Adolescents with developmental disabilities have pertinent developmental and psychosocial needs that require consideration for optimal long-term planing and intervention. Unless the special needs of these adolescents are addressed, obstacles to living meaningful and satisfying lives will occur. This article examines the major issues facing adolescents with developmental disabilities. Nursing implications are discussed.

Pain assessments using three different observational pain assessment tools—the Post Operative Pain Score (POPS), the Nursing Assessment of Pain Intensity (NAPI), and the Pain Rating Scale (PRS)—were made on 98 preverbal children following surgery in an attempt to establish the reliability and validity of the three tools. Two observers (raters) scored pain intensity using each of the three instruments before and after administration of an analgesic. Interrater agreements were satisfactory on all three tools, with Interclass Correlations (ICC) ranging from. 71 to. 85.Ttests supported the discriminant validity of all three tools at thep<. 0001 level. The POPS and NAPI had internal consistency reliability alphas ranging from. 79 to. 88 for the POPS and. 59 to. 77 for the NAPI. Item analyses suggested specific revisions of the tools that might increase their reliability.

This exploratory study examines the relation between female adolescents' emotional autonomy and oral contraceptive adherence. The participants were 51 single female adolescents (16,17, and 18 years old) who had never been pregnant. At the 3-month follow-up, 39 of the subjects (77%) completed the study. Subjects completed the Emotional Autonomy Scale (Steinberg&Silverberg, 1986) initially and at the 3-month follow-up. Oral contraceptive adherence was assessed at the 3-month follow-up. Subjects had relatively high oral contraceptive adherence and autonomy. Oral contraceptive adherence was not associated with autonomy (r=. 22,p=. 21). Autonomy may be more influential on adolescents' seeking oral contraceptives rather than to adhering to their use.

摘要:
This study explored one woman's experience in becoming a mother to her premature son during his prolonged hospitalization. This paradigm case is an exemplar selected from a larger phenomenological study that examined the high-risk perinatal experience. Hermeneutic analysis led to the discovery of the phenomenon ofmaternal workembedded in the experience of this mother. Dimensions of her maternal work included working for love, working for meaningful moments, working amid uncertainty, working against the odds, and working overtime. Understanding the experience of maternal work can assist nurses to acknowledge and support the work inherent in becoming a mother within the neonatal intensive care unit.

Studies of parent-child interactions during perinatal addiction treatment are needed to offer insights into the nature of child development outcomes for this population. A purposeful sample of 17 women in a drug and alcohol addiction treatment program and their infants was studied over the course of their first year in the program. The purpose of this naturalistic field study was to examine parent-child interactions during addiction treatment. The following research question guided the inquiry: What is the nature of parent-infant interaction during the first year in a perinatal addiction treatment program? This study combined grounded theory methodology with the quantitative methods of descriptive and differential statistics. Open-coding of interviews, field notes, and the treatment records followed grounded theory methodology. A time-sampling procedure was used to examine stability of interactions at the beginning of program participation, again when each infant was 6 months old, and when each infant was 12 months. Results are used to describe parent-child interactions across samples and over time.Chi-square procedures were performed on number of children in each family, length of time in the program, sobriety, and mutual enjoyment of interaction. A significant difference in interaction by length of time in the program (χ2= 7.0801, df = 1, p<. 00) as well as in interaction by percent sobriety (χ2= 4.1538, df = 1, p<. 04) was evident. Behaviors of the infants reflected the emotional tone and parenting responses of their mothers. Dyads whose interactions showed a pattern of mutuality were more likely to continue in the treatment program and to maintain sobriety. The ability of a mother to enjoy her infant seemed to be the one factor that made the most difference in child behaviors. Results can be used by clinicians to guide the choice of treatment approaches that support the parent-child relationship during addiction treatment.
