Endurance exercise is usually prescribed with caution for hypertensive patients because of the possible adverse effects of increased blood pressure on cardiorespiratory function. Cardiopulmonary responses were studied in 11 young (37 ± 4 years) and 10 elderly (65 ± 4 years) male subjects during treadmill exercise with wrist weights (WW) and with no weights (NW) at constant 60% of their maximal heart rate for 8 min at each exercise condition. The results revealed that both groups had similar responses at rest. During maximal stress test, heart rate, oxygen consumption and minute ventilation were significantly (p < 0.01) higher in the younger subjects. During 60% WW and NW exercises heart rate, oxygen consumption and minute ventilation were significantly (p < 0.01) lower in the elderly than in the younger group. Diastolic and mean arterial blood pressures were significantly (p < 0.01) higher in both groups during WW compared to NW exercise. However, minute ventilation and rate-perceived exertion were significantly (p < 0.01) higher during NW compared to WW exercise. These measurements indicated a similar pattern of hemodynamics in younger and elderly hypertensive responders, with variable respiratory responses to endurance exercise. In addition, careful observation of blood pressure during exercise with WW is recommended for both groups where an increased afterload is contraindicated. The prevalence of Parkinson’s disease was studied in Izumo City, the Shimane prefecture having the largest population of the elderly in the country. The adult population of Izumo City (56,869 people over 20 years of age) were screened by means of questionnaires sent by mail and by other methods. A total of 66 people, 18 males and 48 females, were found to have Parkinson’s disease. The prevalence of this disease was 82.0/100,000 population, the highest reported in Japan to date. Of the patients affected, 17.1 % showed apparent dementia. Those with dementia were older and were classified at a more advanced Yahr stage than were the nondemented cases. We studied the urinary IgG and IgA antibody in elderly subjects without bacteriuria, with asymptomatic bacteriuria and symptomatic urinary infection. Elderly individuals with asymptomatic bacteriuria had significantly elevated antibody levels relative to controls. When followed up to 12 months with persistent bacteriuria, antibody levels tended to persist but were variable. In elderly women with elevated urinary antibody titers, site of infection tended to be localized to the kidney, but antibody levels within the control range occurred as frequently as elevated antibody levels for renal infection. A chronobiological study was carried out in 8 elderly male subjects (74–85 years) to evaluate the 24-hour vasoactive intestinal polypeptide (VIP) secretory pattern. Eight young adult males (21–32 years) made up the control group. Blood samples were drawn from each subject every 2 h during the day and hourly during the night for a 24-hour period. Mean 24-hour VIP values in elderly (21.1 ± 0.3 pg/ml) and young adults (19.1 ± 0.3 pg/ml) did not differ statistically, but daytime VIP values observed in elderly subjects (21.4 ± 0.5 pg/ml) were higher (p < 0.05) than those recorded in young adults (17.5 ± 0.5 pg/ml). Computerized tomographic (CT) studies with longitudinal follow-up of the relationship between supratentorial and infratentorial brain atrophy in Parkinson’s disease (PD) using our new CT-derived brain atrophy index (BAI) are reported. In those patients with Parkinson’s disease, CT analysis demonstrated a significant increase in brain atrophy compared with controls. Infratentorial BAI correlated significantly with supratentorial BAIs and the decrease in the infratentorial BAI correlated with a corresponding decrease in the supratentorial BAI in those with PD.
