AbstractAnalysis of data on cell reproductive death, cell proliferation kinetics, tumor volume responses and tumor eradication probability after single and fractionated doses of radiation given to experimental tumors, can yield several conclusions which may be relevant for the interpretation of clinical data: 1. considerable differences are observed among various cell types with respect to intrinsic radiosensitivity, but no specific differences are demonstrated for cells from tumors as compared to cells from normal tissues; 2. cell survival curves measured byin vitrocloning techniques for cells irradiated in an experimental rat rhabdomyosarcoma are consistent with the observed probability of tumor eradication, and 3. after a sufficiently large dose of radiation and after two weeks of fractionated treatments, important changes in cell proliferation kinetics have been observed with respect to cell cycle, cell production rate and cell loss rate, which may significantly influence the time of recurrence as well as the probability of attaining tumor eradication.

AbstractExperimental models have been developed for the induction of carcinoma of the larynx and for the study of its pathogenesis. The hamster has been the animal of choice. Polynuclear hydrocarbons, administered intratra‐cheally, induce mostly squamous cell carcinomas. Diethylnitrosamine given systemically induces papillary tumors; some other N‐nitroso‐compounds given topically or systemically also induce laryngeal tumors, including carcinomas. Cigarette smoke inhalation induces preneoplastic and early neoplastic lesions of the larynx, including some invasive carcinomas.In vivoandin vitromethods have been developed for the morphological and biochemical study of target tissues in respiratory epithelial carcinogenesis to identify critical pathogenetic steps and their inhibition. AbstractEvidence accumulated over a period of 60 years has clearly established that a number of different viruses cause neoplasia (of a broad spectrum) amongst a variety of animals. Although it is known that viruses do produce self‐limiting proliferative human diseases, complete verification of a human viral carcinogen remains to be provided. AbstractA compilation of late and early symptoms of patients who ultimately were found to have laryngeal disease was carried out. Notation of even minor changes in the indirect laryngoscopic appearance of the same patients was made routinely. Correlation of symptoms with signs has made possible a better understanding of the natural history of certain laryngeal lesions such as contact ulcer, polyps, nodes, dysphonia plica ventricularis as well as some of the pareses, paralyses and fixations. Identical early symptoms and numerous signs have apparently been ignored in subsequently proven cases of cancer of the larynx.The expanded list of symptoms (33 in number) and the detailed list of signs (54 in number) may now assist the Otolaryngologist in earlier diagnosis. Although the reported findings are not specifically diagnostic of cancer of the larynx, they should draw earlier attention to the larynx as the etiological site of such symptoms. Earlier recognition of minor laryngeal changes, as detailed in the text, should lead to earlier biopsy and tissue diagnosis, earlier and more enlightened public awareness, and earlier and, therefore, more effective utilization of present day modalities in the management of carcinoma of the larynx.

AbstractThere is a need to establish the diagnosis of cancer of the larynx as early as possible. Delay in making the diagnosis should occur rarely if all of the available methods are fully utilized. Having established the presence of a carcinoma it should be possible to define the site and extent of the tumor; only with this additional information can the best treatment be selected.The use of a fiber‐optic laryngoscope or a telescopic laryngoscope (Gould) has made examination of the “difficult larynx” more satisfactory.X‐ray examination, with or without contrast material, has provided useful information regarding extent of the tumor, particularly with regard to its relation to the glottis.Microscopic laryngoscopy has proven to be a most reliable way of identifying “the early lesion” and of establishing the extent of an established tumor, especially if supravital staining is applied and the microsurgical laryngeal mirror and laryngeal caliper are used.The most difficult diagnosis to make at the present time is the presence of residual tumor after radiation, when the tumor does not present on the surface. AbstractPersistence of significant edema of the larynx beyond six months after completion of radiotherapy presents the lavyngologist with a diagnostic dilemma. Review of the results of 43 cases demonstrated a high incidence of residual or recurrent carcinoma. Based upon this experience earlier and frequent laryngeal biopsies are recommended for these cases.

AbstractIndirect laryngoscopy is one of the procedural methods used for achieving a histopathologic diagnosis of cancer of the larynx.A chronologic historical summary of the development of mirror laryngoscopy is presented. The details of the procedure employed for obtaining a biopsy with the aid of topical anesthesia is discussed.
