AbstractThirty‐seven cases of ceruminous gland tumors of the external auditory‐canal are presented. These are rare tumors, each of which has different clinical characteristics. Six adenomas are reported which are benign and require only local excision for long‐term cure. Two muco‐epidermoid carcinomas, a type not previously reported, are presented. The muco‐epidermoid carcinoma tends to iecur locally and radical excision is necessary for control. The most common glandular tumor of the external auditory canal is the adenoid cystic carcinoma. Twenty‐four are reported. Pain is the hallmark of this tumor. The long clinical course ultimately results in death if the tumor is not radically removed at the earliest possible time. Radiation is ineffective. Treatment recommended is (1) wide excision of the external auditory canal, (2) extensive radical mastoidec‐tomy, and (3) total parotidectomy and mandibular condylectomy.Five cases of adenocarcinoma are presented. Four were dead from local invasion and distant metastasis within four years, and the fifth was lost to follow‐up.Careful histologie evaluation and classification of the type of tumor is necessary to predict the natural history and select the proper treatment. Local excision is curative for the benign adenoma. For the three types of carcinoma described, the surgeon must not hesitate to do the radical operation for the early lesion, because it is in the early stages that the opportunity to produce

AbstractThis report presents a case of squamous cell carcinoma of the temporal bone induced by high dose radiation therapy. Several important surgical techniques are stressed which serve to make this procedure far safer in the avoidance of catastrophic hemorrhage.

AbstractAn analysis was made of 31 patients with malignancy of the temporal bone treated at Massachusetts Eye and Ear Infirmary between 1959 and 1975. Treatment modalities included: partial temporal bone resection, subtotal temporal bone resection, total temporal bone resection, radical mas‐toidectomy followed by radiation therapy, radiation therapy alone, and chemotherapy. When tumor was limited to the external ear canal, partial temporal bone resection was effective in 10 out of 11 patients. When tumor involved the middle ear, subtotal temporal bone resection cured 3 out of 5 patients, and radical mastoidectomy followed by radiation therapy cured 5 out of 12 patients. Total temporal bone resection and chemotherapy failed to cure a patient each, but radiation therapy alone was successful in controlling tumor in one patien

AbstractThe diagnosis of sarcoidosis can be established when the case meets the criteria of clinical course, radiologie findings, and supportive histology.When there is variation from the established criteria and the clinical course of age, systemic evidence of disease or the radiologie findings of unilateral or solitary adenopathy are encountered, then ongoing evaluation is necessary to identify a possible second disease existence.Cases have been presented which indicate some problem requiring further investigative studies before acceptance of the diagnosis of sarcoidosis.

AbstractThe etiologic considerations in head and neck lesions rarely include the diagnosis of thymic cysts. A correlative summary of three new cases was analyzed with respect to the already existing body of data. A classification of thymic cysts, their embryogenesis, clinical signs, and appropriate pre‐operative studies were stressed. AbstractSeveral clinical reports suggest allergy as a causative or associated factor in the etiology of Ménière's Disease or endolymphatic hydrops. The A.A.O.O. subcommittee reported in 1972 its criteria for a therapeutic improvement in Ménière's Disease. AbstractAn attempt has been made through this study to establish a tumor suspect pool of 100 patients, each one highly suspect because of a single or usually several strong clinical indicators of a retrocochlear lesion. In turn each patient has been proven tumor negative by virtue of a normal opaque cerebellopontine cisternogram. AbstractThere is a disturbing variation in the reported success rate in Endolymphatic Sac Surgery in the literature. Relatively long‐term (24 months to 10 years) follow‐ups are given in a series of 66 patients in this paper. Results reveal only a 55% long‐term relief of veitigo in these patients. Only 30% of this series of cases had any improvement in cochlear function. There was a definite trend to Awards recurrence of vertigo in the longer follow‐up cases.It is concluded that Endolymphatic Sac Surgery still is important in the treatment of idiopathic endolymphatic hydrops, but is of questionable value early in the course of the disease.
