AbstractThe reliability of a neurotological evaluation in differentiating labyrinthine (end organ) lesions from retrolabyrinthine lesions has been well established. The purpose of this communication is to determine the diagnostic potential of computed tomography (CT) in patients suspected of having retrolabyrinthine lesions on the basis of their neurotological evaluations. Our study indicates that the validity of central vestibular and audiometric signs can be best substantiated with CT. The anatomy of neurovascular bundles within the internal auditory canal and cerebellopontine cistern can be readily visualized and identified by CT pneumocisternomeatography. By obtaining appropriate thin CT sections not only can very small lesions be identified but their anatomic locations can also be accurately established. CT pneumocisternomeatography is the method of choice to diagnose an intracnnalicular acoustic neuroma. An unusually large vascular loop of the internal auditory canal may be responsible for sensorineural hearing loss and tinnitus in some patients.

AbstractCancers in the head and neck are recognized as placing their environs at risk. The orbit is threatened by a variety of cancers originating in adjacent structures because of its anatomic relationship to skin, sinuses, nasal cavity, skull, palate, and salivary glands. The poignancy of this is highlighted by the fact that the orbit contains the organ of sight and also has high aesthetic value. An evaluation of the pathologic physiology of the various neoplasms affecting the orbit, their management, the methods of reconstruction, and the criteria for radical and conservation techniques will be discussed.

AbstractSurgical planing or dermabrasion has many uses in facial aesthetic and reconstructive surgery but often is an underutilized technique. The purpose of this paper is to familiarize the head and neck and facial plastic surgeon with some of these uses extending from trauma to rejuvenation of the aging face. Detail will be presented regarding analysis For conditions where dermabrasion is indicated and in dealing with modifications of surgical technique. The author feels that dermabrasion is the treatment of choice for scars and acne and favors it over chemabrasion or chemical peel even for fine rhytids because the depth of dermabrasion can be precisely controlled and varied according to the location of and the degree of scarring or faciul wrinkles. It offers early and predictable healing without concern for the penetration and absorption of chemical elements (especially phenol). AbstractOne of the causes of nasal airway obstruction during inspiration is an incompetent nasal valve. An incompetent valve is one that collapses during quiet normal breathing due to narrowing of the valve area, a loss of upper and/or lower lateral cartilage support, or a combination of these. The most common etiology is a previous rhinoplasty; trauma and aging are other causes. Treatment is surgical and consists of replacing what is missing with similar tissue. If only the vestibular skin is scarred, a skin graft is used. If cartilage alone is missing, a cartilage graft is inserted. If both skin and cartilage are absent, a composite graft from the concha is the best choice.

AbstractRecent studies have indicated that in a denervated muscle highly specific interactions occur between regenerating axons and the basal lamina of the myofiber precisely at the original synaptic sites. Since these findings suggest that synapse regeneration is facilitated at the original synaptic sites, a knowledge of the distribution of neuromuscular junctions in the human posterior cricoarytenoid (PCA) muscle may guide reinnervation procedures in this muscle. We, therefore, have used histochemical and computer graphics techniques to reconstruct the three‐dimensional distribution of neuromuscular junctions in the human PCA taken fresh at autopsy. The results indicated a more localized pattern of neuromuscular junctions as compared to the human thyroarytenoid muscle. AbstractThis study compares the neck tumor recurrence rate between patients treated with radical neck dissection and those treated with conservation neck dissection. A standard radical neck dissection modified by sparing at least the internal jugular vein or the spinal accessory nerve is defined as a conservation dissection. Six hundred ninety‐one neck dissections performed on 631 patients in the Department of Otolaryngology‐Head and Neck Surgery of the University of Illinois College of Medicine at Chicago were reviewed retrospectively. All patients had been followed postoperatively for at least 24 months. Group I consisted of 422 radical neck dissections. Group II contained 269 conservation neck dissections.We found no statistically significant difference in the rate of neck recurrence between the group of patients who underwent radical neck dissection and the group who underwent conservation neck dissect

AbstractThe petrified auricle is a clinical entity in which the auricle, in part or total, has become stone‐hard and moves as a rigid unit. This uncommon condition is most often due to ectopic calcification of the auricular cartilage. It may occur as a result of local trauma, in association with various systemic diseases such as Addison's disease, hypopituitarism, thyroid or parathyroid disorders, or following radiation therapy. Auricular ossificans (ectopic ossification) is a rare phenomenon in which the rigidity of the petrified ear is due to replacement of the elastic cartilage by bone. In the literature there are presently nine cases documented histologically. Two more cases are reported here.In most cases, the auricular changes are preceded by acute, severe hypothermia (frostbite). Exostoses of the external auditory canal is another disease of the external ear in which the proliferation of bone follows chronic mild hypothermia of the external auditory canal caused by exposure to cold water while surfing. AbstractWe studied the phenotype of T‐lymphocytes isolated from 18 patients with head and neck cancer, their capacity to express Ia antigens upon activation by lectinin vitro, their capacity to function either as responder or stimulator cells in autologous mixed lymphocyte reaction, and thier capacity to cooperate with the normal adherent suppressor cells (NASC).The T‐lymphocytes isolated from these patients have several functional defects including an impaired capacity to activate allogeneic lymphocytes in mixed lymphocyte reactions (MLRs), a lack of a proliferative capacity in autologous MLRs, an impaired sensitivity to inhibition by NASC, and an impaired capacity to express Ia antigens upon activation by mitogensin vitro.These data indicate that, in patients with head and neck cancer, immune function is characterized by a defect in T‐lymphocytes functions which concerns the process of cell to cell cooper
