SummarySerial determinations of the numbers of peripheral blood T‐lymphyocyte subpopulations (T‐helper/inducer and T‐suppressor/cytotoxic cells) were made using monoclonal antibodies in 12 patients with haemophilia who were HIV antibody positive. Eleven patients were clinically severe and one clinically moderate. Follow‐up studies for 12–42 months in nine patients showed persisting abnormalities in five patients, abnormalities which developed in three patients who were initially normal, and a return to normal findings in one patient who was initially abnormal. The remaining three patients were followed up for 3–4 months; two showed persisting abnormalities and one a return to normal. Persistent generalized lymphadenopathy was found in one patient who showed raised levels of T‐suppressor cells initially and low levels of T‐helper cells in the follow‐up, but low T‐helper cells persisting for up to 12 months were not necessarily asso

SummaryUpper reference limits for packed cell volume (PCV) for a hospital population were established by analysis of 14000 consecutive blood count profiles. These were 0.50 for men and 0.45 for women. In 86 patients, pseudopolycythaemia was identified by an increased PCV with red cell mass (RCM)<125% of predicted normal. We have proposed a modified classification of pseudopolycythaemia and identified three subsets on the basis of RCM and plasma volume (PV) values. Although all patients had raised PCV at the time of referral, by the time of blood volume study a lower PCV value was noted in a proportion of patients. Patients with a low PV (<80% predicted normal, Group 1) and those with a ‘high normal red cell mass’ (RCM 113–125% predicted normal, Group 2) had higher and more persistently elevated PCV values on repeated measurement, compared with the patients in Group 3 (‘transient pseudopolycythaemia’). These latter patients have a normal RCM and PV, and although initially fulfilling the criteria for pseudopolycythaemia on the basis of a raised PCV and normal RCM, a high proportion subsequently showed fluctuation in PCV which often fell below the upper reference limit. It is important to diagnose pseudopolycythaemia because of the associated increase in morbidity and mortality, and our observations suggest that whereas a blood volume study is essential to diagnose pseudopolycythaemia reliably, this investigation need only be performed when a patient shows a persistently increased PCV on repeated me

SummaryEight patients with myeloproliferative disorders, five with polycythaemia rubra vera (PRV) and three with essential thrombocythaemia (ET), have been treated with the anti‐folate drug Pyrimethamine for periods ranging from 1 to 24 years. In PRV this treatment was comparable in efficacy to that achieved with Busulphan or radioactive phosphorus, but required more frequent supervision. One patient was controlled on Pyrimethamine, having failed on conventional treatment. The major side effect was thrombocytopenia which was rapidly reversible on stopping the drug. In ET, Pyrimethamine produced satisfactory control of the platelet count and thrombocytopenia did not arise. No neurological sequelae were encountered. One patient developed a non‐Hodgkin's lymphoma of the gut, but there were no other cases of secondary malignancy. SummaryA solid‐phase, one‐step radioimmunoassay was developed for the determination of plasma lactoferrin concentration. The detection limit of the assay is 150 μg/l. Leakage of cellular lactoferrin was minimal when EDTA was used as anticoagulant, while results obtained from serum and from heparinized plasma were not reproducible. The plasma lactoferrin concentration of 35 female and 44 male healthy adults was measured in order to determine normal values. The geometric mean of lactoferrin levels in men is about 10% higher than in women: 483 (200–1500) μg/l in men and 446 (200–870) μg/l in women. Patients with acute and chronic leukaemias were also studied. In 38 patients with chronic myeloid leukaemia plasma lactoferrin levels were increased by three times while the neutrophil count was ten times higher than normal. Normal lactoferrin concentrations were measured in plasma samples from 15 patients with chronic lymphocytic leukaemia in incomplete remission while no detectable lactoferrin was found in samples from those in relapse (10 patients). In the untreated patients or those in relapse (19 cases) of both acute lymphocytic and myeloid leukaemias, plasma lactoferrin concentrations were undetectable while they seemed to return to normal during remission (3 cases). SummaryIn 1985, serum samples were distributed to 27 laboratories within the UK and Eire with the objective of determining the current state of anti‐platelet antibody detection. The sera included five unknown samples for blind assessment and four well‐characterized reference reagents. The laboratories involved used a wide range of different methods but the most commonly used techniques were the fluorescent antiglobulin test and the enzyme‐linked immunosorbent assay. In the trial, all responding laboratories were able to detect potent anti‐platelet alloantibodies but there was a 43% incidence of ‘false positive’ results with samples which had been designated as inert. SummaryAn evaluation of a Coulter Counter Model T660 has been carried out. This instrument is a compact, fully automated, blood cell counter which measures WBC, RBC, Hb, haematocrit, MCV and platelet count on a 100 μl sample of whole blood. The instrument was assessed during one month of regular use. One possible microbiological hazard was detected, which could be corrected by an instrument modification. No mechanical hazards were identified. The instrument as supplied produced some inaccurate WBCs; this malfunction appeared to be consequent on a design fault relating to the burn circuit of the white cell aperture and following instrument modification performance was considerably improved. Results produced were broadly comparable with those produced by a Coulter Counter Model S Plus IV. Precision was satisfactory. Deviations from linearity on dilution were not sufficient to be of practical importance. Carryover was satisfactorily low. Time‐lapse of up to 48 h did not introduce inaccuracy of sufficient magnitude to be of practical importance. Throughput varied from 52 to 63 specimens/h. SummaryThe automated peripheral blood differential counts produced by the Hemalog D, H6000 and Coulter S Plus IV were analysed and compared in 30 cases of chronic lymphocytic leukaemia (CLL) and 12 cases of hairy‐cell leukaemia. All three machines were reliable in identifying CLL and HCL as a ‘lymphocytosis’ and in estimating neutrophil numbers in the disease. The Hemalog D and H6000 produced accurate monocyte counts in the two diseases, but the Coulter S Plus IV was unreliable in this regard since large lymphoid cells were identified as monocytes. The Haemalog D and H6000 were comparable in consistently identifying modestly raised large unstained cells (LUCs) in CLL. SummaryWe describe a modified Phoenix data processing system which has been introduced over a three year period into the Leicestershire Haematology and Blood Transfusion Services. The system is unique, employing two central processing units, which serve six Coulter S Plus IVD Counters and a Coulter S880, three of the analysers being located in satellite haematology laboratories. Details are provided of the system design and experience with the data processing activities within haematology. The blood transfusion laboratories utilize the same computer and this will be the subject of a later report.
