AbstractOtitis media has long been recognized as one of the most common diseases of childhood. A prospective randomized study was designed with patients who had multiple episodes of acute purulent otitis media. Comparison of infection rates was made between patients treated with conventional antibiotic therapy for each episode and patients who had tympaYESstomy tubes placed. Placement of tympanostomy tubes significantly decreased the number of episodes of acute purulent otitis media and was shown to be an effective method of prophylaxis in the “otitis prone” child.Immunoglobulin levels in the middle ear in this group of patients were found to be similar to those levels in patients with otitis media with effusion. AbstractVascular tumors (vascular malformations and hemangiomas) of the temporal bone are uncommon, and guidelines for their management have not been published. In an effort to develop an approach to their management, the present study examined the pathological, clinical, and surgical experience with ten of these lesions treated at the Otologic Medical Group, Inc., and St. Vincent Medical Center in Los Angeles from 1960 to 1980.Intratemporal vascular tumors occurred most frequently at two sites, the internal auditory canal and the geniculate ganglion. Histological features and clinical behavior did not correlate. Both vascular malformations and hemangiomata invaded the facial nerve. Surgical excision in these cases required severence and repair of the involved facial nerve. Complete surgical excision is the treatment of choice of vascular lesions of the temporal bone.

AbstractTympanic membrane perforations are uncommon following the use of ventilation devices; however, persistent perforations should be given special attention for squamous epithelial growth through the perforation to the undersurface of the tympanic membrane. Surgical management is indicated in these cases. Tympanic membrane grafting procedures are adequate for early involvement. Successful grafting depends on excision of the tympanic membrane involved with the undersurface growth of squamous epithelium. Mastoid extension of the squamous tissue ingrowth requires greater surgical exposure. Cases and their management are presented.

AbstractA brief summary of methods for reconstruction of the external auditory meatus for whatever reason, which have been recommended and used by a number of authors, is presented. Recurrence of the atretic problem occurs in a great percentage of individuals necessitating further attempts with an even greater failure rate.In 13 cases with this problem, including congenital agenesis and/or atresia of the external auditory canal, exostectomies, and postsurgical stenosis, I have found that surgery with concomitant use of Merocel® wicks has given a “cure rate” of better than 90%. AbstractThe University of Califorina, San Francisco has been active in the development of the multichannel cochlear implants for rehabilitation of total sensory deafness for the past 8 years. Patients implanted with experimental prototypes of the devices now in use have demonstrated that speech discrimination of 40% to 50% is possible for common everyday words and sentence material. These devices provide the same stimulation to each of the 8 bipolar pairs in the cochlea. They have an internal disconnect device which allows for replacement and upgrading of implanted receiver‐drivers without disturbing the scala tympani electrodes. AbstractSince the incidence of pulmonary tuberculosis in the United States has declined dramatically, this diagnosis is often overlooked in the differential diagnosis of puzzling extrapulmonary disease. Recent publications emphasize that tuberculosis still needs to be considered in patients with chronic ear, nose, and throat complaints. A patient with tuberculosis involving both middle ears, the larynx, and the lung was correctly diagnosed only after a prolonged delay and several visits for chronically draining ears. As a result of the infection, subtotal bilateral tympanic membrane perforations and a profound hearing loss in both ears occurred. This case emphasizes the importance of a high index of suspicion in the evaluation and treatment of patients with symptoms of chronic disease who fail to respond to conventional therapy.

AbstractHemoglobin enhances the virulence of E. coli in the peritoneum. This study investigates the effect of blood on the virulence of E. coli and human oral anaerobes in the mouse neck. E. coli neck infectivity is significantly enhanced by blood. Oral anaerobic infections are not affected.

AbstractMalignant hyperpyrexia (MH) is a syndrome with a mortality of 60%, triggered by a broad variety of anesthetic agents. The most significant recent advance influencing the clinician's ability to safely manage MH individuals is the emergence of dantrolene sodium as both a prophylactic and a therapeutic agent. The purpose of this presentation is to review recent clinical and laboratory studies and advances which are the basis for establishing the pathophysiology of this syndrome and pharmacology of treatment. The FDA has recently approved intravenous Dantrium® for treatment of malignant hyperpyrexia. Intravenous and oral Dantrium® has much theoretical support and experimental evidence which suggests it will significantly reduce the mortality previously associated with this syndrom

AbstractThe Goodale‐Montgomery osteoplastic obliterative frontal sinusotomy has been the ultimate resource in America for the most difficult and intractable cases of frontal sinusitis. This operation respects the integrity of the frontal floor and is based upon a concept with which the author disagrees: that the frontal sinus may be treated as an entity quite separate from the ethmoid. Macbeth, who does not obliterate, avails himself of the excellent transfrontal approach to the ethmosphenoids, but breaches the frontal floor in so doing. American surgeons have not followed his lead. The pros and cons of obliteration are discussed, reviewing relevant clinical and experimental data. The author believes that Macbeth's operation is more rational and effective than Goodale's.
