摘要:
The association of renal tubular acidosis and osteopetrosis in 2 Arabic siblings is reported. In addition, basal ganglial calcification and mental subnormality were present in 1 sibling. The major clinical manifestation in both was periodic hypokalemic paresis. The syndrome appears to be transmitted by recessive inheritance.

摘要:
10 children underwent cadaveric renal transplantation between the ages of 8.0 and 12.5 years for uremia secondary to infantile cystinosis. 6 children are doing well 6–62 months after-transplantation. 3 of the 4 other recipients required a second graft and eventually died of uremia or fulminant viral encephalitis, the other lost her first graft due to accelerated acute rejection and is now on maintenance hemodialysis. No further systemic complications of cystinosis have been observed in the patients with functioning grafts. Our experience confirms that kidney transplantation is the treatment of choice for uremic children with infantile cystinosi

Weanling male rats fed on a hypolipotropic diet develop acute renal failure whose morphological features vary from focal tubular necrosis to cortical necrosis. We have sequentially studied the hemostatic mechanism in correlation with the morphology of various tissues, mainly renal and hepatic, in choline-deficient rats as well as in three control groups. No important changes were observed in the hemostatic mechanisms before the development of tubular necrosis. Along with tubular necrosis a consumption coagulopathy was found, evidenced mainly by a decrease in the activity of factors V and VIII as well as a prolongation in PTTK and Quick’s time and a decrease in platelets. Fibrin degradation products were found in serum and urine and soluble fibrin monomer complexes in the former. Following tubular necrosis thrombi were found in the renal microvasculature. It is possible to speculate that the tubular necrosis induced by choline deficiency could produce an activation of the coagulation system which in turn would lead to thrombosis of the renal microcirculation and cortical necrosi

1α-hydroxycholecalciferol was tried as a therapy for renal phosphate wasting in kidney allograft recipients with normal parathyroid gland activity. During a 3-week period of treatment we observed a significant rise in renal phosphate threshold concentrations and plasma phosphate levels paralleled by a significant decrease in serum immunoreactive parathyroid hormone levels and a significant increase in intestinal calcium absorption. It is suggested that 1α-hydroxycholecalciferol acts on renal phosphate handling in a dual fashion: one is by suppression of parathyroid hormone and the other by restoration of 1,25-dihydroxycholecalciferol levels to an appropriate leve

Various haematological, ferrokinetic and iron absorption measurements were carried out on 15 patients with chronic renal failure who were undergoing regular haemodialysis. There was no evidence that the rate of iron absorption was impaired significantly by the depressed erythropoietic activity present in these patients. In contrast, there was a significant inverse correlation between the rate of iron absorption and the serum ferritin concentration, which has been shown to be an index of the size of the iron stores. This relationship, which was shown with both small (3 mg) and large (50 mg) doses of ferrous iron, was no different from that previously noted in subjects with normal renal function. These results suggest that iron deficiency in patients with chronic renal failure undergoing regular haemodialysis can be adequately prevented or treated with oral iron therapy, since the absorptive mechanism for iron appears to be normal.

In renal failure, absolute calcium excretion is low, but fractional excretion (FE) of filtered load is increased. In order to determine the role of metabolic acidosis in contributing to increased FEca, we have studied thyropara-thyroidectomized dogs in a control phase and following the induction of chronic renal failure, both during spontaneous metabolic acidosis and after correction with NaHCO3. FEca was 3.7% in controls and increased to 13.7% in azotemic acidotic dogs (p < 0.01). After correction of acidosis FENa was not significantly changed, but FEca fell significantly, to 8.1% (p < 0.01), while glomerular filtration rate, plasma calcium and filtered calcium load were unchanged. Thus although FEca is increased in nonacidotic azotemic dogs, acidosis further enhances calcium excretion by inhibiting renal tubular calcium reabsorption. These effects of metabolic acidosis may contribute to hypocalcemia and bone disease in azotemia.

摘要:
Using a modified everted sac preparation, intestinal absorption of 3H-digoxin was studied in rats. Chronic renal failure decreased mucosa to serosa transport and net transport significantly (p < 0.01). There was a significant correlation between degree of uremic intoxication (serum urea) and decrease of 3H-digoxin absorption. Serosa to mucosa transport of digoxin was not influenced by chronic uremia.

摘要:
Replication of Staphylococcus aureus as well as phagocytosis by normal human leucocytes was studied in acetate and lactate containing dialysate in vitro. Bacterial replication was neither impaired nor promoted by the media tested. Phagocytosis occurred at the same rate in both solutions studied.
