ABSTRACTHearing is of special vocational importance in musicians. Musical performance may create sounds sufficiently intense to cause sensorineural hearing loss. Although such losses are usually not severe enough to be compensable under American Academy of Otolaryngology guidelines for occupational hearing loss, they may interfere with the musician's ability to perform the daily tasks of his or her profession. A review of the literature on occupational hearing loss in musicians reveals a substantial need for further research.

ABSTRACTMetastatic carcinoma of the temporal bone is being reported with increasing frequency. A literature review reveals 16 new cases reported in the past 2 years. In this article we present the first case of vaginal squamous cell carcinoma metastatic to the temporal bone reported in the literature. The pathophysiology, clinical presentation, and diagnostic difficulties of metastatic carcinoma to the temporal bone are discussed.

ABSTRACTA case of bilateral congenital cholesteatomas, which occurred in a 15-year-old male was reported. His family had been aware of his hearing impairment since he was 3 years old. He had experienced neither otalgia nor otorrhea. Tympanograms were A type bilaterally, and an audiogram showed conductive hearing loss of about 60 dB on both sides. High resolution CT of a horizontal section revealed soft tissue density occupying the space medial to the body of the incus in the right ear, while the middle ear space appeared to be completely clear on the left side. The membranous cholesteatomas on both sides were accompanied by ossicular anomalies with hypoplasia of the long process of incus and the superstructure of the stapes. This case constitutes the seventh case report of bilateral congenital cholesteatomas in the literature. The literature is reviewed and the pathogenesis of congenital cholesteatomas associated with ossicular anomalies is discussed.

ABSTRACTNumerous epidemiologic investigations have been conducted regarding the incidence of Menière's disease, but the values obtained for different countries vary greatly. This can be attributed to the fact that different criteria have been applied in defining Menière's disease. The objective of this study was to evaluate the incidence and prevalence of Menière's disease in Italy following the American Academy of Ophthalmology and Otolaryngology guidelines. A total of 111 cases with Menière's disease were diagnosed in a district of southeastern Latium over a 13-year period, with an incidence of 82 per 106. A figure 3.4 times greater was found in hospital personnel, thus showing that a greater availability of medical facilities increases the number of diagnoses. Sex was found not to be a determining factor. The authors suggest that the incidence reported in this paper is an underestimate and that the actual one remains undetermined, but probably higher than generally believed.

ABSTRACTTwo cases of ankylosis of the temporomandibular joint (TMJ) after otologic and neurotologic surgery involving an approach through the temporal fossa are reported. The etiology is believed to be that of fibrous contracture within the temporalis muscle, which is incised during the procedure. If the possibility of this rare complication is anticipated, the immediate instigation of prophylactic jaw exercises and forced opening with tongue depressors can significantly limit morbidity. If the condition is allowed to become established, then surgical correction by coronoidectomy may be necessary.
