To determine whether a single morning plasma level of 17-hydroxyprogesterone (170H-P), androstenedione, testosterone and progesterone reflected the degree of control of 21 -hydroxylase congenital virilizing adrenal hyperplasia (CVAH) as indicated by 24-hour urinary 17-ketosteroid and pregnanetriol excretion, 142 simultaneous 24-hour urine and morning blood collections were made from 65 patients with CVAH. Patients were grouped into five categories on the basis of age, skeletal age, and sex. Paired blood and urinary data were analyzed. The results suggest that androstenedione is the most reliable indicator for all patient categories. Testosterone is an excellent indicator for children of both sexes and for adolescent and adult females. Levels of 170H-P are difficult to interpret, as they can be several fold higher than the normal values when adrenal suppression appears adequate on the basis of urinary data. In general, progesterone is a poor indicator.

62 cryptorchid boys aged 2–6 years were selected at random either for surgical or for hormonal LH-RH treatment. As all biopsies from boys operated show typical histological and ultrastructural signs of cryptorchidism, it can be concluded that only true cryptorchid patients are included in our study. LH-RH treatment was successful in 16 (55%) of 31 boys. Median 30-min response values to LH-RH test of LH were initially normal in all boys. Those treated successfully remain normal after treatment whereas unsuccessfully treated patients have significantly lower LH 30-min response values at the end of the 4 weeks of LH-RH treatment. FSH response was not statistically different before or after treatment. This might indicate that a normal LH response is necessary for testicular descent. In the 15 boys operated after 4 weeks of unsuccessful LH-RH treatment, testes biopsies show recruitment of the Leydig cell precursors and development of juvenile Leydig cells with an increase in their content of lipoid droplets and smooth endoplasmic reticulum. No adverse effect on the number of spermatogonia could be observed after 4 weeks of LH-RH treatmen

Highly sensitive and specific radioimmunoassays to measure urinary FSH and LH were developed and applied to unextracted urine. The use of radioimmunoassays of gonadotrophins in 0.1 ml urine gave results which were significantly correlated (p < 0.001) with those obtained in acetone precipitates of urine. Gonadotrophin levels determined in unextracted urine were not influenced by variations in urine pH and by dilution. Urinary immunoreactive FSH appeared closely similar to pituitary standard, whereas LH was heterogeneous and partly different from the purified pituitary preparation. The urinary excretion of gonadotrophins was studied in 16 normal and 50 diabetic children and adolescents in relation to different time periods throughout day and night and to pubertal development. Diabetic subjects were shown to be similar to control subjects with respect to the daily excretion of gonadotrophins and creatinine. In both sexes, a sleep-wake pattern of FSH and LH excretion was observed, the lowest values being obtained during the night and the highest during the morning between 8 and 12 h. This circadian rhythm was found even in prepubertal children. Marked changes seem to occur throughout puberty: gonadotrophin levels excreted in morning urine increase mostly until midpuberty (stage 3) whereby the circadian pattern is most apparent; night excretion of gonadotrophins remains at a prepubertal level until midpuberty and slightly increases in late puberty (stage 4–5). The effects of surgical trauma and hemorrhagic shock on the circadian rhythmicity of corticosteroid secretion in the rat were investigated. The estimations were performed at 4-hour intervals for a 24-hour period. Control animals exhibited a characteristic circadian rhythm of plasma corticosterone with peak concentrations occurring at 8 p.m. followed by a gradual fall during the night, reaching a minimum at 8 a.m. Severe stress induced by hemorrhagic shock or surgical trauma caused a dramatic alteration in corticosterone rhythms which persisted up to 72 h following surgery or hemorrhage. It is apparent that the physiological mechanisms which regulate adrenal rhythmicity are disrupted for a prolonged period following major stress or trauma.

This study examines the effects of ovarian hormones on the glycaemic actions of insulin, glucagon and epinephrine. Ovariectomized adult female mice were treated with replacement doses of oestradiol, progesterone, both hormones combined or vehicle only for 15 weeks. Compared with intact control mice, ovariectomy did not significantly alter insulin-induced hypoglycaemia. However, treatment with oestradiol or progesterone alone, but not in combination, increased the hypoglycaemic action of insulin. The hyperglycaemic effect of glucagon and epinephrine was increased by ovariectomy and reduced by the ovarian hormone treatments. The results indicate that oestradiol and progesterone individually synergize the hypoglycaemic action of insulin, but mutually antagonize each other in this respect. Oestradiol and progesterone, individually and in combination, appear to suppress the glycogenolytic action of glucagon and epinephrine.
