To determine the relative trochanteric overgrowth after ischemic necrosis of the femoral head during treatment of congenital dislocation of the hip (CDH), we reviewed radiographs of patients with ischemic necrosis and no femoral side surgery. The articulotrochanteric distance (ATD) was recorded after physeal closure. Ischemic necrosis was classified with the systems proposed by Salter et al. and Kalamchi and MacEwen. The resultant overgrowth was correlated with these systems. The articulotrochanteric distance in 29 patients with a Trendelenburg gait was also measured. The Kalamchi classification is more useful for predicting relative trochanteric overgrowth. Children with an ATD of ≤ 0 mm are likely to have a Trendelenburg gait.

A prospective study involving 62 newborns diagnosed as having subluxable or dislocatable hips has established the efficacy of treating such cases from birth. Patients were treated first with a simple abduction splint, leading from a plastic abduction brace to the Pavlik harness and finally, a hip spica. Routine clinical examination showed this to be necessary until hip stability was achieved. This approach, together with continuing, detailed, clinical examinations, produced a 100% success rate with no iatrogenic complications.

Passive range of motion (ROM) of seven hip joint movements were measured in 60 healthy, full-term neonates to determine 95% normal ranges. All neonates lacked full hip extension, and lateral rotation was greater than medial rotation. No significant differences in hip ROM were found between males and females or between Hispanic and Caucasian neonates. Pearson correlation coefficients were calculated. Positive correlations, although not strong, were found between birth weight and birth length, abduction in flexion and medial rotation, and abduction in flexion and lateral rotation. Negative correlations, although weak, were found between birth weight and medial rotation, adduction and ending flexion, adduction and lateral rotation, and adduction and abduction in flexion. The methodology is comprehensive and detailed, and provides a procedural basis for future developmental data.

The purpose of this study was to establish a relationship between the magnitude of femoral anteversion and osteoarthritis of the hip. Sixteen osteoarthritic subjects were compared with 18 control subjects by an improved computed tomography (CT) scanning technique. Femoral anteversion was accurately determined with little radiation exposure. There was no significant difference in femoral anteversion between arthritic subjects and a “normal” control population. There was no significant difference in femoral anteversion between the involved and the uninvolved hips of these subjects with osteoarthritis. These results suggest that performing a derotational femoral osteotomy to prevent osteoarthritis is not indicated. The cause of primary osteoarthritis is multifactorial and does not depend solely on preexistence of higher femoral anteversion.

Fourteen patients (16 hips) with idiopathic chondrolysis of the hip were retrospectively studied at the Shriners Hospital from 1973 to 1986. Follow up averaged 84 months (range 13–180 months). All 14 patients were female. All but one were premenstrual. Each patient presented with an insidious onset of pain in the hip, thigh, or knee and radiographic symmetrical joint space narrowing. Three of the 16 hips required fusions, and only five regained a full range of motion. Partial restoration of the joint space occurred in eight hips. Essential treatment consists of unloading the hip joint while maintaining motion.

Fifteen patients (18 hips) whose hips underwent subcapital wedge osteotomy for severe chronic slipped capital femoral epiphysis were studied. The results of 14 patients were excellent, and the patients themselves believed the results of the operation to be excellent. One patient had both avascular necrosis and cartilage necrosis. The postoperative follow-up period ranged from 2 to 17 years (average 10 years 3 months).

An easily learned and applied method of accurately determining the blind zone of the femoral head is presented. This method is based on a geometric analysis of individual pins relative to the subchondral plate as apparent on mutually perpendicular roentgenograms. This method can be used with fluoroscopy or plain radiographs for intraoperative or postoperative evaluation.

Vibratory studies were performed on 10 adolescent subjects being followed for congenital scoliosis. The average age was 12 years 4 months and the average curve was 32°. Twenty-one age-matched controls underwent identical studies. Threshold to detection of vibratory stimuli in the upper and lower extremities was quantified. Results showed that the scoliotic subjects were consistently less sensitive than the controls in the lower but not the upper extremity. The occurrence of hyposensitivity in only the lower extremity of persons with congenital scoliosis implies that the sensory deficit is secondary to the spinal deformity itself rather than a primary etiologic factor.

Nine patients with multiple hereditary osteochondromata underwent ankle surgery for valgus deformity. The indications for operation included pain from trauma of the prominent masses, pain in the ankle joint associated with the deformity, limited ankle motion, and undesirable cosmesis. The procedures included excision of osteochondromata, fibular lengthening, and medial tibial hemiepiphyseal retardation by inserting staples. Excision of osteochondromata as an isolated procedure relieved pain and improved cosmesis but did not alter the tibiofibular length discrepancy or the ankle valgus. Lengthening of the fibula and medial tibial hemiepiphyseal stapling, alone or in combination, corrected the valgus deformity of the ankle. At final follow-up (mean 43 months), no patient had pain or functional impairment.

Twenty-two patients with severe pes cavovarus secondary to Charcot-Marie-Tooth disease who underwent triple arthrodesis were retrospectively reviewed with an average follow-up period of 12 years, 4 months. Although only 32% of the patients had good objective results, 88% had good or excellent function and 86% were satisfied with their result. Radiographic follow-up averaged 10 years, 4 months. Talonavicular pseudarthroses were present in 15% of the feet and were symptomatic in all but one foot. Twenty-four percent of the ankles and 62% of the feet demonstrated radiographic evidence of degenerative joint disease.
