AbstractThe ability of certain patients with an allergic diathesis to adapt to the ingestion of specific foods to which they are allergic accounts for the present confusion relative to the role food hypersensitivity plays in clinical medicine. An understanding of the concept of adaptation in ingestant allergy, with its stimulatory and withdrawal periods, aids both patient and physician in appreciating the problems involved in the recognition of food hypersensitivity.Specific adaptation undergoes a chronological development from the non‐adapted stage through full adaptation to eventual maladaptation. Recognition of the stage of development present offers further diagnostic help in evaluation of the allergic patient.Reintroduction of a specific food into the diet after establishment of an induced nonadapted phase provides an accurate means of determining hypersensitivity to individual ingestant

AbstractThe terms milk allergy, milk sensitivity, and milk intolerance are used in the medical literature with different meanings. Milk allergy and lactase deficiency to which they are usually referring can be specifically differentiated and diagnosed. A modification of the Rinkel oral test meal can be used to separate these two entities for medical diagnostic accuracy and understanding.

AbstractThe ideal surgical treatment for glomus jugulare tumors is total removal. Efforts have been made periodically to accomplish this since shortly after this tumor was first identified in the early 1940's. This paper describes a method of removal using a combined approach through the neck and temporal bone, preceded by a course of preoperative irradiation therapy. The early results that have been obtained using this procedure in 10 patients are reported.

AbstractThe CO2laser has been used in a variety of lesions for two and one half years in areas other than the larynx. These lesions include viral papillomas, leukoplakias and erosive lichen planus in various otolaryngological sites. Laser therapy is compared with other modalities of treatment for similar lesions and the advantages and disadvantages of this type treatment is evaluated.

AbstractFree flap reconstruction is discussed with emphasis on microsurgical technique utilized in union of blood vessels 1 mm in external diameter. Proper microsurgical instrumentation, magnification and small vessel suture technique are all of importance in achieving a successful microvascu‐lar anastomosis. Technical maneuvers employed in identifying and avoiding small vessel thrombosis are presented. Surgical techniques, as presented here, have allowed us to successfully use free flaps to reconstruct large oral cavity defects following ablative cancer operation

AbstractThe influence of various toxic substances and of drugs with ototoxic side effects upon energy generation, energy utilization, and membrane processes of the cochlea were studied. None of the drugs tested interfered with energy generation to as great an extent as did anoxia or cyanide and 2,4‐dinitrophenol. Ouabain produced a pronounced interference with energy utilization of the stria vascularis. The “loop” diuretics ethacrynic acid and furosemide produced a reduction of energy utilization of a lesser degree than did ouabain. The “loop” diuretics do not seem to exert their toxic action upon strial Na+K+‐ATPase, but may act by interfering with strial adenylate cyclase. Aminoglycoside antibiotics and diuretic and nondiuretic mercurials seem to exert their primary noxious action upon cochlear function by interfering with membrane processes of the structures bounding the co

AbstractTumor cells contain a variety of antigens, including tumor associated antigens. The tumor associated antigens can be clinically useful as marker's for detection of cancer and some may also mediate host resistance against tumor growth. Much emphasis has been placed on the detection of circulating tumor associated markers, with radioimmunoassays for carci‐noembryonic antigen (CEA) being extensively utilized.At present, CEA does not appear to be promising for initial detection of cancer, but may find an important place in the determination of prognosis and early detection of recurrent disease. Many cancer patients have depressed immunologic competence, yet they frequently have cell‐mediated immunity against tumor associated antigens. AbstractThis study presents a retrospective look at 115 patients evaluated here from 1957 to 1974. In this series, 57 percent were males and 43 percent females, 84 percent were Caucasian and 16 percent Negro. Lesions confined to or originating in the antrum made up 67 percent while non‐antral lesions were 33 percent. Mean age at diagnosis was 59.1 years. Smoking and drinking history did not appear to be contributory.Antral lesions were retrospectively staged according to Sisson's TNM classification. Sixteen tumor types were involved, with the most common being epidermoid. Diagnosis was most often made by intranasal or Cald‐well‐Luc biopsy. Most frequent symptoms, as well as earliest symptoms, were nasal obstruction, localized pain, and epistaxis. Average duration of symptoms was 6.4 months.Therapy was generally in the form of radiotherapy alone, preoperative radiotherapy and surgery, surgery and postoperative radiotherapy, or surgery alone. Local recurrences occurred in 44 percent of antral lesions and 50 percent of non‐antral lesions. Regional (cervical) nodal metastases developed in 25 percent of antral lesions and 11 percent of non‐antral lesions. Distant metastases developed in 30 percent of antral cases and 35 percent of non‐antral cases. Five‐year survival was 32 percent (35 percent determinate) for the total group. The more advanced the staging of the antral lesions, the worse the prognosis. Best survival figures were in the areas of preoperative radiotherapy and surgery at 38 percent (43 percent) and surgery alone at 56 percent

AbstractThe approach to the diagnosis and treatment of the patient with the unilateral opacified maxillary antrum presents a challenging problem. AbstractHead and neck neoplasms may invade the mediastinum by direct extension or metastases to the tracheoesophageal or jugulo‐subclavian lymph nodes. The clavicles and manubrium are a barrier to adequate resection in this type of disease. In 1962, the senior author reported six mediastinal dissections for stomal recurrences after laryngectomy.Later techniques for staging the removal of manubrium and clavicles and preparing the regional flaps were devised to avoid major complications arising from operating in thin area which usually had prior treatment with radiation and/or surgery. Over 60 transsternal radical neck dissections have been performed in the past 20 years. We have in the past evaluated the morbidity and survivals in stomal cases and present here our revised indications and technique
