The purpose of this study was to explore the experiences in health-care settings children report being afraid of. A 29–item Likert-type questionnaire was developed based on interviews with 140 school-age children. Another sample of 84 children was then administered the Child Medical Fear Scale (CMFS) in a school setting. Findings reveal that medical fear scores, in general, declined with age in this group of children. There were no significant differences in overall fear scores for children of different age groups. However, younger children did rate certain items related to intrusive procedures as most fearful. There were no strong, significant relationships between reported fear levels and gender, race, or other sociodemographic variables. Further study using the tool with hospitalized and chronically ill children is suggested. Recommendations for use of the CMFS in clinical practice include use of the scale to assess an individual child's fear level prior to providing preparation as well as its use as evaluation instrument to determine the effectiveness of various preparation programs.

It has been postulated that interventions aimed at specific disease- and treatment-related problems can diminish the stress of chronic disease in the adolescent. This report defines these problems in 28 cystic fibrosis (CF) patients with advanced disease who participated in a multicentered, mutlidisciplinary study of the impact of nocturnal oxygen therapy on the progression of their disease. The patients were followed over a period of six months to three years. Over the period of follow-up, 8 of 28 died. They were repeatedly visited at home by a nurse who recorded the problems spontaneously discussed by the patient or family. The problems were divided into three major categories that were revealed in sequence over approximately one year. Initially, the problems discussed related to CF management, then to growth and development including daily activities, school/work, and sexuality, and ultimately, to stresses on family relationships. This provided a framework in which to tabulate the types of nursing interventions that may prove to be most effective in diminishing the stresses confronted by the CF young adult who is attempting to maintain normal growth and development in the presence of increasingly severe disease.

With the use of a pre- and posttest design, the effect of a one-week camping experience researched knowledge, attitude toward diabetes, and self-concept of diabetic and nondiabetic children in an integrated residential camp. Subjects for this study were 63 children with diabetes and 18 children without, ages 8 to 14. A modified Etzwiler Diabetic Knowledge Questionnaire, a modified Attitude toward Disabled People Scale, and a Piers-Harris Children's Self-concept Scale were administered on days one and six. Subject characteristics considered to be related to the variables under study were assessed. Pairedt-tests, analysis of covariance with repeated measures, and Pearson correlation techniques were used in data analysis. While both the diabetic and nondiabetic groups significantly increased in knowledge about the disease, in neither group was their change in attitude toward the disease significant nor did the nondiabetic subjects demonstrate a statistically significant difference on this variable. Age of onset was inversely correlated with knowledge gain scores, and self-concept were positively correlated with the knowledge scores. The groups did not differ in knowledge acquisition, change in attitude toward the disease, or self-concept. The effect of the camping experience was essentially the same for the two groups.

A model of nursing care for families with an infant who requires home apnea monitoring is described. The family's adjustment to having such an infant is related to the quality and content of the interaction between nurse and family. The nursing interventions are more efficient when directly related to the family dynamics and the phases of adaptation to having a baby at home on an apnea monitor. This model is derived from three identified stages of family adaptation: (a) crisis period, (b) return to normalization, and (c) weaning phase. In the crisis period the nurse develops a trusting and collaborative relationship with the family so as to help them manage their uncertainty as they establish a sense of competence in their ability to parent their infant. This includes helping the family learn and master the more technical areas essential for a successful monitoring experience. The period of normalization is characterized by a large degree of stability which enables the nurse and the family to address other health related issues. The nurse helps the family see their infant in terms of normal growth and development and his role as a member of the family. The weaning phase encompasses the period of time required to accomplish the task as well as the period of adjustment following discontinuation of the monitor. This nursing approach, as part of a multidisciplinary team, has been successful in decreasing emergency room visits, and patient hospital days while increasing family functioning for more than one hundred families in the past two years.
