Frequently there is a temperature elevation in children following operative procedures or trauma involving the long bones. A combined clinical and laboratory study investigated the possibility that this was secondary to blood resorption. A clinical study was conducted by reviewing the records of all children admitted to the hospital over a 10-year period with closed femoral or tibial fractures. All these patients demonstrated a temperature elevation to at least 101°F. In the laboratory study, rabbits of different ages were bled, and this blood was reinjected into their quadriceps muscle to create hematomas. Control rabbits were hemorrhaged and either were not reinjected with their blood or were injected intramuscularly with lactated Ringer's solution. Those rabbits that received the intramuscular blood injections developed significant temperature elevations, and the younger the rabbit the more pronounced the temperature rise.

The causative factor responsible for producing a femoral fracture was determined in 74 children 5 years of age or younger admitted to Oklahoma Children's Memorial Hospital over a 5-year period. Almost one-half of the fractures (34/74) were sustained as a result of suspected or confirmed abuse: most (17/26) of the fractures in infants less than 1 year of age were a result of abuse. Seven automobile-pedestrian accidents were causative in children 2 years or older. An accurate history, good record keeping, common sense, and awareness of the incidence of abuse will assist the orthopedist in recognizing abuse when it is likely, and sparing the family when it is not.

We conducted a radiographic retrospective study in children to determine spinal lordosis and kyphosis. A total of 218 lateral standing full-length radiographs were studied: 104 normal and 114 scoliotic patients. Normal X-ray films were defined as those obtained for suspected disorder (e.g., scoliosis) with no abnormalities detected by the radiologist or orthopedist. Thoracic kyphosis is defined as the angle between perpendiculars drawn from the inferior endplate of T5, and the superior endplate of T12. Lordosis is the angle between perpendiculars from the inferior endplate of L1and the superior endplate of L5. The angle between the inferior endplate of L5and the top of the sacrum is the L5–S1angle. Statistical analysis was undertaken to compare lordosis and kyphosis radiographic measurements with age, sex, height, and weight employing the SAS package program. No significant relationships were noted between degree of scoliosis, age, sex, height or weight, and kyphosis, lordosis, or L5–S1angle. Neither could correlations among the three radiographic measurements be made. Lordosis measured 40° (range 31–49.5°) in normal and 48.5° (range 40–55°) in scoliotic patients. Kyphosis measured 27° (range 21–33°) in normal and 28° (range 16.5–36°) in scoliotic patients.

Pulmonary function was correlated with patient age and degree of thoracic scoliosis in 25 patients with Duchenne muscular dystrophy in a retrospective, longitudinal study. The observed forced vital capacity (FVC) was found to peak at approximately the age when standing ceases, then to decline rapidly. Thoracolumbar curves were found to be insignificant in adversely affecting pulmonary function in patients with Duchenne muscular dystrophy. Percent FVC was found to be the parameter of pulmonary function that was most strongly correlated with age and scoliosis measurements. In addition, age and thoracic scoliosis together were better predictors of percent FVC than either one alone. Each 1 year of age had approximately the same negative influence on percent FVC that each 10° of thoracic scoliosis had; both decreased percent FVC by approximately 4%. A regression equation for percent FVC is presented which predicts that the patient who has had scoliosis progression halted by spinal fusion would, subsequent to the surgery, show a slower rate of decline of percent FVC and that this rate is quantifiable, predictable, and dependent solely on the patient's advancing age. Therefore, early spinal instrumentation and fusion is advocated in the patient with Duchenne muscular dystrophy.

Six hundred infant hips were examined with ultrasonography and the findings compared to radiography. Twelve hips were dislocated and could be recognized as such on the sonograms. The development of the acetabular roof can be determined through assessment of the preformed, nonosseous acetabular rim. Consequently, dysplasia of the hip can be recognized early. Ultrasonography does not replace clinical investigation, but it does reduce the need for radiography, reducing the exposure of the child to radiation.

摘要:
Patients with nail-patella syndrome have deformities of the elbows and knees which can cause bothersome symptoms requiring treatment. We have found in a small series of four patients that quadricepsplasties have been uniformly successful in treating recurrent subluxation or dislocation of the patella at 10-year follow-up. Radial head resection has been effective in relieving minor amounts of pain and eliminating the bump caused by the posteriorly dislocated head. The range of motion of the elbow and forearm has not been improved.

Two cases of “whistling face syndrome” with the typical manifestations in the face, hands, and feet in a father and son are presented. From studying them over a 3-year period and from reviewing 46 reported cases, we conclude that (a) hand and foot deformities do not improve spontaneously with growth and better results are achieved by early surgery, (b) decreased intermaxillary distance contributed more than intercommissural distance to feeding and anesthetic difficulties in our patients, and (c) thickened and contracted joint capsules and myopathic changes are thought to form the basic pathology in this syndrome.

Two cases of osteoblastoma of a rib associated with progressive scoliosis are reported. The unusual localization of the neoplastic lesions and their close relationship to the lateral deviation of the spine suggest an etiologic basis for these patients' scoliosis. The need for early diagnosis and surgical excision of the rib lesion in the treatment of scoliosis resulting from osteoblastoma of a rib is stressed.

Five children with popliteal webbing and flexion contractures of the knees are described. When their associated anatomical abnormalities are considered, classification into two distinct syndromes, multiple pterygium and popliteal pterygium, is possible and of importance in genetic counselling. Soft tissue surgical corrections of the flexion contractures were undertaken in each case. The operative technique, local abnormalities, and orthopaedic management are discussed.

Seventeen Muslim children of central Calcutta with postinjection fibrosis of the deltoid and abduction deformity are presented. The abduction deformity developed 3 to 7 years after receiving the first injection. Winging of the scapula and inability to bring the arm by the side of the body were the presenting symptoms in most cases. Excision of the main fibrotic band, which generally occurred in the intermediate fibres of the deltoid, along with incision of the thinner fibrotic bands when present, resulted in satisfactory correction of the deformity in 93.5% of the cases. The power in the deltoid remained Medical Research Council grade 5 in spite of loss of muscle bulk due to fibrosis.
