摘要:
A comprehensive study of haemostasis has been performed in a homogeneous group of 25 adult patients with conservatively treated chronic uraemia. We have found prolonged bleeding time, impaired platelet adhesiveness and aggregation, and decreased platelet factor 3 activity, increased values of fibrinogen, of factor VIII activity and related antigen, and of combined levels of factors II, VII and X. Non-significantly abnormal concentrations of factor XIII and of plasminogen and significantly lower values of plasminogen activators and higher values of urokinase inhibitors and anti-plasmin have also been found. Fibrinogen degradation products were detected in the urine of some patients.

A comprehensive study of haemostasis has been performed in a homogeneous group of 20 patients with nephrotic syndrome without renal failure. We have found unchanged number of platelets and a significant increase of platelet adhesiveness and aggregation; increased levels of activity and related antigen of fibrinogen, of factor VIII, of activity of factors II, VII and X and of antigens of factors XIII. Antithrombin III was unchanged in plasma and was detected in the urine. Euglobulin lysis times were decreased, and levels of plasminogen and its activators were increased after a venous occlusion test. At the same time urokinase inhibitors and antiplasmins were increased not only after, but also before a venous occlusion test. Fibrinogen degradation products have been found in the urine of all our patients but not in their sera.

摘要:
The effect of ultraviolet A phototherapy on uraemic pruritus was investigated in a placebo-controlled trial. Equivalent and significant reduction of itch was noted in both placebo and treatment groups as assessed by analogue scales and interviews. This reduction in itch could have been the result of either a placebo response or a biological effect of blue light.

The effect of water immersion to the neck on renal calcium and magnesium handling was studied in 11 nephrotic patients. There was an increase in the urinary excretion of both calcium and magnesium on immersion, and a return towards preimmersion control values in the hour following immersion. Clearances of calcium and magnesium, and fractional excretion of calcium and of magnesium all increased significantly during water immersion, and decreased in the postimmersion hour. However, magnesium excretion was 10–50 times greater than calcium excretion. Fractional excretion of sodium was highly significantly related to fractional excretion of calcium (p < 0.001) and magnesium (p < 0.001). The relationship between fractional excretion of phosphate and fractional excretion of calcium was statistically significant (p < 0.05), as was that between fractional excretion of phosphate and that of magnesium (p < 0.01). Magnesium and calcium fractional excretions were significantly correlated (p < 0.01). 摘要:
The antibody and cell-mediated immune responses after influenza vaccination were compared in 33 patients with chronic bronchitis and in 40 hemodialyzed patients. The results suggest a moderate impaired response in the hemodialyzed patients since the antibody titers and the level of cell-mediated immunity were consistently lower in these patients as compared to those suffering from chronic bronchitis. Nevertheless, the vaccine elicited an immune response in all the pre-vaccine seronegative dialyzed patients.

Recent investigations point to the nonenzymatic glycosylation as a cause of long-term complications in diabetes mellitus. We describe an enzymatic activity that cleaves glucose from the glomerular basement membrane (GBM), present in lysosomal preparations of diabetic lymphocytes. The GBM, nonenzymatically glycosylated or obtained from rats with diabetes, were incubated with enzyme preparations, separated on Sephadex G-25 and applied for glucose measurement on gas chromatography and mass spectroscopy. The lysosomal preparation of diabetic lymphocytes cleaved from rat GBM, which were nonenzymatically glycosylated 300–500 ng glucose/mg GBM protein, from diabetic rat GBM 300 ng glucose/mg GBM protein. A lysosomal preparation of normal lymphocytes failed to do so, indicating enzyme induction in the diabetic state. Control studies with the glycosylated hemoglobin Aic confirmed this finding and showed the specificity of the enzyme, as α-glucosidase and β-glucosidase failed to cleave the N-glycosidic bond between glucose and the protein. The ultrastructure of human renin granules has been studied from a case of Bartter’s syndrome. Renin bodies present roundish (R), lobulated (L) or sharply angulated (SA) shapes. The latter are indicative of the highest rates of renin synthesis and thereby of the greatest concentrations of the pressor enzyme. Their singular shape is consistent with the form monoclinic crystals assume when they grow in an isotropic medium. R granules form owing to filling of Golgian vesicles by renin. 摘要:
The effect of peritoneal dialysis on plasma ascorbate levels was investigated in 32 patients suffering from end-stage renal disease. Our studies demonstrated a high peritoneal clearance of ascorbic acid resulting in a significant loss into the dialysate. The quantity of ascorbic acid lost by patients undergoing peritoneal dialysis was proportional to the predialysis ascorbic acid levels. Since the ascorbic acid lost from the plasma during peritoneal dialysis is not adequately replaced by dietary consumption of vitamin C, patients undergoing chronic peritoneal dialysis should receive ascorbic acid supplementation as an important part of their therapeutic regimen.

Sclerotic processes of glomeruli in chronic pyelonephritis (CPN) and chronic diffuse proliferative glomerulonephritis (CDPGN) were investigated in a lectin binding study in connection with an immunofluorescent examination of protease inhibitor deposition. Ulex europaeus agglutinin-I (UEA-I), which is specific to a certain terminal α-L-fucosyl residue of glycoconjugates, specifically labelled intact endothelia of glomerular capillaries, peritubular capillaries and blood vessels in human kidneys. Segmental or global loss of the UEA-I binding with glomerular capillaries was observed in the sclerotic areas where αi-antitrypsin (α1AT) deposits were always detected in the glomeruli with segmental or global sclerosis of CPN. This high correlation between loss of UEA-I binding and α1AT deposition was also observed in the affected glomeruli of CDPGN.
