AbstractFor epithelial cancers which account for the majority of human malignancies chemoprevention by definition, consists of intervention measures taken during the develop ment of the neoplastic process up to the stage of in situ carcinoma. When considering chemopreventive strategies, at least two phases of the epithelial neoplastic process must conceptually be distinguished. Firstly, the initation and early reversible promotion phase, secondly, the later more autonomous phase of tumor development. We suggest that these distinctions are of crucial importance as the effective dose of chemopreventive agents may differ, depending on the stage of tumor formation. Failure to take these considerations into account could result in negative or inconclusive chemoprevention trials.

AbstractThe Milan Cancer Institute has been involved in clinical research on breast cancer for the last 25 years, and although the previous studies have been mainly concerned with trials aimed at refining the chemotherapy, radiotherapy, and surgical procedures, in recent years we have been progressively interested in detection and prevention programs. In this latter context, we have developed a special interest in research in chemoprevention with retinoids.

AbstractThe year 1987 has been marked by many things besides the Iran Scam hearings, First Family surgery, and the stock market crash. There has been a tremendous information explosion in all fields, especially in the field of health and specifically preventive medicine. One no longer asks one's doctor what to eat to prevent diseases because one can read about it in Wednesday's food section, Sunday's magazine, or in womens' magazines, or you can send for or pick up an NCI or an ACS booklet in any doctor's office. Terms like saturated fats, dietary fibers, HDLs, omega-3 fatty acids, and selenium, are public domain terminology, reflecting the fest-moving changes in all areas of medicine and the public's hunger to be informed. This is the yield of consumerism for which the American appetite is endless.
