The purpose of this study was to evaluate the developmental outcome of a group of very low birth weight children (VLBW) with and without severe bronchopulmonary dysplasia (BPD), at the time of discharge from the nursery. Children matched for birth weight and gestational age with mild or no BPD served as controls. All of the children were evaluated developmentally at two years of age, using the Bayley Scales of Infant Development. Neuromotor function was assessed by occupational and physical therapists. The data suggested that children with severe BPD performed more poorly on developmental testing and had a higher incidence of neuromotor pathology than did controls. However, the controls did manifest suspicious neuromotor findings. The data suggest that infants with BPD are not a homogeneous group. Very low birth weight infants, whether or not they have severe BPD, are at risk for having developmental delays. These infants should be closely monitored from the time of nursery discharge.

This article reviews the literature concerning psychological characteristics of adolescents with idiopathic scoliosis. Although much has been written on the topic, it is largely based on clinical judgment rather than empirical data. In spite of methodological flaws, the few empirical studies available raise questions regarding the following issues: (1) maladjustment occurs in a significant number of adolescents with scoliosis; (2) there are sex and age differences in response to scoliosis diagnosis and treatment; and (3) noncompliance is a major factor in treatment. Implications for further research are discussed.

This report reviews the subject oflatchkey children. It provides statistics on the number and ages of children involved and the hours per day that they remain without parental supervision. The results and methodological problems of the few studies on latchkey children are discussed, and suggestions for future research are offered. Possible solutions for these children and their parents are suggested.

This report provides a selected overview of the concept of attachment and its formation in handicapped children. The implications of attachment development in more severely impaired children are discussed. The presentation of preliminary data on attachment from 50 mother-child dyads suggests that this phenomenon does exist and can be assessed in very impaired children. Child attachment responses formed a continuum ranging from bland or neutral behaviors to subtle acknowledgment of mother's presence or absence (e.g., by nodding or glancing) to a more pronounced response (e.g., smiling, crying, sitting up, locomoting). Compared to nonhandicapped children of similar developmental age, the responses of the children in this study were less complex, provocative, and differentiated. The implications of these data for pediatric practice and promoting attachment in families of severely impaired children are discussed.

Intellectual and educational testing, employing primarily the Wechsler Intelligence Scale for Children-Revised (WISC-R) and the Wide Range Achievement Test (WRAT), was administered to two groups of children, one with acute lymphoblastic leukemia (ALL), the other with a solid tumor (ST) which didnotinvade or involve the central nervous system (CNS). Initial testing was completed within 30 days following diagnosis and repeated one year and three years later. Separate ANOVA procedures on the ALL and ST patients were completed, with followup ANOVA procedures for two age groups. The ALL patients declined significantly on WISC-R vocabulary (p < 0.05) and Full Scale IQ (p < 0.05), but no significant changes were obtained from the ST patients. Separation of the ALL group into two age groups, <8 years and ≥8 years revealed significant declines in the younger group on Verbal IQ, Performance IQ, Full Scale IQ, and WRAT arithmetic. However, no significant declines were observed in the older ALL group. The results suggest that declines in ability and achievement are limited to those ALL patients treated with CNS prophylaxis at relatively young ages.

In the population of learning-disabled (LD) children, there appears to be a subsample who demonstrate basic dysfunction in sensory integration. Previous research suggested that sensory integration (SI) therapy may be effective in improving the academic performance of this subsample of LD children. This study is a systematic replication of previous research on the effects of SI therapy on LD children. The 87 LD children in this study manifest moderate SI dysfunction, a vestibular processing disorder, perceptual deficits, and reading retardation. Data on a subgroup of 45 children with markedly depressed postrotary nystagmus are presented. After nine months of individual SI therapy, no systematic effects on any of the dependent variables were discernible. The implications of these findings for further research are discussed.

The increasing emphasis on screening the pediatric population for developmental delays requires that rapid and efficient screening tests be developed and evaluated. This study assessed the ability of three rapid screening procedures to predict full Denver Developmental Screening Test (DDST) results. Ninety children were randomly assigned to one of three prescreening groups. The first group was prescreened with the Prescreening Developmental Questionnaire (PDQ), administered in the “standard” fashion, i.e., mother reads and answers 10 questions about her child's development. The second group was prescreened using a modified means of administering the PDQ (PDQ-M); a health care professional read each PDQ question to the mother. The third group was prescreened with the Alpern-Boll Developmental Profile II, a relatively lengthy procedure utilizing parent report information. All of the children were then tested with the DDST. Each of the prescreening procedures was highly predictive of performance on the DDST. The Alpern-Boll profile, as anticipated, demonstrated greater validity than the PDQ and PDQ-M on six of seven indices of prescreening test validity, generated from a four-group referral matrix. The PDQ-M was more similar to the Alpern-Boll on four indices, while the PDQ was more similar on three. It was judged that the two PDQ procedures were equivalent in their validity characteristics. The PDQ-M appears to be an acceptable means of rapidly screening preschool children for developmental delays.
