Blocked tympanostomy tubes re-create the problem for which they were initially inserted. Hydrogen peroxide offers a noninvasive alternative to extracting the blocked tube and inserting a new tube. In this article we present the results of a long-term retrospective study of 95 patients (113 ears) who had occluded ventilation tubes following myringotomy. An 85% rate of success in opening the clogged tube with hydrogen peroxide is documented. The ototoxicity of the treatment is also examined. Ninety-nine percent of the patients showed no immediate change in bone conduction thresholds following the use of hydrogen peroxide. We have concluded that hydrogen peroxide is a convenient, noninvasive, and inexpensive treatment blocked ventilation tubes.

Although it is well recognized that collapse of the external meatus can mimic a conductive hearing loss in the test ear, attention must also be directed to the masked ear during masked bone conduction threshold testing. Failure to recognize collapse of the masked ear may result in a false air-bone gap in the test ear. Two such cases are presented and the mechanism analyzed. Suggestions as to identification and resolution of this problem are discussed.

The late results of tympanoplasty were analyzed three to fifteen years postoperatively in 124 ears of children aged 2 to 14 years with noncholesteatomatous chronic otitis. The primary graft take rate was 92% and only 4% of the ears had perforations at follow-up. The hearing results were very good. The overall results are similar in children operated on at the ages of two to seven years and in children operated on at the ages of 8 to 14 years. We recommend tympanoplasty in children of all ages, including preschool age.

Time-compressed speech has been used to identify the site of central auditory disorders and auditory processing problems. Some authors suggest that the determination of audiometric normalcy should include satisfactory performance on a speech intelligibility test sensitive to central auditory dysfunction, such as time-compressed speech. Before scores from patients with auditory disorders can be meaningfully interpreted, however, it is necessary to establish normal performance on standardized recordings. Thus, this study was undertaken to generate speech intelligibility functions in a normalhearing population using the Auditec CID W-22 recordings with 30% and 60% timecompression. The 30% condition revealed a slope of 3.24% per decibel and the function approached a plateau at 35 dB sensation level (SL). The 60% condition produced a function that increased gradually at 1.8% per decibel over the 20 to 80% intelligibility range. An intelligibility score of 82% was observed at 46 dB SL where the function approached an asymptote. These functions provide a standard against which subjects with central auditory dysfunctions can be compared. The relative difficulty that normal subjects had with the 60% time-compressed speech suggests that this condition may be too difficult for some subjects with central auditory disorders.

Mucocutaneous lymph node syndrome (Kawasaki's disease) is an acute febrile, exanthomatous illness in which massive lymphadenopathy may occur in the cervical area. This lymphadenopathy may mimic other conditions and present a diagnostic dilemma. We present a case report of a child whose initial clinical signs, symptoms, and laboratory findings were compatible with acute coalescent mastoiditis; however, the child was found after close observation to have Kawasaki's disease. The case demonstrates the similarity between the two diseases and how close observation can prevent unnecessary surgery. When considering the rare diagnosis of mastoiditis, other entities need to be considered in the differential diagnosis before surgical intervention is undertaken. If the patient is unstable or a threatened complication of mastoiditis is apparent, however, immediate mastoidectomy is required.

The present study investigates the occurrence of asymptomatic posterior canal wall breakdowns following intact canal wall tympanoplasty (ICWT) with mastoidectomy. Twenty-four ears out of 501 (4.8%) presented with a posterior canal wall atrophy. A partial defect was found in 15 ears (62.5%), while a subtotal or total atrophy occured in the remaining nine ears (37.5%). Posterior wall atrophy could be considered among ICWT complications during follow-up visits, but, according to our investigation, its incidence remains low.
