摘要:
ABSTRACTVideo‐assisted thoraco‐scopic surgery is a new approach that offers an alternative to open surgical ligation of the patent ductus arteriosus. Advanced video technology, precise pediatric instrumentation, and innovative surgical techniques have facilitated the development of this approach. From April 1993 to February 1995, this procedure was performed at a large pediatric medical center on more than 100 pediatric patients with a wide range of ages and weights. There was a collaborative effort among all disciplines, and peri‐operative nurses on the cardiac OR team were challenged to become proficient with this new procedure through inservice sessions, ongoing education, and hands‐on experience.AORN J64 (Oct 1996) 526‐535.

ABSTRACTMoyamoya syndrome is a chronic cerebrovascular disorder that is characterized by a progressive narrowing of the intracranial internal carotid arteries and their main branches and a compensatory dilation of smaller arteries (ie, moyamoya vessels) at the base of the brain. Common signs and symptoms in children include recurrent episodes of cerebral ischemia, transient hemiparesis, seizures, and cerebrovascular accidents (CVAs). The diagnosis is made primarily by cerebral angiography, and surgical treatment consists of pial synangiosis, whereby surgeons suture the superficial temporal artery to the pia mater under microscopic visualization. The goal of surgical intervention is to promote the development of collateral circulation to the brain and decrease the risk of CVAs, transient ischemic attacks, and seizures. Follow‐up angiography demonstrates that pial synangiosis results in excellent postoperative collaterization of ischemic areas of the brain.AORN J(Oct 1996) 540‐554.

ABSTRACTCochlear implant procedures are available for children who are diagnosed with severe hearing loss. Cochlear implants can restore children's perceptions of sound through the use of electronic devices. Perioperative nurses should be knowledgeable about all surgical aspects of cochlear implant procedures and be aware of the months of preparation before surgery and the lengthy rehabilitation afterward. This article discusses the historical development of the cochlear implant procedures, describes the hearing process, describes candidate selection, and discusses the perioperative care for a child receiving a cochlear implant in a case study presentation.AORN J64 (Oct 1996) 561–571.

ABSTRACTChildren with significant baby bottle tooth decay and children who are challenged by significant physical and emotional limitations are candidates for dental rehabilitation procedures with general anesthesia. Rehabilitation of children's primary teeth is important to prevent pain, infection, and tooth loss. To provide skilled care for children undergoing dental rehabilitation procedures, perioperative nurses must understand normal dentition, tooth anatomy, and caries prevention and formation and be able to meet children's developmental and emotional needs. Perioperative nurses also must collaborate with dentists to maintain appropriate supply inventories that are unique to dental rehabilitation procedures.AORN J64 (Oct 1996) 573–579.
