ABSTRACTThe prevalence of tinnitus and dizziness in healthy elderly adults has been uncertain. This study investigated 267 people between the ages of 57 and 91 1/2 years. The subjects had no history of noise exposure, systemic diseases associated with tinnitus or disequilibrium, and had normal otoscopic examination results. Twenty-four percent reported tinnitus. Five percent experienced dizziness. Additional studies will help clarify the prevalence of these symptoms in the elderly “normal” population.

ABSTRACTA selection of commonly used metal ossicular prostheses were tested to see if they were attracted by the 5000-gauss magnetic field of a magnetic resonance imager. None of the prostheses tested was attracted by the magnetic field. Using a model system, it was found that some of the metal objects perturbed the magnetic field, thus affecting the image produced by the scanner. Therefore, while metal implants may be safe in a powerful magnetic field, they can alter the field, resulting in abnormal images.

ABSTRACTThe evidence for the existence of secondary endolymphatic hydrops in patients with perilymphatic fistulas is reviewed and the indications for treatment of such hydrops in patients with fistulas are discussed. The indications for and results of combined fistula repair by middle ear exploration and endolymphatic sac surgery in fifteen patients are presented. The preliminary results in these patients appear encouraging. Patients with recurrent perilymphatic fistulas, or with symptoms of attacks of spinning vertigo after fistula repair, and those who present with severe imbalance with findings of a perilymphatic fistula appear to be candidates for combined fistula and sac surgery.

ABSTRACTSpontaneous cerebrospinal fluid (CSF) otorrhea is rare. We present four new cases and an analysis of the literature.Two distinct subtypes occur. Seventy-two percent of cases are the childhood type with congenital defects of the otic capsule. Meningitis, usually pneumococcal and frequently recurrent, occurs in 92% of these cases. CSF otorrhea follows myringotomy for a presumed serous effusion. The child usually has unilateral and sometimes bilateral absence of cochlear and vestibular function and commonly exhibits a Mondini deformity. CSF usually enters the inner ear through a dural defect in the lateral aspect of the internal auditory canal and exists through the oval window. Treatment should consist of stapedectomy and packing of the vestibule with muscle or subtotal petrosectomy.Twenty-eight percent of cases of spontaneous CSF otorrhea are the adult type characterized by bony dehiscenses, most commonly of the tegmen tympani or tegmen mastoideum and less commonly of the posterior fossa plate. The meningeal defects are either meningoencephaloceles or simply holes in the dura. Therapy should consist of a mastoidectomy in conjunction with a transtemporal supralabyrinthine (middle fossa) approach if a meningoencephalocele of the tegmen is found.

ABSTRACTMonostotic fibrous dysplasia, an unusual disease that can involve the temporal bone, will frequently cause an acquired stenosis of the external auditory canal. Three patients with this disorder who presented with occluded canals are described. Two individuals developed external canal cholesteatomas medial to the obstruction, one of whom eventually developed a postauricular abscess and infected draining sinus. The third patient presented with a restenosis nine months following canalplasty.The cases reported illustrate three surgical criteria necessary to manage these unusual cases successfully: (1) removal of sufficient diseased bone to create a patulous canal; (2) resurfacing denuded bony areas with thin split-thickness skin grafts to prevent soft tissue contractions; (3) an adequate meatoplasty. Postoperatively, the reconstructed canals have remained patent and stable during follow-up periods ranging from one to four years.Clinical, radiographic, and pathologic features of fibrous dysplasia are discussed, the differential diagnosis is presented, and the relevant literature is reviewed.

ABSTRACTThe diagnosis of acoustic neuroma is being made more easily with the advent of late-generation, high-resolution computed tomography. It is known that acoustic tumor growth in the elderly is often slow. On the other hand, it is believed that an acoustic tumor can grow very rapidly in the young. A case of acoustic neuroma in a young man with computed tomographic evidence of extremely rapid growth over a period of fifteen months is presented.

ABSTRACTXanthomata are soft tissue tumors composed of lipid-laden “foamy” histiocytes associated with cholesterol clefts and inflammation. They are considered to be specialized granuloma rather than true neoplasmas and are usually associated with disorders of lipid metabolism, most commonly one of the hyperlipoproteinemia syndrome. A rare case of a xanthoma involving the temporal bone and causing extensive destruction of the skull base is presented. Otalgia and infection are the most common presenting symptoms of this disorder, with multiple cranial nerve palsies resulting in some cases. Treatment includes conservative surgical debulking, dietary restriction of fat and cholesterol, and pharmacologic reduction of serum lipids.

ABSTRACTEight new cases of “ear-pits-deafness syndrome” are reported here; in particular, the audiometric and tomographic findings of the ear and renal echography are discussed. Although it is difficult to give a nosologic definition of the syndrome, these cases can be classified as branchio-oto-renal dysplasia and branchio-oto dysplasia.

ABSTRACTStapedectomy was performed in sixty-seven ears with tympanosclerotic footplate fixation. This was confined to the footplate in 90% of ears, and there was an association with cholesteatoma and chronic infection. Large fenestra stapedectomy was performed in 91% of cases and the surgical procedure performed was tympanoplasty in 91% of cases. Three cases (4.5%) suffered a dead ear by three years following the procedure, but only one immediately. A variety of surgical procedures were used to reconstruct the tympanic membrane to oval window connection, and the air-bone gap did not depend on the choice of prosthesis in the short term. Retention of the incus led to later increasing air-bone gap, probably due to refixation by tympanosclerosis. Long-term results are analyzed for each method of reconstruction and each category of ossicular status identified at surgery. Small fenestra procedures produced less cochlear hearing loss at high frequencies. Surgical technique is discussed with reference to the pathophysiology of tympanosclerosis, and the role of hearing aids is considered.

ABSTRACTA 13-month-old child was seen and evaluated for persistent aural bleeding following the placement of a pressure equalizing tube. A vascular mass was identified in the middle ear space. The radiologic evaluation is discussed. The mass was surgically resected for both diagnostic and therapeutic reasons. Pathologic examination revealed a benign hemangioma. The significance of this diagnosis is discussed.
