Twenty-three children with fractures of the radial neck were reviewed. Fractures with an angulation of >60° were treated by open reduction and internal fixation with transarticular Kirschner wires through the humerus and the radius. These broke at the joint in two patients, and we now avoid the humerus by using oblique Kirschner wires. Some limitation of movement, especially of forearm rotation, was more common in children with associated lesions of the same elbow and with open reduction. We think that this reflects the severity of the injury rather than the presence of associated lesions or the surgery itself.

Fracture of the forearm with ipsilateral injury of the elbow joint is a rare traumatic entity in children. Twenty-four such cases were treated in our clinic during the last 4 years (1981–1984). The treatment was surgical in six cases and related to the elbow injuries, while the forearm underwent closed treatment. The results of these cases, regardless of treatment and severity of the injury, did not differ appreciably from the results of common forearm or elbow injuries.

Stump overgrowth requiring surgical revisions occurred in 58 of 1,015 (5.7%) skeletally immature patients with congenital and acquired amputations. Acquired below-knee amputation was the most common type needing revision (18.5%), and the congenital group not requiring a conversion had the least overgrowth problem (1.7%). When the primary amputation occurred after the age of 12 years or when disarticulation was carried out, revisions were unnecessary. The younger the patient, the greater the incidence of repeated revisions. Study of the histological changes in eight juvenile congenital and acquired amputees revealed two patterns: (a) reactive connective tissue stratification and (b) a bursa.

Transepiphyseal plate osteotomy of the medial tibial condyle was performed in three children (four knees) with severe progressive tibia vara and excessive ligamentous laxity. Two knees with Blount disease underwent tibial valgus osteotomy and epiphysiodesis associated with transepiphyseal plate osteotomy, and two knees with pseudoachondroplasia underwent tibial valgus osteotomy. Follow-up study at the time of skeletal maturity revealed satisfactory alignment of the leg, favorable congruity of the joint, and good ligamentous stability in all cases. Our experience indicates that the present procedures are excellent, although there are very few documented cases and almost no description of the postoperative clinical courses.

A method is described for early spica cast application in treatment of pediatric femoral shaft fractures whereby the variables are reduced and the cast is applied in stages. The fracture reduction is carefully planned, and molds are checked before final casting.

Two cases of Kawasaki disease (mucocutaneous lymph node syndrome) with peripheral vasculitis of the extremities are described. Ischemia began 2–3 weeks after the onset of symptoms and was present for >24 h before hospitalization. Despite supportive therapy, gangrene of the extremities resulted in amputation. The vasculitis causes inflammation and occlusion of vessels, and therefore therapy aimed at arterial dilatation may not be beneficial. Early recognition of ischemia and treatment with correction of hypovolemia, anticoagulation, and hyperbaric therapy may be useful. Steroids may decrease peripheral vasculitis, but there is also an associated increased risk of coronary aneurysms.

Protrusio acetabuli may be characterized by an inward displacement of the femoral head into the pelvis. A 20-year-old patient with trisomy 18, who is also notable for her longevity, developed bilateral protrusion over a course of 2 years. We believe that trisomy 18 should be considered a cause of protrusio.
