AbstractBlood glucose meters are now in common use in many hospital wards, in casualty and emergency departments, and also for home‐monitoring of diabetic patients.Recurring problems in blood glucose measurements have been reported, and we present here a simple algorithm to solve the most common errors encountered in the use of the Glucometer II (Ames, Slough UK), with explanations of the expected effects. Some of the checks, eg reagent strips and light window checks, will also be applicable to other blood glucose meter

AbstractOne hundred diabetic children and their parents completed a questionnaire about their hypoglycaemic episodes. The median frequency of episode was once every two months, but 23% of children were experiencing one or more each week. Episode frequency did not correlate with age, insulin dose or glycosylated haemoglobin. In most instances hypoglycaemia occurred at no regular time but 11% were usually nocturnal. The commonest identified daytime cause was exercise. Only half the children could always confidently recognize their hypoglycaemic episodes. Symptoms sometimes varied markedly between each episode. Children of 6.5 years and younger were frequently thought to feign symptoms of hypoglycaemia.Parents recognized hypoglycaemia in their child most commonly by first noticing facial pallor, changes in their children's behaviour (eg irritability, aggression, disobedience, confusion) and sweating late in the episode. Children themselves most commonly reported feeling tremulous or experiencing sudden involuntary muscle jerks and sweating as the first recognizable symptoms. A wide range of other signs and symptoms were described. Children reported bizarre feelings and experiences during hypoglycaemia. Sixteen percent of parents had, at some stage, used glucagon to treat severe hypoglycaemia in their child.This study provides important basic information about how parents, children and health professionals can recognize hypoglycaemia and has modified the advice which we give to diabetic children and their parents.

AbstractEducational holidays provide an opportunity to assess the advice given to parents and diabetic children and provide insight into the difficulties that they face in everyday life. We describe our experience of camping with 43 diabetic children for two weeks. Hypoglycaemia was common but severe nocturnal episodes may be avoided by assessing the blood glucose in the evening and providing extra carbohydrate if the glucose value is less than 4.5 mmol/l at that time. Children describe hypoglycaemic symptoms which may differ from those described in standard textbooks and are predominantly neuroglycopaenic. It was impossible to improve diabetic control using only twice daily blood glucose monitoring. In conclusion, children should be encouraged to increase the frequency of daily blood glucose testing and parents should be provided with alternative methods of assessing diabetic control such as the results of glycosylated haemoglobin measurements.
