摘要:
The energy required to walk using a newly designed knee-ankle-foot orthosis (KAFO). has been evaluated. The new KAFO locks the knee during stance and allows free-knee motion during the swing phase of gait.The energy required for gait on level ground and on a slope with a 5-percent incline was evaluated inapostpolio subject. Comparisons were made between the standard locked-knee KAFO and the free-knee (unlocked) configuration. The oxygen consumption rate (ml/kg/ min) and energy cost (ml/kg/m) were significantly lower during free-knee gait. The results of the study show that a KAFO design that allows free-knee motion during swing is effective in lowering the energy required for walking.

摘要:
People with spinal cord injury at the C5/C6 motor complete level typically have relatively well-preserved biceps function but minimal or no voluntary control of triceps. The authors previously have shown the lack of voluntary control of triceps results in deficiencies in speed and accuracy of elbow movements as well as reduction in reachable workspace. The authors also have shown these deficiencies can be corrected by the addition of constant extensor torque at the elbow. The purpose of this article is to describe a prototype constant-torque elbow orthosis and illustrate the improvements in function it affords people with C5/C6 tetraplegia.

In the evolution of quality assessment activities within the healthcare field, outcomes measurement has become the principal objective for demonstrating optimal care. Clinical pathways increasingly are being used as the methodology of choice to achieve acceptable outcomesThis article briefly describes outcomes and clinical pathways. It also suggests a practical, step-by-step approach for O&P facilities to use in the development of such pathways for orthotics and/or prosthetics patients. This approach will facilitate the impact that each provider and setting of service has on the patient's outcome. Finally, an abbreviated bibliography is provided as a resource for further reading

摘要:
The purpose of this article is to evaluate the preference of transtibial amputees for the Silicone Suction Socket (3S) versus the supracondylar PTB prosthesis with pelite liner and to explore the reasons for their preferences.Eight patients with residual-limb problems were asked to wear each type of prosthesis for a minimum of 10 weeks (except if no definitive 3S was made due to problems with the trial socket) and subsequently were asked to complete questionnaires to discuss comfort, ambulation and activities of daily life, working conditions, and sports activities with the prostheses. Two patients did not receive a definitive 3S because they could not walk comfortably on the trial socketTwo of the six amputees who used both socket designs preferred the 3S, two preferred the supracondylar PTB with pelite liner, and two were satisfied with both prostheses.

摘要:
The concept of making inferences from research data is discussed in this article. Three hypothetical examples of research projects in O&P are developed. In each example, the emphasis is ondrawing inferences from research data rather than the statistical methods used.The first research example compares patient ratings of two spinal orthoses using the t-testfor independent means. A hypothetical data set is used to illustrate common statistical inferences made from the analysis. Practical inferences also are examined.The second research example uses a one-way analysis of variance (ANOVA) and post-test comparisons to evaluate four lengths of a wrist-hand orthosis for carpal tunnel syndrome. A hypothetical data set is analyzed, and common statistical and practical inferences are illustrated.The third research example uses the method of multiple regression analysis. This example introduces the concept of significant explainer variables, percentage of variance explained and prediction models. In each case, appropriate statistical and practical inferences are made from the hypothetical data.The three research examples distinguish between the important concepts of statistical inference and inferences of practical significance. These two key concepts are developed and discussed in some detail.

摘要:
This article discusses the basic characteristics of nonparametric statistical tests, contrasting them with the characteristics of parametric statistical tests. Examples for performing nonparametric statistical tests on practitioners' own data also are included.Nonparametric tests can be used with data that are of the nominative (e.g., characteristics such as right and left, male and female) and ordinal (e.g., mild, moderate and severe) levels of measurement, which may not follow the normal distribution curve or comply with other assumptions required of data analyzed by parametric statistical methods. However, the results of analyzing data with these nonparametric statistical methods can yield important information about the degree to which qualities of one group of data differ from those of another group.
