In order to adequately assess the needs of families dealing with infants with congenital anomalies, health care providers should understand the needs and events of a healthy pregnancy, labor, delivery, and postpartum period with a healthy infant. Therapeutic interventions can then be developed to stimulate and enhance growth and development of individuals and the family as a whole.

The decision to have surgery was identified by chronically disabled adolescents as one of the events that was critical to their hospitalization experience. Twenty-four adolescents who had been randomly selected from a list of planned surgical admissions and their parents were tracked through hospitalization in a pediatric-orthopedic setting. Participant observation and intensive interviewing were employed to gather the adolescents' and the parents' perceptions of the structure and management of their experiences. The resulting field notes and interview transcripts were first examined to delineate topical themes, including the decision to have surgery and various activities and relationships in which the adolescents and their parents participated. Second, the data were coded with categories that reflected these themes. Third, data from topical categories were combined to describe conceptual themes including critical events and the strategies used to participate in them. Fourth, the data from conceptual categories were combined to form an overall description of the experience, including the structural framework derived from the critical events and management strategies that were derived from the various forms of participation used by the adolescents and parents during the hospitalization. The decision to have surgery was examined as a selected critical event, including a description of the characteristics that shaped the content or meaning of the decision; the strategies used to manage the decision; an evaluation of both the event and its management; and clinical, research, and theoretical implications for nursing practice. Case history material complements the discussion.

A descriptive study of the rate of learning socially acceptable (independent and dependent) bowel and bladder management for the child with myelodysplasia was done. All those children born with myelodysplasia and being followed in an interdisciplinary clinic were included (N = 525). Ages ranged from birth to 18+ years. They were classified into one of three categories according to the level of their lesion:a) sacral,b) low lumbar, orc) high lumbar or thoracic. Data were analyzed by the Breslow and X2statistical techniques. The children with high lumbar and thoracic level lesions acquired independence more slowly than those with lower level lesions (p=. 0008). Development of independence was started for children with low lesions as early as 4 to 7 years of age; however, it was not until 10 to 11 years of age for those with high lesions. By 7 to 9 years of age, 50 percent of the children with low lesions had achieved socially acceptable urine collection but were still dependent for stool collection; they became totally independent for both urine and stool collection by 11 to 12 years of age. In contrast, only 40 percent of the children with high lesions had learned independent toileting skills by 17 to 18 years of age (at the end of the study). Delay in independent urine care was attributed to complexities of both clean intermittent catheterization and application of penile and stomal collection devices. Those children with impaired intellect were the slowest to acquire self-toileting skills (p=. 08). The rate of learning toileting skills was not affected by the presence of lax or weak anal tone, abnormal upper extremity function, the child's sex, or age on entering our program.

A majority of children diagnosed with acute lymphocytic leukemia remain in continuous remission 5 years following diagnosis due to improved medical treatment. However, the term“cure”is seldom used, and these children and their families must cope with the uncertainty of a life-threatening illness. The literature indicates that many of these young patients as well as their siblings develop psychosocial problems secondary to this stressful diagnosis. Children's attempts to cope with the impact of leukemia must be evaluated within the context of the families of which they are a part. This paper contrasts the dynamics of two vastly different families and the effects their interactional patterns have on the coping behavior of the patient and siblings. Emphasis is placed on the functions and influence of the marital dyad. A discussion of nursing implications and interventions is included.

This paper describes a training program for teaching children with leukemia to handle ridicule, derogation, and harassment in the form of teasing by peers. It first outlines the basic principles of teasing interactions, then describes the nonverbal behaviors that the teased child must not/must exhibit and the verbal strategies that he must learn to use. The procedures for teaching these new responses are provided in sufficient detail that the interested adult could adapt the program to the teasing problems of a specific child. The paper concludes with a discussion of the strong positive impact the program has on the self-confidence of children who are now able to cope with what previously had been, for them, an insurmountable problem.

The purpose of the study was to describe a population of women who successfully combined breast-feeding with maintaining employment. Ninety-six women responded to a questionnaire that covered: reasons for breast-feeding, reasons for returning to employment, working conditions, physiological problems, effect on self-concept, guilt feelings, techniques, sources of information, support systems, and the relationship with the father. Most of the women reported that continuing to work outside the home while successfully breast-feeding resulted in positive effects on their self-image and their relationship with the father. Over half experienced engorgement and milk leakage. Most had the support and help of their co-workers and husbands. Many women suggested it would be helpful to receive breast-feeding information about combining breast-feeding with working and to experience open discussion on the topic with other mothers.
