摘要:
The majority of transtibial (belowknee) amputations occur in elderly patients with systemic vascular disease. Rehabilitation efforts toward prosthetic ambulation are frequently delayed awaiting postoperative healing of the vascular compromised limb. Rehabilitation becomes more difficult, more costly and less successful the longer it is delayed after surgery, especially among elderly amputees. Early ambulation reduces the risk of complications such as thromboembolism, pneumonia and deconditioning in the older patient as well as enhances remaining life. An early coordinated post-amputation rehabilitation program reduces the time to prosthetic ambulation and the risk of further debility and failure among elderly amputees. The rigid removable dressing was incorporated into the program and found to be a safe method of residual limb shrinkage among elderly dysvascular transtibial amputees.

摘要:
The Lower-Extremity Orthosis (LEO) study (Evans, Gowland, et al.J is a randomized controlled trial assessing the efficacy of ankle-foot orthoses (AFOs) in treating preambulatory children with spastic diplegic cerebral palsy. The range of neuromuscular and skeletal presentations manifested by the cerebral palsy child created a challenge when attempting to standardize the orthotic prescription criteria and orthotic design for the study. The study was more difficult because of the differing approaches to cerebral palsy management and the variety of orthosis designs available. Consensus techniques were used to develop the standardized orthotic management protocol for the LEO study. Consensus methods have been used to define levels of agreement on controversial medical problems (1-3). A combination of the Delphi and Nominal Groups consensus methods was used to develop the orthotic management protocol for the LEO study. Current orthotic management techniques were collected through a survey sent to orthotists and therapists (CP Orthotic Management Survey), and this information was used to draft the protocol. Therapists, ortholists and researchers involved in the LEO study revised the draft. This article documents the development method of the orthotic management protocol as applied to orthotic management problems.

摘要:
The orthotic management of Familial Dysautonomia presents many challenges to the clinic team. The clinical manifestations of this disorder are many and cause accepted orthotic management regimes to be modified. This article presents the characteristics of this disorder as well as the orthotic management of the condition as prescribed at the Hospital for Special Surgery Familial Dysautonomia Clinic. Modifications to the standard TLSO and CTLSO designs that have shown promise will be detailed. Disorders accompanied by similar clinical features also may benefit from the orthotic changes described.

摘要:
A thermoplastic head immobilization system was designed and fabricated at the University of Virginia Medical Center. The department of neurosurgery and the department of prosthetics and orthotics produced a cost-effective head restraint device that is adaptable to a variety of imaging systems. This article highlights the issues involved in material selection, fabrication and the resulting application in MRI imaging studies.

ABSTRACTResearch is the acquisition of new information by scientific means. Research and development (R&D) is the exploration of that information to solve a problem, and product development is the process of making it a practical reality. These concepts are easily confused, particularly when one's attention is focused on new devices. Another element that must be considered is the technology transfer. Very real constraints exist for afield as small as prosthetics and orthotics that is rapidly adopting exotic new technology. These constraints involve issues of cost and information to guide manufacturing and utilization efforts. The O&P profession must weigh carefully issues involving the evaluation of new techniques and devices. O&P clinicians will also play an essential role in this area as well as in research, R&D and product development.
