Kawaski's disease is an increasingly prevalent condition seen in children. Its symptoms of fever, conjunctivitis, erythematous rash, and lymphadenopathy resemble scarlet fever. However, the disorder is penicillin resistant but responds well to high doses of aspirin. Its etiology remains unknown.In 1980, an outbreak of 57 cases of this disease was documented in Massachusetts. Manifestations such as behavioral changes, extreme dehydration, seizure disorders, and cardiac involvement were noted, posing a unique challenge to the nurses in their attempts to care for the children and their families. Due to scarcity of existing literature pertaining to Kawasaki's disease, the staff had no plan on which to base their nursing care. However, an innovative group of nurses developed an effective, comprehensive plan of care geared toward diminishing the stress experienced in the course of hospitalization.

Numerous studies have shown the relationship between a hearing impairment and delayed language development in children. Using an open-ended questionnaire, the authors surveyed 24 parents of children (18 months to 12 years; hard of hearing to profoundly deaf) enrolled in a program for the hearing impaired. Preliminary data gathered concerned 1) age of the child when the problem was suspected, 2) the first person to suspect the presence of a hearing loss, 3) factors responsible for arousing suspicion, and 4) those behavioral variations that concerned the parents. Useful information in all categories was obtained, with results validating the suspicion that many children were identified late—only two were identified by a professional practitioner. A thorough review of the literature confirming the authors' findings as well as a description of the Limited High Risk Register, a tool designed to assist in the early screening of hearing impairment, is presented.

Pediatric cardiology clients, in various stages of diagnosis and repair, and their parents were observed during clinic visits, hospitalization for cardiac catheterization, and hospitalization for palliative surgical repair. In addition, the parents were interviewed and asked to identify factors they found overwhelming, frightening, or threatening. The stressors are identified and supported with examples of parent and client behavior. The nursing interventions utilized are identified and their effects exemplified through the use of case studies. A clinical specialist's role in pediatric cardiology is proposed.

Using labeling theory the authors investigated the influence of socioeconomic status, ethnic status, and level of injury on recognition and reporting of child abuse by nurses and physicians. Respondents judged vignettes (emergency room forms) in which these variables were experimentally manipulated. Physicians' judgments regarding child abuse were affected by socioeconomic status (SES), ethnic status, and level of injury. Nurses' judgments were affected by level of injury but not SES and ethnic status. The implications of nurse and physician judgments are then explored in regard to identification of treatment, research data from official records, and subsequent etiologic theories. The nurse's role as a member of the health-care team in recognizing and reporting child abuse is then considered.

Siblings react to the death of a brother or sister. These reactions may be nightmares, a change in school behavior and performance, depression, feeling rejected by parents, withdrawal, along with many other reactions. Parents, friends, and health care professionals can be an instrument in helping siblings cope. Play therapy may be a therapeutic approach with siblings for prevention and crisis purposes.

摘要:
The focus of this article is the development of a family adaptation tool to be utilized by nurses in a two-stage referral mechanism. This tool can assist in the development of the clinical practice of maternal-child and community health nurses to further facilitate a positive childbearing experience.Baccalaureate nursing students were utilized as data collectors in an effort to involve them in nursing research as well as exposing them to a distributive approach to health care. This allowed development of nursing interventions at two levels, the hospital and the home, and identified the referral mechanism as a liaison between the two environments. The tool is particularly helpful in assessing the areas of maternal adaptation and the impact of the environment.
