Intraoperative facial nerve monitoring provides a potentially useful adjunct to recent surgical advances in neurotology and neurosurgery. These measures further aid the surgeon in preserving facial nerve function by enhancing visual identification with electrical monitoring of mechanically evoked facial muscle activation. Facial nerve monitoring in neurotologic surgery may achieve the following goals: (1) early recognition of surgical trauma to the facial nerve, with immediate feedback made available to the surgeon through monitoring of mechanical activation; (2) assistance in distinguishing the facial nerve from regional cranial nerves and from adjacent soft tissue and tumor with selective electrical stimulation; (3) facilitation of tumor excision by electrical mapping of portions of tumor that are remote from the facial nerve; (4) confirmation of nerve stimulability at the completion of surgery; and (5) identification of the site and degree of neural dysfunction in patients undergoing nerve exploration for suspected facial nerve neoplasm or undergoing decompression in acute facial palsy. This paper provides an overview of intraoperative facial nerve monitoring principles and methodology and reports a recent clinical investigation that demonstrates the utility of facial nerve monitoring in translabyrinthine acoustic neuroma surgery.

This issue ofThe American Journal of Otologyincludes the third of a series of articles on facial nerve disorders, which collectively are known as the “Facial Nerve Manual.‘’In the early 1980s, Dr. Mark May chaired the AAO-HNS Facial Nerve Committee and organized the Facial Nerve Study Group. One of the tasks he initiated was the creation of a Facial Nerve Manual, assigned to a small group of clinicians at the COSM Meeting in the spring of 1984. The purpose of this work was to disseminate practical information on the management of selected facial nerve problems, one of the primary charges of the committee.Work began slowly, but surely. During Dr. Gale Gardner's tenure as committee chairman, manuscripts were reviewed and edited by Dr. Nels Olson, andThe American Journal of Otologyagreed to publish the manual. Perhaps more than anyone else, Dr. Olson worked tirelessly to create a finished product of consistent style and content.Ever since the original writing and editing, controversial topics continue to emerge. As each chapter is published individually over this year, concepts may change, and the reader is encouraged to explore these issues in more depth.Drs. Docs' Discussion is a regular feature ofThe American Journal of Otologyand represents a summary of an actual Monday evening discussion at the Otologic Medical Group (OMG) in Los Angeles. The following discussion was taped on November 16, 1987, and edited for AJO by Drs. Sheehy and Benecke. Present for the discussions were Drs. Sheehy (moderator), Antonio De La Cruz, John House, Ralph Nelson, Bill Luxford, Jennifer Derebery, Jim Benecke, and Clough Shelton. Also taking part in the discussions were Drs. Linthicum (House Ear Institute), Hitselberger (neurosurgeon), and Jean-Marc Sterkers (otologist from Paris). Twenty-four registrants from the Temporal Bone Surgical Dissection Course at the Institute were present.
