AbstractOrbital floor fractures can result in herniation of orbital tissues and impairment of ocular function. In this experimental study, orbital defects were created surgically in cats. The gross and histologic healing of these defects was compared among animals implanted with silicone rubber sheeting or absorbable gelatin film and animals that received no implants. Both implanted animal groups demonstrated enhancement of healing. The absorbable gelatin film showed less migration, less inflammatory response, and improved healing. This animal study supports the use of absorbable gelatin film in the surgical management of orbital fractures.

AbstractSusceptibility to many autoimmune diseases is associated with inheritance of certain Human Leukocytic Antigens (HLA), determinants which have become diagnostic or prognostic markers. In this study of HLA antigen frequencies among patients with autoimmune sensorineural hearing loss, 39 patients with this otologic disorder were typed for HLA‐A, B, C, and DR antigens. A significant increase of Cw7 occurred in 51% of patients compared to 21% of 627 matched controls (relative risk, 3.95). Trends towards increased frequencies of Cw4, B35 and reduced frequency of DR4 were also observed. These data suggest a possible immunogenetic predisposition to this otologic disorder. AbstractRound window closure is an uncommon complication of advanced otosclerosis found in 1% of 30,000 stapedectomy cases. A review of 81 patients representing 110 operation cases was made to determine what factors may alert the otologic surgeon to the patient with round window closure and what factors may identify the patient who will achieve the greatest benefit from stapedectomy.

AbstractPatients with primary hyperparathyroidism have a parathyroid adenoma approximately 80% of the time. Preoperative localization of parathyroid adenomas has been significantly improved by the utilization of high resolution (high frequency) real‐time ultrasound. AbstractA double blind study comparing a single dose of amoxicillin versus a 10‐day course of amoxicillin in the treatment of acute otitis media (AOM) was undertaken. All children diagnosed with AOM in a pediatric clinic were requested to participate. After tympanocentesis was performed, patients were randomly assigned to receive either amoxicillin (100 mg/kg) followed by placebo every 8 hours for 10 days or placebo followed by amoxicillin (40 mg/kg/D) for 10 days. Patients returned to clinic on days 3 and 10 post‐treatment.Seventeen patients were enrolled: 7 received single dose therapy and 10 received 10 days of therapy. There were no failures in the control group and 3 failures in the single dose group (day 5, day 6, and 11) (p=0.05). Middle ear isolates included:S. pneumoniae 7, H. influenzaa 4, B. catarrhalis 1, and no pathogen in 5. AbstractTwenty‐six infants and children underwent anterior cricoid split (ACS) in an attempt to avoid tracheotomy. Twenty (77%) were extubated successfully without tracheotomy; six (23%) failed and required tracheotomy. Our experience corroborates that reported in the literature. To date, 106 (77%) of 138 patients who have undergone ACS have been extubated successfully without tracheotomy. These results substantiate the validity of the operation when done for the proper indications. Substantive glottic and tracheal pathology indicate a high likelihood of failure of ACS.It is not clear precisely why the ACS is effective. Subglottic cysts are a factor in some cases. AbstractSeveral treatment forms exist for tinnitus. These multiple treatments speak to the elusive etiology of tinnitus. Many authors have reported tinnitus reduction or elimination with electrical stimulation. This study evaluates a new device (Audimax Theraband•) which delivers inaudible, transdermal, mastoid electrical stimulation. Thirty patients suffering from chronic tinnitus were evaluated in a single‐blind crossover fashion. Since the electrical stimulation is inaudible, each patient served as his or her own control. Patients received actual or placebo stimulation over a period of 2 weeks. They rated their tinnitus levels and the effectiveness of the device before and after using the Theraband.
