摘要:
A 96-hour course was created to instruct prosthetic and orthotic clinicians on computer-aided design!computer-aided manufacturing (CAD/CAM) principles and techniques. This course was presented using distance education technology to link the instruction site (Toronto, Canada) with the student site (Ottawa, Canada). Through interactive computer graphics and standard telecommunications, course participants studied the CASDaM, CAN FIT-PLUS, Shapemaker and University of Texas CAD I CAM systems. A qualitative evaluation of each software package was submitted by each participant during course evaluation. This education method was endorsed by course participants and instructors as a viable technique for clinical CAD/CAM instruction.

摘要:
The halo fixation device is used widely for treating traumatic injury to the cervical spine. Its major complications, pin loosening and infection, can be minimized by retightening, increasing torque and practicing local pin care. Local pin care includes applying a reagent to the skin at the pin site and to the halo ring hole or pin threads. The coefficient of friction at the halo ring/pin thread interface is a major determining factor in the relationship between the applied torque and the radial compressive stress applied to the skull. This relationship was determined for halo pins coated with reagents in common clinical use. The type of reagent used had a profound effect on the coefficient of friction and the radial compressive stress. At 0.9 N-m (8 in. -lbs.) the mean radial compressive stress using pins coated with betadine, hibiclens and hydrogen peroxide averaged52 MPa (7,550psi), 49.1 MPa (7,130psi) and53.5 MPa (7,770 psi), respectively, compared to uncoated pins and pins coated with antibiotic ointment, which averaged 66.7MPa (9,680psi) and 70.3MPa (10,200psi), respectively. These latter two radial compressive stresses were precipitously close to the ultimate radial compressive strength of cranial bone.

摘要:
The SMART®Wrist-Hand Orthosis (WHO) is a new method of providing grasping function for quadriplegic patients. The use of shape memory alloy (SMA) actuators in rehabilitation technology (SMART) allows for a new type of actuator in addition to traditional actuators such as Bowden cables, pneumatic cylinders, electric motors and linear ratchets. It provides excellent function and ease of use.

Ankle disarticulation amputation in patients with peripheral vascular disease is sometimes complicated by delayed healing or residual limb volume fluctuation. This change in shape and contour presents several challenges to the prosthetist charged with producing a static prosthesis that must meet the demands of a constantly changing residual limb.This article describes a method of producing a preparatory ankle disarticulation prosthesis that can be quickly and inexpensively fabricated, and can adapt to changing residual limb size and shape.

摘要:
This article discusses the evaluation, impression, modifications, fabrications and rehabilitation of a patient with bilateral hip disarticulations necessitated by severe decubitus ulcers secondary to a 7th thoracic level (T-7) spinal cord injury. The care of this patient was a challenge due to bilateral amputations at such a high level and the special needs of a patient who has a spinal cord injury.

摘要:
Surgically successful Syme amputations have often been followed by use of less successful prostheses. A unique design for medial-opening Syme prostheses has been developed to address issues of comfort, flexibility, strength, lightness and durability. Parallel goals of reducing fabrication complexity, and therefore time, as well as providing ease of adjustment were also achieved.The design employs carbon graphite material in conjunction with a construction technique that is compatible with commonly used plastic fabrication methods. The procedure is designed to be readily adaptable to advances in materials technology. This article details the fabrication procedures for this medial-opening Syme prosthesis.
