An important complication of paracentesis with disposable needles is injury to the lens being caused by the intracocular part of the needle. The new needle has a locking mechanism which prevents its penetrating too deeply into the eye and thus reduces the danger of lens injury. The tip of the needle has been extended in the shape of a lance to reduce the pressure during the puncture of the cornea and to avoid the Seidel phenomenon. The needle can be combined with a tuberculin syringe or pipette. It is superior to disposable needles, particularly in situations in which unexpected movements must be taken into account, i.e. during paracentesis in children, paracentesis where assistants are involved or during cumbersome manipulations.

In this multicentre double-blind study, 100 insulin-treated diabetics with background retinopathy were randomly assigned to treatment with either 250 mg ticlopidine b.i.d. (49 patients) or placebo (51 patients). The primary aim of the study was to assess the evolution of retinopathy by fluorescein angiography performed annually for at least 3 years. The metabolic control of diabetes was evaluated by quarterly assessement of the haemoglobin Ai level. Safety parameters, especially haematologic variables were closely monitored. The proportion of patients with favourable results was higher in the ticlopidine group (55.2%) than in the placebo group (36.6%). However, this difference did not reach statistical significance (p = 0.123), most probably because of the too limited number of patients studied. In the placebo group, a significant relationship was found between the unfavourable evolution of diabetic retinopathy and more fluctuating haemoglobin Ai levels. Such a relationship was not observed in the ticlopidine group. This could be regarded as an expression of the therapeutic effect of the compound. The number and nature of side-effects was similar in both treatment groups. However, the proportion of patients who prematurely discontinued study treatment because of side-effects was significantly higher in the ticlopidine group. Mean levels of biological, and in particular haematologic, parameters did not change significantly except for a significant increase in the cholesterol level in the ticlopidine group, which, however, remained within normal limits.

Over a 1.5-year postoperative period, surface precipitates were observed on hydrogel intraocular lenses (IOLs) by specular microscopy. In eyes with prolonged postoperative inflammation, a higher number of cells was observed. In these cases, the cells remained on the IOL surface for a longer period. We found very few small round cells and fibroblast-like cells on hydrogel IOLs during the postoperative period of our follow-up. Only in 8% (16 eyes) foreign-body giant cells were observed. From these 16 eyes, 81.25% showed an increased postoperative inflammation due to a preexisting inflammatory disposition or increased intraoperative trauma. In eyes, where a glaucoma or a corneal triple procedure was performed, foreign-body giant cells were seen in 27.3 or 10%, respectively. Fifty percent of these 16 eyes showed postoperative iridocapsular synechiae. These data show that an increased postoperative inflammation or iridocapsular synechiae cause cell reaction in most of the cases. Excluding all these cases, foreign-body cells were seen in only 1.5% on hydrogel IOLs. These results suggest that p-Hema is a material with high biocompatibility. In 43.5%, we found pigment deposits on the surface of hydrogel IOLs. These pigment granules induced almost no cell reaction with almost no phagocytosis of the pigment debris. In 7% of the cases, dust-like white precipitates of uncertain origin were seen on the IOL surface and posterior capsule; in 5%, amorphous debris was observed. Fine scratches, obviously caused by the polishing process during manufacture, were seen on the hydrogel IOLs.

Tear samples of 20 patients who were determined to have positive hepatitis B surface antigen (HBsAg) after an acute hepatitis B infection and aqueous humor samples of 10 patients who were HBsAg carriers were tested for HBsAg, and the results obtained for serum and these fluids were compared. In 85% of the 20 patients who were HBsAg positive, HBsAg was found to be positive in tear samples. In 50% of the other 10 patients in aqueous humor was also found to be HBsAg positive. Our findings show that tears and aqueous humor are important in the spreading of hepatitis B infections.

The in vitro exposure of the silicone fluid Dimethicone 1,000 to radiation from the Nd:YAG laser results in the formation of transient breakdown gases which are composed mainly of methane, ethylene and traces of ethane, as identified by head-space gas chromatography. These hydrocarbons may interfere with the intra-ocular environment in the management of retinal detachment.

Glycosaminoglycans (GAGs) extracted from the trabecular meshwork (TM) of rabbit eyes were analyzed by cellulose acetate electrophoresis and sequential enzymatic degradation, and compared with GAGs extracted from the adjacent tissues including the cornea, sclera, iris and ciliary body. The TM contained hyaluronic acid (HA), chondroitin 4- and/or 6-sulfate (CS), dermatan sulfate (DS), keratan sulfate (KS), heparan sulfate (HS) and an unidentified band of Alcian-blue-stained material which was resistant to degradation by the enzymes and chemicals used. The GAGs of the rabbit TM included: 13.9% HA, 31.6% CS, 47.1% DS, 4.6% KS and 1.2% HS, and were distinct from those of the cornea and the sclera.

The anterior segments of rabbit eyes were dissected into two types of corneoscleral explants: one a large ring-shaped block of tissue from one eye and the other consisting of small blocks of tissue from the other eye which was dissected into 8 parts. Explants were cultured for up to 2 weeks. The glycosaminoglycan (GAG) profiles of the organ-cultured rabbit trabecular meshwork were then investigated and compared with respect to explant size. The results revealed: (1) similar trabecular GAG profiles in both sizes of explants, (2) trabecular GAG profiles obtained within the culture period came to resemble those of noncultured specimens and (3) incorporation of the radiolabels continued for up to 5 days, with 2 days sufficient for GAG analysis.

A 66-year-old woman had a unilateral infarction in the cerebellar peduncle. Saccadic eye movements were intact in this patient, while smooth-pursuit eye movements were impaired. Magnetic resonance imaging disclosed that the lesion was well localized in the superior cerebellar peduncle and the juxtar-estiform body. These findings support the idea that saccade signals from the cerebellum project to the brainstem through the hook bundle which was recently confirmed in the monkey.

We report a case of evolutive high hyperopia in a child aged 10 years. It is associated with organic amblyopia due to a large papillomacular retinal fold. Ultrasonography and CT scan helped to establish the diagnosis. This is probably a hereditary disease affecting the development of the posterior sclera.
