The prevalence of 39 HLA antigens of series A, B and C was determined in 63 patients with Giant cell arteritis. In 44 patients the diagnosis was established by means of temporal artery biopsy and in 19 it was based on clinical criteria. The prevalence of the antigens was compared with that in two control groups of different ages. No significantly increased antigen prevalence was seen in our patients, as compared with controls. There were no differences in the prevalence of HLA antigens between groups of patients with different clinical forms of GCA. Nor was any difference in the prevalence of HLA antigens found between younger and older healthy individuals.

From 88 sera of patients with juvenile rheumatoid arthritis 30.6% showed antibodies to denatured type I collagen and 31.8% antibodies to type II collagen, a percentage which corresponds to radioimmunoassay results in adult RA. Rheumatoid factors were demonstrated with Waaler-Rose test in 14.7% and with latex test in 6.8% of investigated patients. Results of investigations with collagen types I and II correlated in 73.7%. Whilst type I collagen antibodies were found with equal frequency in active and non-active stages, type II antibodies were twice as frequent in active stages as in non-active stages. Sixteen sera of children with non-rheumatoid diseases had no collagen antibodies. JRA sera and controls differed with statistical significance in regard to collagen antibodies. 36 sera of children with Still's syndrome showed antibodies to type I collagen in 13.8% and antibodies to type II collagen in 33.3%. Both types of antibodies appeared more frequently in clinically active stages. The sera differed from controls with statistical significance in regard to collagen antibodies.

Arthritis of the sacro-iliac joints is a fairly common condition occurring either in isolation or as a part of pelvospondylitis. Because of the difficulty of establishing a diagnosis before X-ray changes have appeared, various radionuclide scanning techniques have been developed to make possible an early diagnosis. Opinions have differed as to the reliability of radionuclide methods using“bone seeking”radiopharmaceuticals. Patients with clinically and/or radiologically proven sacro-iliitis were imaged by means of99m-Tc-EHDP. An S.I. index was calculated as the ratio of the maximum activity of the sacro-iliac joints to the minimum activity of the iliac wings. The accuracy of the method was very good, and it is recommended as a supplement to clinical examination and X-ray studies.

Liver function was studied primarily by determination of serum gamma glutamyl transferase and alkaline phosphatase. In subsamples of patients the investigation was extended by determination of serum aminotransferases, isoenzyme analysis of alkaline phosphatase,99mtechnetium scintigraphy, and liver biopsy. In 182 in-patients with rheumatoid arthritis, the serum gamma glutamyl transferase level was elevated in 47%and serum alkaline phosphatase (of liver origin) in 24%. A concomitant increase in serum aminotransferases was found in 15% of patients with elevated gamma glutamyl transferase level. A closely similar pattern was found in 45 patients with non-rheumatoid arthritis (ankylosing spondylitis, psoriatic arthritis, reactive arthritis, and undefined arthritis), and in 5 patients with polymyalgia rheumatica. In 23 patients with non-rheumatic inflammation (pneumonia), liver dysfunction was common, though the pattern of serum enzyme changes was different. In rheumatoid arthritis, liver scanning showed irregular or low uptake, but biopsy only indicated reactive hepatitis. Hepatotoxicity could not be traced to any single drug or combination of drugs given. On the contrary, chloroquine appeared to reduce serum gamma glutamyl transferase, and corticosteroids had a similar effect on serum alkaline phosphatase. In patients not treated with corticosteroids, both serum gamma glutamyl transferase and alkaline phosphatase were weakly to moderately correlated with laboratory indices of disease activity (ESR and serum orosomucoid). The frequently occurring isolated increase of serum gamma glutamyi transferase and/or serum alkaline phosphatase in arthritis may be an un specific reaction to inflammation.

