This article briefly outlines the anxiety that parents harbor when their child is diagnosed with Apnea. The authors discuss the nursing implications that are necessary to help the parents adjust to and cope with the illness. Education and support is stressed for the family.

Continuity of care is dependent on effective patterns of communication. Currently useful patterns of communications are nonexistent between the nursing staffs of tertiary medical centers and community hospitals. Nurses in community hospitals have difficulty obtaining discharge data regarding a child's admission to the tertiary center. In order to promote continuity of care between health agencies, Continued Help in Lending Direction (C.H.I.L.D.) has been proposed to assure a statewide telecommunication system between nurses at tertiary centers and community hospitals. Through the use of the hot-line, nurses in outlying hospitals will immediately be able to obtain information from the pediatric tertiary center if a child is later seen in the emergency room or admitted to the community hospital.This article proposes a communication network that would allow nurses working in hospitals throughout a state or region to have direct contact with a pediatric medical center through the simple process of dialing the telephone. The name of the proposed communication network is C.H.I.L.D. which stands for Continued Help in Lending Direction. The service would be available 24 hours a day. Information communicated through the system would allow nurses in community hospitals access to information which is needed to plan care for children with complex needs in the community. Information currently available on written referrals that have been sent to other health professionals wouldnolonger be lost or unavailable to nursing staff. The results of the initiation of this type of service include improved child care and a better informed nursing staff. While the continuity of care will be improved, it is hoped, the satisfaction of the family and the child with the health care delivery system will increase also. In addition, it should decrease unnecessary admissions and prevent complications related to planning which is based on inadequate data. Job satisfaction of nurses may increase as well.

The second part of a two-phase clinical research project was undetaken to implement a reliable, valid, and easily administered instrument for measuring parental participation in the care of the hospitalized child. Eleven parental participation activities were identified and validated in the first part of the project through exploratory research with parents of hospitalized children, previous research, and an expert panel. In the second part of the project, which is the focus of this report members of an eight-member task force were used as consultants for development of the instrument and for planning inservices. Two videotaped vignettes of a nurse interviewing a mother during admission of her child to a pediatric orthopedic setting formed the basis of the inservices. The first vignette familiarized the nursing staff (N =31) with the instrument. The second vignette was used to measure the inter-rater reliabiality of the staff through completion of the instrument after viewing the tape. Eighty percent agreement was obtained with criteria established for rating the second vignette with the instrument which attests to its potential as a reliablc method of measuring parental participation.

摘要:
Hospitalization for the school-age child (age 6–12) is a time of crisis that can impact achievement of normal developmental tasks as well as self-esteem. Many stressors are inherent in the hospital experience of the child and include concern about physical harm, separation, the unknown, and loss of autonomy. Art, an acceptable form of play or fantasy for the school-age child, can assist him to cope with hospitalization. An autobriographical scrapbook is one coping strategy that not only nurtures creativity in intellectual and physical development but also serves as a ventilatory mechanism. Participating with the child in creating the scrapbook allows the nurse to assess perceptions and adjustment of the child to the hospital experience. In assisting the child, the nurse gives him a positive feeling about nurses and the entire hospitalization experience.

The attitudes toward mothering of 78 primiparas were assessed when their infants were one month, six months, and twelve months of age, using a modification of Roth's Mother-Child Relationship Evaluation (MCRE). Changes in attitudes toward mothering during the infant's first year and relationships between subjects' attitudes toward mothering and their perceptions of their own mothers' mothering were examined. Differences in attitudes toward mothering were also examined considering selected sociodemographic variables. There was a significant positive correlation between subjects' attitudes toward mothering on Acceptance, Overindulgence, and Overprotection, with their perceptions of their own mothers' attitudes toward mothering. Attitudes toward mothering did not change significantly during the infant's first year. Scale scores for Acceptance were negatively correlated with scores on Overindulgence, Overprotection, and Rejection. Mothers who were referred for infant care or parenting skills scored significantly higher on the Rejection scale than those unreferred mothers. Unmarried mothers scored significantly higher on Over-protection and Rejection scales and lower on the Acceptance scale than married mothers.

Infant colic is a little understood phenomenon which has dramatic clinical effects upon the family. This article reports the findings from a Grounded Theory qualitative study that investigated the question of what processes are associated with infant colic in the family. The sample consisted of 50 parents, nurses, and physicians. A Parenting Banking Theory was developed from the data as a metaphor for the social situation that occurs when infant colic behaviors present, A summary concludes with projected areas of need for nursing research, practice, and education for nursing care of families whose infants experience colic.The family nurse and physician are the primary contacts for parents of infants with colic symptoms. This paper presents a theory, based on open-ended interview data, that describes and predicts the family dynamics resulting from the colic experience and hypothesizes interventions. Testing of the theory, especially in the area of intervention and outcomes, is needed. Such testing would serve to quantify the elusive nature of infant colic.
