AbstractEighteen children born to parents who had previously received chemotherapy or radiotherapy were examined for physical health, growth, and development. The immunologic and the hematologic status of these children was also evaluated. Their ages ranged from birth to 15 years. The children had a careful history and physical examination to detect any abnormal symptoms or signs, and the parent's previous treatment was carefully documented. Four sets of parents had children while one of the parents was on active treatment (2 male and 2 female). Of the male patients, one patient's wife had a baby that was“small for gestational age”at birth and had transient failure to thrive; the other child was normal. Of the female patients, one offspring was small for gestational age at birth and the other was normal, but both continued to have failure to thrive for up to 17 months and 26 months, respectively. Ten parents procreated after being treated with chemotherapy and/or radiotherapy, to whom 14 children were born. One child was a stillbirth with multiple congenital abnormalities, and another child had trisomy 13–15 and died 6 months later. The other 12 children were normal at birth, but one child is under the 5th percentile for growth at twelve months of age. In all children studied, immune function test, complete blood count, and viral tilers were considered normal for age. In our study, we found that three out of four children born to parents who were on chemotherapy had failure to thrive. Of the 14 children born to parents who conceived after being off chemotherapy, 11 were found to be normal in growth and development. These results imply that there is a high risk of complications in children born to parents who procreated while receiving chemotherapy. Further studies are needed to develop better guidelines for counseling cancer patients who want to have children.

AbstractIn vivo and in vitro observations of cellular immune response in 70 patients with squamous cell cancer of the oral cavity and in 40 age-matched normal controls, were made using delayed hypersensitivity responses to DNCB, PPD, andCandida albicansextract (Dermatophytin‘0’), absolute lymphocyte counts, absolute T-cell numbers, and PHA-induced lymphocyte blastogenesis reactions as parameters. The results were correlated with clinical stage, tumor size, lymph node involvement, tumor differentiation, lymphoreticular responses, and outcome during a one-year follow-up period. A significant degree of impairment of both in vivo and in vitro parameters was found in oral cancer patients compared to normal control. The impairment was more prominent in advanced stages. Lymph node involvement was associated with impaired dermal hypersensitivity to recall antigens as well as a reduced T-cell population and blastogenic response. Only delayed hypersensitivity response to DNCB, PPD, andCandidashowed a correlation with histologic features such as tumor differentiation and lymphoreticular response. Although absolute lymphocyte counts and T-cell population were reduced in the primary stage of the disease, the functional capacity of isolated lymphocytes to undergo blast formation was retained. PHA-induced lymphocyte blastogenesis showed a significant impairment only when the tumor was well established and disseminated beyond its local confines. Delayed hypersensitivity responses to DNCB, higher T-cell counts, and blastogenic indices were associated with recurrence-free survival. Immunologic parameters provide prognostic information beyond the clinical stage of the disease. Therefore, it seems that a multiparametric in vivo and in vitro observation of cellular immune response may be useful as an indicator of clinical course and prognosis of patients with squamous cell cancer of the oral cavity.

AbstractPrevious reports have indicated that human urothelial cancer is more sensitive to combination radiation-cisplatin than to either treatment given alone and that the treatment schedule may be an important factor in determining the magnitude of tumor response. In the present study we have investigated the effect of timing and the sequence of combination“radiation-cisplatin”on human urothelial cancer grown in nude mice. Tumors were exposed to 1000 rod x-irradiation and received cisplatin treatment in the amount of 5 mg/kg once on each specified day before or after radiation. Best tumor responses, characterized by significant tumor growth delay and tumor regression, were observed when cisplatin was given on Day 3 and 6 postradiation. The present work has indicated that timing and sequence of treatments in the combination“radiation-cisplatin”are important factors in determining tumor response and may be of assistance in formulating the most effective clinical trials of urothelial cancer patients.

AbstractPreviously published results of short-term tests (STT) applied to chemicals, identified as carcinogens and noncarcinogens, are compared and discussed. Inspection of the data shows that carcinogens and noncarcinogens are about equally STT-positive. Although it may be possible to observe suggestive indications in the data, it is difficult to be certain that these indications are necessarily relevant to or predictive of human carcinogenic risk. The assumption that STT are predictive, if incorrect and accepted, would seem to have the potential for causing harm to public health by leading to falsely recognized carcinogens as noncarcinogens and noncarcinogens as carcinogens. Technological and legal preparations have been made for rapid regulatory decisions on the basis of STT. The data, however, appear to caution against utilizating these means too quickly.
