&NA;A case of thyrotoxicosis associated with hyperemesis gravidarum is described. In pregnancy, thyroid hormone economy is modified by 1) an estrogen dependent increase in the concentration of thyroxine binding globulin and bound thyroxine, 2) increased net thyroid hormone utilization, and 3) an increase in TSH independent thyroid hormone secretion. The last of these is due to stimulation of the thyroid TSH receptor by human chorionic gonadotropin (hCG). The thyrotoxicosis of hyperemesis gravidarum results from excess thyroid stimulating activity of hCG and is therefore limited to the first trimester, during which peak hCG levels occur. This condition should be distinguished from Graves' disease, which is likely to persist. Low‐dose antithyroid therapy is appropriate if patients are symptomatic and can be stopped during the second trimester.
