AbstractTwo studies are reported in an attempt to give a broader interpretation of the management of salivary gland disorders. One study analyzes 232 patients seen over a six year period and divides the grouping into parotid, submaxillary, and accessory salivary gland disorders. The numbers and percentages of patients who underwent surgery are reported along with a pathological break‐down of the various lesions encountered. Eighty‐five (36%) of the 232 patients had a documented surgical procedure. Thirty‐eight of the entire 232 patients had tumor involvement, for a 16% incidence of neoplasia.At the same time 244 patients had various sorts of salivary gland surgery in the four local hospitals. Of these, 210 had either a parotidectomy or submaxillary gland excision. AbstractWhat is the incidence of non‐neoplastic disease when dealing with masses of the major salivary glands? A knowledge of this information would aid the physician in making decisions relative to the management of the masses. Furthermore, the expected malignancy rate may need to be adjusted according to the findings of this study.Many reported series of salivary gland neoplasms show the relative incidence of benign vs. malignant neoplasms; however, not every patient with a mass of the salivary gland who comes to surgery is finally diagnosed as having had a neoplasm. Knowing the relative incidence of neoplastic vs. non‐neoplastic salivary gland diseases has important diagnostic, therapeutic, and prognostic implications.Records of all patients who underwent either a parotidectomy or submandibular gland excision from 1975 to the present at the Eye and Ear Hospital of Pittsburgh were reviewed; 73% of the parotidectomies performed were done for neoplastic disease. AbstractA survey of over 250 consecutive parotidectornies performed at The Mason Clinic indicates a surprising incidence of metastatic malignant disease to the parotid gland. Four percent of all parotidectornies performed indicated a metastatic focus of malignancy from an unsuspected primary outside areas of the head and neck usually implicated in parotid disease. Twenty‐five percent of all cancer discovered at parotidectomy was metastatic malignant disease to the parotid gland. The most common site was the lung. A high index of suspicion, evaluation of pathologic specimens by electron microscopy, and detailed metastatic work‐up for patients with malignant disease in parotid lymph nodes are recommendations which should be considered by the surgeon performing parotid procedu

AbstractIn the general population, 90% of the malignancies developing in the upper respiratory tract are squamous cell carcinomas. Analysis of 1081 neoplasms in 1023 renal transplant patients reported to the Denver Transplant Tumor Registry indicates that squamous cell carcinoma reMayns the predominant malignancy which develops in the upper respiratory tract of immunosuppressed individuals. Comparing the incidence of cancers of the oral cavity (excluding lip cancers) in the immunosuppressed population with the general population it is rather similar (3.5% vs. 4%). When lip cancers are included, one sees a great excess of tumors developing in transplant patients (15% vs. 4.6%). Thus the excess of head and neck epithelial malignancies among these patients is confined to neoplasms of the lip.Examination of the 25 patients reported in this study reveals an unusually young age of the patients, the average age being 41.8 years compared with the general population where most head and neck tumors occur in the 55‐65 age group. The average time posttransplantation when cancer developed was 5.3 years, supporting the concept that the more effective the immunosuppressive regimen for prevention of graft rejection, the greater the susceptibility for the development of cancer. Since 90% of transplant patients have serological or clinical evidence of active herpes virus infection, it is possible that this microorganism may play a role in the development of certain head and neck malignancies, especially those involving the lip

AbstractCritical assessment of head and neck cancer with respect to staging has, on occasion, been disappointingly ineffective. We have attempted to correlate the incidence of measureable uptake of cobalt 57 tagged bleomycin by primary squamous cell carcinoma and metastatic cervical lymph nodes. Forty‐six cases have been evaluated with respect to histopathological confirmation of the suspected metastatic disease. We have found that this diagnostic measure increases our acumen in staging of head and neck cancer.The relevance of the Co‐Bleo scans as a diagnostic aid is reported in 46 cases. Malignant tumors greater than 2 cm in size appear to demonstrate active uptake of the imaging agent. Small tumor size and excess background radioactivity contribute to the false‐negatives (17%). Inflammatory conditions or benign tumors of the salivary apparatus may result in minimal uptake, thus, a false‐positive result (10%). AbstractA perilymph fistula is an abnormal communication between the fluids surrounding the membranous labyrinth and the middle ear space. Because of the potential hazards of meningitis, permanent hearing loss, and occasionally incapacitating vestibular symptoms, early recognition and prompt repair of the perilymph leak is important.One hundred thirty three cases of perilymph fistulas are presented, stressing the clinical characteristics, evaluation, and management of patients with this otologic entity. The historical and contemporary literature on this subject is reviewed. A pathophysiological basis for perilymph fistula formation is presented, based upon certain anatomic, physiologic and mechanical principles involving the temporal bone and surrounding structures.For the purpose of publication, the Materials and Methods and Evaluation sections of the paper have been omitted. AbstractTraditionally, patients with tympanotomy tubes are admonished not to get water in their ears so that ear infections will not occur. This study reviews 100 children during the past three years who were given instructions to allow bath and shower water to get into their ears, and who were allowed to swim as long as they used polymyxin/neomycin/hydrocortisone ear drops at night after swimming. Only rarely did problems arise, and these were easily managed. No long‐term harmful effects occurred. AbstractAuditory brain stem responses (ABRs) were reported with two groups of patients with confirmed and suspected unilateral hearing loss. AbstractNasal septal abscesses are uncommon. Sixteen cases from the Massachusetts Eye and Ear Infirmary were reviewed retrospectively and are presented. The diagnosis, bacteriology, and pathophysiology are discussed. Immediate therapy is indicated to avoid cosmetic deformity or intracranial infection. Treatment is based on diagnostic needle aspiration, antibiotic coverage and surgical drainage.
