Throughout its history, nursing has used a variety of measures to promote and maintain skin integrity. Air-fluidized therapy has been found to be successful in decreasing the number of problems associated with the integementary system by its ability to heal open skin areas rapidly and in a safe manner. For the pediatric nurse, this form of therapy is becoming recognized as an adjunct to care.

The initial psychosocial impact of the diagnosis of insulin-dependent diabetes mellitus is evidenced by grief responses and crisis reactions. An overview of classic theories on grief and crisis intervention, as well as literature specific to diabetes, provides a basis for the development of appropriate nursing interventions for the pediatric diabetic client and family. The nurse, as a part of the health care team, can assess the psychosocial impact of the disease, and assist the family in the ongoing adaptation to life with diabetes. Emotional support, assistance with coping strategies and resources, patient education, tailoring of the treatment regimen, maintenance of hope, and close follow-up are essential nursing actions.

Children in greater numbers are being sent home from the hospital on low-flow oxygen therapy. Positive aspects of discharging children including the decreased cost, the normal home environment, and increased socialization are discussed. Criteria for discharge and a plan and guide for discharge teaching are also included in this article. Discharging children on oxygen is discussed as a viable alternative to prolonged hospitalization.

The purpose of this study was to compare two methods for maintaining intravenous therapy in preterm infants. A quasiexperimental research design was used. The model for the study was Roy's adaptation model (1). The population for this study was all infants, in neonatal intensive care units, undergoing intravenous therapy. A convenience sample of all infants admitted to one neonatal intensive care unit was used. Two hundred infants were the subjects. One group (n= 100) of infants received intravenous therapy via scalp vein needle, and the other group (n= 100) of infants received intravenous therapy via teflon catheter. A data collection form was attached to each subject's chart. The number of attempts necessary to insert the intravenous cannula, the duration of time in hours that the cannula was in situ, and the reasons for cannula removal were compared. Statistical analysis byt-tests and chi-squares showed that the infants in the teflon catheter group were younger gestationally, weighed less, and spent more time on the respirator, more days on intravenous therapy, and more days in the hospital (p<.0001). No significant difference was found in duration times between the two groups. Scalp vein needles primarily were removed for tissue edema, while teflon catheters were removed for tissue edema, cannula clotting, and cannula leakage (p<.007). These findings indicated that for this sample of preterm infants, the teflon catheter was the most efficient cannula for intravenous therapy.

The perceived needs of adoptive and biologic parents were investigated by questionnaire in this exploratory, descriptive study. Fifteen adoptive and 15 biologic parents were surveyed about their perception of their informal, emotional, and judgment-developmental needs. Variables such as age, sex, numbers of other children, and the parents' feeling of preparedness for parenting were studied.Results of the study revealed that adoptive parents perceived fewer needs than biologic parents in all categories. Sex of the parent did not correlate with a specific category of need. Age of the parent was significant for adoptive parents. Number of other children correlated significantly for both groups. Previous child-care experience and perceived preparation for parenting correlated significantly for adoptive parents. Preparation for parenthood correlated significantly for both adoptive and biologic parents.

Deinstitutionaiization of persons with mental retardation/developmental disabilities is presently in process. Persons who formerly resided in large institutions will now be integrated into the community. States vary in the way that they fund and provide services to these persons. Nurses need to be aware of the trends in their own state and be supportive of persons who will receive community-based services. This article provides examples of financial support in two states and also suggests nursing interventions across the life span.
