A study was to determine if children with a meningomyelocele are at risk for having a lowered self-concept. Study participants included 19 children between the ages of 8 and 17 years who have a meningomyelocele and 30 children 8 to 17 years old without a meningomyelocele who were siblings of the children with a meningomyelocele. Self-concept was measured using the Piers-Harris Children's Self-Concept Scale, which the children completed in thier homes.One-way analysis of variance was applied to the self-concept total score and six cluster scores. No significant differences between groups were demonstrated for total self-concept. A significant difference (p≤.05) was found between groups for the anxiety self-concept cluster. There were no significant differences between preadolescent and adolescent groups of children with a meningomyelocele. When degree of disability was examined, the children who wore long leg braces showed a trend for lower anxiety and intellectual/school status cluster scores (p≤. 05).The study results indicate that children with a meningomyelocele did have some lower self-concept scores and that the degree of disability influenced certain aspects of self-concept.

Based on interview data from 56 families, the authors describes parents’views of how siblings participate in and respond to a short-term pediatric hospitalization. Specific patterns of sibling contact with the hospitalized child are presented and the role of siblings in the family's overall adjustment to the situation is explored. Data revealed that most siblings maintained regular, direct contact with the hospitalized child. Almost half the siblings participated actively in accommodating family life to the hospitalization by assuming additional responsibilities in only 16 percent (n=17) of the siblings. Negative responses clustered in those siblings cared for outside their own home and in the 4 to 11 year age range.

The kidney functions as an excretory, regulatory, and endocrine organ. The healthy kidneys: 1) excrete metabolic end-products as well as excrete and selectively reabsorb eledtroytes; (2) regulate water, acid-base balance, and systemic blood pressure; and 3) produce erythopoietin, which is responsible for the regulation of red cell production in the bone marrow, and activate vitamin D to its biologically active form-both endocrine actions. Chronic kidney diseases involve a gradual destruction of the functional kidney units or nephrons. The remaining healthy nephrons compensate, but with the continuing loss of nephrons, eventually the disease overwhelms the adaptive mechanisms. The result is a breakdown in the excretory, regulatory, and endocrine functions of the kidneys, leading to accumulation of protein waste products, sodium, potassium, phosphate, and water. As diet is the only source of most of these substances, dietary treatment becomes an integral part of management in the chronic renal failure patient. The treatment can be successful only when the relationship of the diet to the disease and knows how to plan daily meals to meet the dietary prescription.

This study was instrumental in initiating programs for more complete delivery of nursing care in the Cerebral Palsy Clinic. The data have provided information valuable in assessing already established nursing interventions. In a large clinic setting the nurse's time can be very limited, and the study has been most helpful in setting priorities to meet the patient's and parents’needs. This should ease the stress facing most parents in caring for their disabled child. In the future, a needs assessment, such as this one, could be used as a valuable tool for re-evaluating nursing care.

Children with cerebral palsy and their families encounter numerous problems, needs, and concerns that are within the realm of nursing care. This study documents needs perceived by parents as indicated on a questionnaire. Data collection took place in the waiting area of an outpatient cerebral palsy clinic in a large pediatric referral hospital. Questionnaires were administered to a convenience sample of 203 parents over a 6-month period. Parents responded to the statement,“It would help me/my child to talk with a nurse about this.”Parents selected items from a list of 31 possible topics and identified topics of concern to themselves or to their child. As many as 78 percent of parents identified one or more topics for themselves to discuss with a nurse, but only 34 percent of parents identified one or more topics for their child to discuss. A limitation of the study was that no data were collected documenting mental retardation or speech disorders, which could have accounted for the fewer number of topics identified for the child.The primary benefits of the study are two-fold: (1) Needs perceived by parents for themselves and their children were identified and prioritized. (2) Variables that are significantly related to number of needs were identified.
