with capillary non-perfusion result from the obstruction of both the orthograde and the retrograde axonal flow in the retinal ganglion cell axons. After recovery, funduscopic and perimetric evidence of nerve fibre bundle defect was found.
