Twenty-one corneas with lattice dystrophy were studied histologically to evaluate the magnitude of superficial lesions. The compromise included epithelium disarray and atrophy, degenerative pannus, disruption and absence of Bowman’s layers, amyloid deposits, and elastotic deposits. These changes were progressive according to patients’ age at time of keratoplasty. Oxytalan fibers, an elastic component, were found in affected corneas. The relation of these fibers to elastotic deposition and abnormal fibrillogenesis in lattice corneal dystrophy is discussed.

We examined 55 patients treated with hemodialysis and found 10 who showed localized chorioretinal degeneration, probably caused by a choroidal vascular disturbance. These changes manifested as a triangular patch, peripapillary or segmental chorioretinal lesions, and localized pigment epithelial alterations on fluorescein angiography. It is possible that chorioretinal complications may be a problem in patients receiving hemodialysis.

Several members of a large pedigree suffering from hereditary congenital external ophthalmoplegia, an autosomal hereditary disorder of ocular movements, were examined and surgically treated. From nystagmographic findings it was concluded that the main cause of this disorder is of supranuclear origin. Specimen of the inferior oblique muscle revealed no abnormalities or showed decrease of type I muscle fibers.

Two anatomo-clinical cases of neuro-ophthalmic sarcoidosis are reported where recognition of systemic involvement was delayed because unusual and quite isolate visual symptoms were present. The first patient had granulomas involving the intracranial optic nerve, presenting as optic nerve glioma. The second case presented as a chiasmal syndrome sharing unusual perimetric evolution. This binasal hemianopsia was probably the consequence of pre-chiasmal circulatory changes. Neuro-ophthalmic sarcoidosis should be included in the differential diagnosis of any involvement of the anterior visual pathways since these localizations are not as rare as was formerly thought. Improved awareness of this condition is a result of better neurodiagnostic studies by computed tomography which was not accurate enough a few years ago.

A 14-year-old girl had right proptosis, facial asymmetry, and gradually increasing poor vision. X-ray findings showed sclerosis in the sphenoidal bone and the base of the skull. Partial excision of dysplastic bone, decompression of the optic canal, orbit, and superior orbital fissure, and reconstruction of the upper orbital wall were performed one year later. Histopathologic examinations disclosed fibrous dysplasia of sclerotic form. There has been no progression of the disease since the surgery, and good cosmetic and visual results had been achieved.

SH U 454 is a new echocontrast agent which was developed primarily for echocardiography. It seems that there is a further potential in ophthalmic ultrasound diagnostics. SH U 454 produces contrast enhancement in the orbit, choroid and ciliary body, and permits determination of intraocular blood flow parameters in dogs’ eyes. The effects are measured with digital picture processing techniques.

The appearance of cataract in human lenses mainly consists in the formation of a protein network initiated by oxidative processes. Two types of covalent bondings are produced in the cristallins during cataractogenesis: (1) The formation of C-N bonds produced from amino groups of the cristallin proteins and aldehyd or keto groups derived from the oxidation of sugars or unsaturated fatty acids. (2) The formation of S-S bonds from SH groups specially in the gamma-cristallins. These disulfid bonds are scarcely reducible by reductants such as ascorbic acid or glutathione. Experimentally these SS bonds can be produced in rabbit lenses by incubation with riboflavin in the light. The presence of Pherajod® during the incubation period prevents the oxidation of SH groups.

15 cases of ocular retinoblastomas, 10 differentiated and 5 undifferentiated, were studied with the following antibodies: neuron-specific enolase, S100, and tubulin. All cases turned out negative with S100 and tubulin labeling. The well-differentiated retinoblastomas were NSE-positive, and only 2 of the 5 undifferentiated retinoblastomas were positive for the same antibody. The observed results let us conclude that S100 and tubulin are not useful in diagnosing this entity and that enolase, although it showed its value in the differentiated retinoblastomas, did not have the same effect in the undifferentiated retinoblastomas, as it was negative in 3 out of 5 cases.

[123I]-5-iodo-2-thiouracil (123I-ITU) was evaluated as a radiopharmaceutical for tumor detection in 10 patients with proven choroidal melanoma. Uptake of 123I-ITU was measured with a specially designed single eye probe collimator, 24 h after administration of 123I-ITU. Increased uptake in the tumor-bearing eye as compared to the fellow nontumor bearing eye was found in 7 out of 10 cases when the probe was located 3.5 cm in front of the eye (p < 0.01). By using a double pinhole collimator tests were positive in 3 out of 10 123I-ITU studies only. Tests with 123I-ITU were compared with 67Ga tests in the same patients. The 67Ga tests with the single eye probe collimator were positive in 6 out of 10 cases when the probe was located 6 cm in front of the eye. With the double pinhole collimator tests were positive in 7 out of 10 67Ga studies. It is concluded that 123I-labeled thiouracil is at least as useful as a radiopharmaceutical for ocular melanoma diagnosis as 67Ga-citrate, provided measurements are performed with a single eye probe.
