摘要:
Since their introduction in the mid- 1980s, roll-on silicone sleeves have evolved into a widely used method of prosthetic suspension. Through the course of clinical practice, certain recurring skin conditions have been associated with roll-on silicone sleeve use. The purpose of this research project was to review pertinent literature and develop a survey to investigate the incidence of dermatological problems in silicone suspension sleeve users. Data were collected from patients as well as practitioners. Strong trends were noted indicating the incidence of dermatological problems in silicone suspension sleeve users is influenced by aging, activity level and “use patterns.” Observations made in clinical practice that currently have not been scientifically analyzed are noted separately.

Each individual holds an idealized mental picture of his or her physical self; he or she uses this image to measure concepts related to body image. Once an individual's percepts or concepts of this body image are altered, emotional, perceptual and psychosocial reactions can result.Psychosocial well-being often is affected by such factors as anxiety and depression, concerns with self-esteem, and satisfaction with life. In the general population, a person's positive or negative feelings toward his or her body have been found to affect his or her well-being.Living in a society of mostly ablebodied persons, individuals who have disabilities must contend with comparing the appearance of their bodies and functional capabilities to those of others around them. Mental health practitioners often see physical deviation from the norm ar central to people's behavior and personality.The following article strives to provide prosthetists and orthotists with insight into the relationship between body image and psychosocial well-being.

摘要:
A lower-limb prosthesis is the mechanical device with which an amputee's residual limb interacts with the walking surface. The pressure and shear forces that affect the residuum due to prosthesis use are the sources of pain, residuallimb skin problems and gait deviations. Direct approaches to reducing these problems include improving fit, alignment technique and socket design as well as increasing cushioning with socket liners.A summary of typical malalignments and their consequences is presented. The malalignments are considered sources of excessive moments applied to the residuum, which simplifies the analysis of a patient's gait. A better design of prosthetic joints could improve prosthetic gait.This article addresses the key mechanical parameter of prosthetic joints, namely the dependence “moment of resistance/angle of deflection.” A mathematical model has been developed that links stresses on the residuum in transtibial amputees with the moment of resistance in the prosthetic ankle at the critical gait phases. Analysis of the model yields a substantial decrease in stresses on the residuum during the most demanding, load-bearing phase of stance if the moment of resistance in the ankle is similar to that seen in the biological ankle joint. Gait study shows use of the experimental rolling-joint prosthetic foot more closely simulates normal gait synergy than the SACH foot.

摘要:
This article discusses a new endoskeletal design for prosthetic hands for young children with upper-limb deficiencies. Working mechanisms of fingers were constructed and tested to evaluate the concept. Preliminary results indicate this endoskeletal approach has merit for possible improvement in efficiency, appearance and feel compared to existing mechanical hands. In the coming months, the foam covering with skin over the endoskeletal mechanisms will continue to be developed and tested.

With today's rapid healthcare reforms, the O&SP practitioner's ability to quantify the quality of healthcare delivery and treatment cost-effectiveness is seminal to the future success of clinical practice. The evolution of total quality management in the manufacturing sector created concepts of quality control procedures. Today, government, managed care and other industries are assessing and using outcomes measures as the principal mode of quality evaluation of healthcare delivery.The future of successful patient management will require routine documentation and quantification of patient information, which can be accomplished with the help of existing low-end to high-end technology. The reasons why practitioners should use outcomes measures and how they can be applied to all aspects of clinical practice are discussed and illustrated.The O&P practice can benefit from objective documentation, data collection and outcomes measures if these tasks are performed in a routine and uniform manner; these practices can improve patient evaluation and measurement, aid in selecting the most appropriate appliances, help negotiate provider contracts, reinforce ethical practice management and minimize the risk of litigation.

摘要:
An improved cable guide assembly applicable to conventional transhumeral prosthetic devices has been designed. When subjected to varying rotational and directional cable stress, this assembly has the ability to automatically align itself, minimizing cable stress. The assembly is particularly applicable to prosthetic devices and, besides extending cable life significantly, the occasional changing of the cables is facilitated and can be easily accomplished by the patient.
