The mechanics of human gait involve synchronization of the skeletal, neurological and muscular systems of the human body. This article will enhance the gait assessment abilities and processes of the novice clinician by providing a brief overview of normal gait followed by identification of pathological and pathomechanical gait patterns. Also presented is a systematic approach to clinical gait assessment that emphasizes the need for the development of clinically cost- and time-effective tools to document and quantify data in the assessment of human gait.

Many patients referred for orthotic or prosthetic services will have associated neurologic deficits. These must be recognized by the practitioner prior to the design and fitting of an orthosis or prosthesis to avoid costly and potentially harmful errors in design and application. Commonly seen deficits are loss of protective sensation, impaired proprioception and spasticity. Any one of these may be associated with fixed soft tissue or bony deformity. A working knowledge of sensory and motor examination, combined with careful history-taking, will greatly enhance the clinical skills of the prosthetistlorthotist in this challenging aspect of practice.

Orthotists must be able to perform accurate evaluations of a patient's joint range of motion and muscle volition to provide successful outcomes to patient management. Discussed are basic concepts related to the successful achievement of these evaluations. Because physical therapists perform these evaluations daily as an integral part of their therapy, references are made to their texts and methods.

The National Commission on Orthotic and Prosthetic Education (NCOPE) has re-evaluated O&P educational philosophy for the past 18 months. Important decisions have been made concerning both primary education and postgraduate training. Primary education will now focus on clinical knowledge with the technical aspects de-emphasized. Postgraduate education will be structured and essentials will be written to address the clinical and technical skills necessary to educate competent practitioners. O&P education is now recognized by the American Medical Association as a legitimate allied healthcare profession, and NCOPE has become a member of CAHEA, the major accrediting agency for health-related occupations. This affiliation has forced NCOPE to rewrite and upgrade its primary and certificate programs' accrediting procedures. These are important advances for O&P education. This shift in educational philosophy will produce more competent practitioners for the future.

Ankle-foot orthoses (AFOs) are categorized four ways with respect to their shape: posterior spring, anterior spring, side-stay and spiral. In this article the flexibility of these types of AFOs fitted to the limb is measured using a muscle training machine. A comparison of the flexibility of different types of AFOs is made.
