摘要:
The provision of behavior management training for parents and families of children with behavioral and developmental disabilities is a nursing role. Two case studies show the potential usefulness of a newly developed multimedia approach to family training. The features of the innovative approach includea) individualized instruction, extensively utilizing multimedia, based on a thorough evaluation of the patient, family members, and their interaction;b) practical implementation of basic behavioral management principles and the patient's specific behavior management plan;c) healthy familial interactions where family members are able to recognize and meet each other's needs while simultaneously maintaining their own self-esteem; andd) assistance for the family in establishing community support and services.Social interactional theory provides a framework for the above approach to family training. That is, the persistence of effects of behavioral management is a function of how others react to changes in a target child: Unless improvements in behavior produced some alterations in the family's reactions to the child, the effects would be short lived. For this reason, rather than treating the child alone, the parents and other members of the family are trained to manage their own behaviors as they relate to the child's behavior. Indeed, a family and community-centered approach maximizes the effectiveness of care rendered to this vulnerable population.

A program was set up to provide developmental stimulation and health services to siblings of children already in placement owing to neglect, abuse, or other family difficulties. Several previous studies have confirmed the positive effects of this intervention with young children. The program was planned by an interdisciplinary team of a nurse, a psychologist, and a psychiatrist.Nursing students visited the homes of families weekly to provide developmental stimulation, health, and parent education. They determined the best way to meet the needs of the parents to enable them to interact with their child, and determined the developmental level of the child and the stimulation that would be appropriate. Nursing students also met with the same interdisciplinary team to discuss families, determine other services needed, and plan for the future.Evaluation of the family's participation in the project was done by the nursing student and the family. Most families remained in the project for several years. The project provided a long-term learning experience in child health nursing as well as an opportunity for students to work with other disciplines, and use such resources as a toy library. Clinical situations provide examples of the difficulties and the success of the project.

This naturalistic study was done to determine whether or not the graduate curriculum was adequately preparing students to function in the advanced nursing role. This article focuses on the advanced nursing practice of four master's-prepared child health nurses. Vignettes of this practice are highlighted and they are assessed using the domains of nursing practice that were described by Benner (1).The entire study, involving 30 master's-prepared nurses, determined that additional attention in the graduate program needs to be placed on the following areas: development of collegial relationships with physicians and other health care providers, development of successful strategies for influencing bureaucracies to respond to patients' and families' needs, articulation and validation of the value of the role of the advanced nurse practitioner, and development of the formal and informal consultation function.

Over the past five years, acquired immunodeficiency syndrome (AIDS) has been documented among homosexuals, intravenous drug users, and hemophiliacs. Another group is now emerging from the pediatric population. The signs and symptoms are markedly similar to those found in the adult AIDS patient. For the pediatric nurse, this syndrome presents challenges that demand thoughtful care.

This study, based on the previous work of Maslach and Pines explores the relationship between personal and job-related variables and burnout for pediatric nurses employed in acute-care settings. Personal variables studied were marital status, level of education, age, and family responsibilities. Job-related variables were employment status, shift assignments, patient care overload, years in position, years of nursing experience, and overtime.The study was conducted in 13 acute-care pediatric units of general hospitals in the Queens-Nassau-Suffolk areas of New York. The sample was 74 female registered nurses. Principal measuring devices utilized were 1) the Tedium Measure; 2) a data sheet; and 3) two open-ended questions for reporting job-related sources of stress and satisfaction. Descriptive statistics were used in analyzing the personal and job-related vriables on the data sheet, responses to the Tedium Measure, and responses to the open-ended questions. Correlation coefficients were obtained between personal and job-related variables and the Tedium Measure.Based on the Tedium Measure, nurses showed evidence of burnout. Only one variable (Percentage of Children on Unit With Social/Behavioral Problems) was significantly correlated with burnout. Thus a generalized approach to stress reduction as an aid in the prevention of burnout appears appropriate. This study and the emerging documentation of other researchers strongly suggests that it is essential to examine the need toincreasepositive factors to reduce stress and associated burnout. Suggestions are offered to increase the positive factors which the pediatric staff nurses stated were their greatest satisfactions such as time to give family-oriented care, patient teaching, and emotional support to children and parents.
