A review of the clinical profile of congenital sensory neuropathy with anhydrosis is presented. It is stressed that major diagnostic criteria of this recessively inherited condition should be limited to insensitivity to pain with normal tactile perception, anhydrosis, recurrent unexplained fever, self-mutilation, mental retardation, hypotonia, histologically normal sweat glands and variable autonomic abnormality. A case conforming to this description is reported and compared with 13 published cases. Special investigations of the autonomic nervous system through measurement of urinary catecholamine metabolites and psychophysiologic variables were conducted on this patient. Based on the analysis of 5 x 24-hour urine, values of metabolites of dopamine and epinephrine were normal. Metabolites of norepinphrine, such as 3-methoxy-4-hydroxy phenylglycol and normetanephrine, however, were significantly low when compared with those of four controls, suggesting decreased peripheral and central norepinephrine activity. Polygraph recording and evaluation of some orienting response components revealed no obvious signs of autonomic perturbation and, specifically, no phasic electrodermal activity. These two findings (biochemical and electrodermal) strongly suggest an autonomic imbalance, specifically the sympathetic component, both central and peripheral. It is suggested that autonomic disorder is an integral part of the syndrome and may be demonstrated by special investigations.

摘要:
The prevalence of focal cerebral injury in children with delayed speech acquisition is unknown. Among 30 children with a delay in speech acquisition, computerized brain tomography revealed focal left temporal lesions in two of the children. Unilateral temporal lobe damage may account for a small proportion of children with delayed speech acquisition.

Much of the literature dealing with this topic has limitations. Additional study is recommended since changes in therapy or prophylaxis of both otitis and hearing deficits may have major effects on cognitive and emotional development.

The developmental disabilities are specific diagnoses on a spectrum of chronic neurologic handicaps. Complete diagnosis and habilitation planning demands awareness of the underlying continuum of cerebral dysfunction common to all the disabilities. Careful attention to this continuum can result in the recognition of critically important associated deficits. Developmental dissociation is a useful developmental phenomenon in the early diagnosis of a particular disability. Developmental deviancy is an aid in early detection of milder disabilities or associated deficits. Professionals familiar with the entire spectrum are necessary to provide optimal care for developmentally disabled children.

摘要:
Results of a questionnaire assessing pediatricians' perceived competence regarding psychosocial, developmental and traditional medical problems were analyzed. The data were compared with a similar questionnair distributed to seventh grade teachers, youth and parents assessing problems for which pediatricians were utilized. The results reveal that pediatricians rate their competency higher in the area of traditional medical problems. The implications of this study are discussed.

The family's perception and expectation of child psychotherapy was evaluated. A modified version of a survey of expectations was administered to children (and their accompanying parent) before and after their initial intake interview. Generally the child and parent anticipated that the therapist would raise psychiatric issues, while being supportive in approach. Child and parent, however, perceived that the therapist raised fewer medical and psychiatric issues than expected. Approximately 50% of the children felt that their behavioral problems were integrally related to family psychodynamics. Parents, however, often perceived that the problems involved one person and were not a family issue. Although many children did not recall the specific reasons for referral for therapy they conceptualized the problem as one related to some “difficulty” they were having. It appears that children often begin treatment with sophisticated and perceptive notions of the therapy process and the need for getting help.

Behavioral characteristics of very low birth weight infants (<1,501 gm) were determined with the term Brazelton Neonatal Behavioral Assessment Scale and Selected items from a preliminary form of the scale for premature and at-risk infants. The behavior of 188 infants was assessed at a time as optimal as possible for social interaction. Our very low birth weight infants were upset less easily, had several reflexes with below average scores, and were less responsive to visual stimuli than term infants of other studies. Improved description of interactive abilities and tolerance of the testing procedure was made possible with some items from the preliminary premature scale. Orienting responses in 6 infants tested after 44 weeks because of prolonged illness were less than optimal with a single exception. This suggests that behavioral capacity, not age, should define where the Brazelton scale is not applicable.Perinatal factors including birth weight, gestational age, and age that weight gain began, predicted some motoric and state control responses. Whether this predictive capacity will persist or change with time awaits results of follow-up of these infants.

The present paper presents two techniques (i.e., a modified time out procedure and a response cost procedure) with which pediatricians can counsel parents regarding how to manage discipline problems outside the home and difficulties associated with children's morning routines in preparation for school.