Nineteen patients with PHO with onset in childhood are presented. In 7 cases the syndrome was found in other family members too. In clinical, X-ray scintigraphic, arteriographic, and plethysmographic investigations, joint affection was found in 14 patients. Clubbing of the fingers and toes was seen in all the patients and a periosteal reaction was found in the long bones in 11 patients. Arteriography was carried out in 5 patients and showed hypervascularization and arteriovenous anastomoses in the clubbed fingers. The gas analysis of the arterial blood did not show any pathological changes. The histological finding showed poorly vascularized and oedematous periarticular tissue and ossification in the periosteal area, while in the synovial membrane, slight, nonspecific inflammatory changes were seen.

Twenty-five male and 25 female consecutive patients hospitalized because of psoriasis were examined. The clinical and radiographical findings are presented in detail elsewhere. Increased serum IgA, C 3, C 4, and C 3 PA concentrations were found in patients both with and without arthropathy and/or sacro-iliitis. Increased IgG concentrations were found in patients without arthropathy, and of CI inhibitor concentrations in patients with arthropathy and/or sacro-iliitis. IgG and IgA concentrations were lower in patients with arthropathy than in those without, the difference being most significant in patients with arthropathy of large joints. An association was found between increased C4 concentration and sacro-iliitis, increased CRP concentration and sacro-iliitis and increased C3 concentration and phalangeal joint arthropathy. C4 and CRP concentrations were not associated. A close association between CRP and SAA was observed. Our results indicate that psoriatic arthropathy is not a single uniform joint disease, but represents different forms of arthropathy.

Parameters of cell-mediated and humoral immunity were examined in 8 patients with clinically inactive SLE undergoing levamisole treatment. Lymphocyte transformation with PHA and PMW was temporarily increased in the first month of therapy; at the same time absolute numbers of peripheral blood lymphocytes, T cells and B cells increased slightly. However, these differences were not significant statistically. In 1 patient with a diminished percentage of T cells and an increased percentage of B cells, levamisole induced normalization. Antibodies to DNA decreased in 6 patients, while litres of ANA decreased only in one patient. Serum levels of Clq and C3 increased in 7 and 3 patients, respectively. Four patients were treated with levamisole for 12 months, during which time there was neither clinical exacerbation nor deterioration of renal function and the concomitant corticosteroid therapy could be reduced.

Fifty consecutive adult patients hospitalized for psoriasis were examined. Persistent low back pain, large joint arthropathy, phalangeal joint arthropathy and radiological sacro-iliitis were frequent findings (18–28%), but were not associated with one another. Chest expansion and spinal mobility were not reduced in the patients who had roentgenological sacro-iliitis. Low back pain at night and large joint arthropathy were positively associated. Peripheral arthropathy and roentgenological sacro-iliitis were associated. There was no sex difference in the frequency of signs or symptoms. Four patients (8%) had ankylosing spondylitis. It was concluded that in psoriatic patients, the different signs of joint affection are only partly related.

Immune complexes were detected by Clq-binding activity (Clq-BA) in 58% of 107 patients with seropositive rheumatoid arthritis (RA), and in 24% of 31 patients with seronegative arthritis. The difference is significant. Immune complexes were detected by the anticomplementarity test (AC) in approximately half of the patients without significant inter-group differences. No correlation was observed between the two methods. In 35 randomly selected patients with seropositive RA, a significant correlation was observed between Clq-BA and the functional class and the latex titre for rheumatoid factor. In patients with seronegative RA, the Clq-BA was significantly correlated to the number of joints with impaired mobility. The AC test was correlated to the erythrocyte sedimentation rate and the concentration of hemoglobin and gammaglobulin in patients with seropositive RA. As regards the clinical activity of disease, erythrocyte sedimentation rate and hemoglobin seem better parameters than the Clq-BA and AC tests.

摘要:
Indoprofen was given intravenously (bolus followed by infusion) and orally to 7 patients suffering from active rheumatoid arthritis with knee joint effusion. Indoprofen readily penetrated into synovial fluid in amounts which were directly correlated with areas under curves of plasma drug levels. Synovial fluid acted as a compartment distinct from the central (plasma) compartment. Indoprofen caused a substantial decrease in E2and F2αprostaglandin concentration in synovial fluid.
