This report will describe the clinical findings of a collaborative program to provide appropriate neuropsychiatric skills for 60 children ages 8 to 14 (grade 3 to 8) and their teachers, comprising the entire population of two sef-contained units (eight classes) for the treatment of children designated as severely emotionally handicapped but not autistic. Of the 60 children, 36 (60%) were suffering from learning disabilities. Eighteen of these had neurological signs suggestive of organic defect of the central nervous system; the remaining 18 children with learning disabilities had specific or developmental learning disabilities characterized by perceptual immaturity in spatial orientation and/or temporal organization, but the classical neurological examination administered to them was within normal limits. Ten additional children were classified as having an attentional deficit disorder, and 11 additional children were functioning at a borderline level of cognitive ability. Fifty-nine of the 60 children were suffering from diagnosable psychiatric disordes: 21 (35%) severely anxious, 13 (22%) clinically depressed, 12 (20%) conduct disorders, 6 (10%) oppositional, 3 (5%) schizophrenic, 2 (3%) obsessive-compulsive disorder, and 2 (3%) personality disorder. Applications of these findings are suggested.

Four factors are commonly considered to indicate “high risk‘’ for child abuse/neglect: (1) low birth weight, (2) young maternal age, (3) mother with single marital status, and (4) low socioeconomic status (SES). These factors were investigated for Oahu, Hawaii, in four separate retrospective studies (using 100 abused cases and 100 controls), to evaluate their usefulness for screening at delivery. Data were obtained from a sample of 2.036 confirmed cases, during the period from 1977 to 1979. Contrary to many studies from mainland United States, low birth weight and single marital status werenotassociated with increased risk of child abuse/neglect. Single marital status in Hawaii may not be associated with increased risk because it does not have the same social and cultural connotations as it does on the mainland United States. This observation suggests a need to consider regional and cultural variations in investigating and screening for child abuse/neglect.

This study compared values placed on functions relevant to the well-being of young developmentally handicapped children by parents, community members, and clinicians. Thirty-two (32) parents of developmentally handicapped children and 32 adult community members were sampled in this study. The results are reviewed.

Observations were performed on 45 very low birth weight (VLBW) newborns at 7 and 21 days of life, using a Neonatal Behavioral Assessment Scale specifically modified for the extremely premature infant. Findings were analyzed using multiple and stepwise regression techniques to determine the influence of intrapartum and neonatal factors which might influence neurobehavior.Perinatal asphyxial descriptors accounted for significant variance on day 7 behavior; however, their impact was muted by day 21. At this time, jaundice and postnatal nutrition revealed a prominent and independent behavioral influence on the VLBW survivor.

Pediatricians are often the first professionals consulted by families concerned that their teenagers may be experiencing emotional distress. Assessing the meaning of psychologic symptoms in adolescents can be a difficult task. The physician is frequently called upon to determine if an adolescent's difficulties fall within the spectrum of normative behavior or whether psychopathology is present which demands therapeutic intervention. In making this decision, the pediatrician must conduct an evaluation which considers multiple clinical factors. This report presents a framework for the pediatrician in evaluating the psychologic functioning of adolescent patients.

This paper offers guidelines to pediatricians and other developmental specialists who might be involved in counseling parents of autistic children. Although the paper focuses on the initial informing session, much of the material also applies to followup counseling of parents and other family members. The need for careful diagnosis is discussed as well as recent changes in thinking about autism that bear upon parental counseling. Suggestions are offered to facilitate the parents' understanding of their child's autistic condition, degree of cognitive and adaptive impairment, and long-range prognosis. Information is also provided to help in dealing with parents' questions about etiology and reactions to the diagnosis. The final section reviews material that should be considered by the clinician in making recommendations for treatment and education.

In modern Western culture, “mass hysteria” has typically taken the form of an outbreak of a mysterious physical disorder among adolescent girls in whom no organic etiology or precipitant can be identified. If properly managed (i.e., diagnosis confirmed and publicly announced), the illness is usually of relatively short duration. This is a report of an incident of contagious hysteria among preadolescent girls, manifested as grieving reactions during what should have been a happy event—a birthday party. The complex relation between situational, developmental, physical, and personality factors in mass hysteria are discussed.
