AbstractA case is reported of an elderly woman, a resident, in a nursing home, who developed dysphagia over a short period. Clinical and radiological examination disclosed that she had a large laryngocele and a moderate sized Zenker's diverticulum. The management plan required consideration of her social circumstances, and careful ranking of the medical priorities. Both the laryngocele and the Zenker's diverticulum were excised at a single operation and she was successfully returned to her nursing home environment.

AbstractLaryngotracheal stenosis has not been reported previously as a manifestation of spondylocpiphyseal dysplasiu. Two recent cases are reported detailing the upper airway pathology encountered. The clinical features of spondyloepiphyseal dysplasiu are reviewed in an attempt to develop a treatment protocol for such patients with laryngotracheal stenosis. The ultrastructural abnormalities found in these patients' cartilages are emphasized. Caution is advised before attempting laryngotracheoplasty.

AbstractThe presence of thryoid tissue within the tracheal lumen is an unusual cause of upper respiratory obstruction. Thyroid tissue in this location may represent true ectopic thyroid or may be caused by invnsion of the trachea by a malignant process.This paper presents two patients from the same family who were found to have thyroid tissue in the trachea. The first illustrates the clinical course of a thyroid malignancy within the trachea, and the second had a true ectopic intratracheal thyroid mass.The embryology, diagnosis, and management of ectopic intratracheal tyyriod are outlined.

AbstractInflammatory illnesses of the pediatric airway cause significant morbidity and mortality. Bacterial tracheitis is a distinct entity with features common to both croup and epiglottitis. Ten patients between the ages of 3 months and 12 years were treated at Children's Hospital, Boston, MA., for bacterial tracheitis. The clinical presentation and medical management is discussed. Seven of the patients required both direct laryngoscopy and bronchoscopy; one patient required urgent intubation; and one patient required indirect laryngoscopy. In one patient the diagnosis was based on clinical and radiographic findings in conjunction with tracheal aspirates. Seven of the 10 patients did well with aggressive medical management. Three patients required endotracheal intubation. No patient required tracheotomy, and there were no cardiopulmonary arrests. It is of particular interest that although the patients in this series presented in a manner similar to that of patients in other published series, the management is significantly different and the overall outcome is significantly better.

AbstractInverted papilloma of the nose and paranasnl sinuses is a tumor not usually associated with childhood. These five cases from the Massachusetts Eye and Ear Infirmary provide the only series in the literature. The patients ranged in age from 6 to 20 years. One case involved the septum exclusively; the patient experienced a recurrence. The other four cases were of sinus origin. One of these patients also experienced recurrences before definitive surgery. Another presented with n simultaneous squamous cell carcinoma. Evaluation of these cases demonstrates that the same successful, comprehensive surgical management advised for adults is justifiable in this younger age group.

AbstractOsteoradionecroses of the maxilla and base of skull are rare phenomena, usually seen after combined therapy for malignancies of the maxillary sinus. While the mandible is most commonly affected by osteoradionecrosis, the maxilla and skull base may also be affected when preoperative or postoperative radiotherapy is combined with surgery.Contributing factors may be: high radiation dosage delivered to the treatment volume (>6000 rads), loss of tissue protective effects due to surgery, decreased vascularity caused by surgery and radiation, and proximity of a contaminated field.Onset of symptoms mny vary. One patient presented 25 years after postoperative radiotherapy. Major symptoms were pain, trismus, and purulent discharge. The best diagnostic modality remains the history and physical exam, as the area is readily accessible. CT scans may be helpful in diagnosis and treatment planning.Therapy should follow time honored principles of local wound care. Home irrigations and hyperbaric therapy have been helpful in encouraging early sequestration and rapid healing.

AbstractSinusitis tends to occur in immunodeficient and immunosuppressed patients during periods of severe leukopenia. This group of patients includes those with primary immunodeficiency diseases, patients with leukemia receiving chemotherapy, and those undergoing bone marrow transplantation or kidney transplantation. The clinical and radiographic signs may be minimal or initially unimpressive. Sinusitis clue toAspergillus, Phycomycetes, orPsendomonasmay be fulminant and even fatal, requiring extensive surgical procedures for control.

AbstractPsendomonas aeruginosa (Ps. Au.)infection of the maxillary sinus has been reported as an incidental finding on routine antrostomy; however, it has also been noted in several studies as the significant organism in the etiology of chronic sinusitis. Four case reports of culture verifiedPs. Au.maxillary sinusitis are presented. The therapeutic modality used in two of the cases was a Caldwell‐Luc operation and in two, an intranasal antrostomy. In all cases, multiple irrigations through the surgically created nasoantral windows were done postoperatively, as was the instillation of gentamicin ophthalmic drops intranasally.
