Exercise can induce proteinuria, hematuria and cylindruria in normal individuals. This suggests that exercise adversely affects glomerular function. In this study we examined the impact of moderate daily treadmill exercise on the glomerulonephritis (GN) of ‘one-shot” bovine serum albumin (250 mg/kg i.v.) serum sickness in rabbits. We found that exercise alone increased serum creatinine concentration (Scr) but exercise plus GN did not increase Scr further. Blood urea nitrogen values were unchanged. Albuminuria and the renal histopathology findings were not different between the exercised and non-exercised groups of rabbits. Muscle cytochrome oxidase and mitochondrial protein concentrations were not increased in the exercised animals. We conclude that exercise, below the level that causes exercise adaptation in muscle enzymes, does not adversely affect this form of acute

Acute, ischemic renal failure was induced in rats by clamping of the renal vessels for 1 h. Following the termination of the ischemic period 5 g/kg of dimethyl sulfoxide (DMSO) were administered intravenously as a 20% solution in saline. Control animals received normal saline. There were no deaths among the DMSO-treated animals. Urine flow began within 15 min of DMSO infusion. 24 h after the experiment the mean blood urea was 73 ± (SEM) 14 mg/100 ml (n = 29). All the control rats died during the week following the experiment. The mean blood urea at 24 h was 276 ± 18 mg/l00 ml (n = 20). In 10 additional animals perfusion of the kidney with DMSO prior to the closure of renal vessels protected the organ from ischemic damage. Hereditary nephritis associated with hematologic abnormalities seems to be an exceptional occurrence. We have observed a family in which nephritis was combined with May-Hegglin anomaly. A girl and her father suffered from proteinuria; a paternal uncle received a kidney graft; a paternal grand aunt died on periodic hemodialysis. The girl, the father and the uncle presented macrothrombocytopenia (40–100 × 109/l, size 4–8 µm) with prolonged bleeding time (which precluded renal biopsy) and cytoplasmic inclusions in neutrophils (Döhle bodies). These hematologic abnormalities characterize the May-Hegglin anomaly. This kind of association has not been reported on

Calcium transfer during intermittent peritoneal dialysis (IPD) was studied in four groups of patients with chronic renal failure. Group I dialyzed with a dialysis fluid concentration of 2.16 mmol/l. Group II dialyzed with the same calcium content, but with a hyperosmotic solution. Group III dialyzed with a high calcium concentration, 4.47 mmol/l in dialysis fluid and in group IV the patients dialyzed 5 cycles against a calcium-free solution followed by 5 cycles with normal calcium content. In the first three groups a significant rise in serum total calcium and ionized serum calcium took place. At balance studies a positive net transfer of calcium from dialysis fluid to blood was demonstrated, the largest transfer occurring in group III. High osmolality in dialysis fluid is of no importance for the calcium transfer. In group IV a significant fall in serum total calcium during the first 5 cycles was removed. The loss of albumin to the dialysate did not affect the calcium transfer as well as the changes in serum albumin and pH did not influence the calcium state during IPD.

Localization of acid phosphatase (AcPase) in proximal convoluted tubule (PT) cells obtained from human kidney biopsies of 25 patients with glomerulonephritis was studied by electron microscopy. AcPase activity was detected in the Golgi apparatus, GERL, microvesicles and cytosomes. Evidence suggestive of the fusion between AcPase-positive bodies and large apical vacuoles was seen, and release of AcPase into the lumen and basal membrane of PT cells sometimes encountered. The possible significance of this phenomenon is discussed.

Carotid pulse tracings and M-mode echocardiography were recorded in 25 patients on maintenance haemodialysis pre- and post-dialysis. Myocardial function, as assessed by fractional shortening and velocity of circumferential fibre shortening, was depressed in 7 out of 25 patients pre-dialysis (28%). Acute haemodialysis resulted in significant changes in body weight, mean arterial pressure, urea, creatinine and packed cell volumes in all patients. Left ventricular function, however, improved significantly only in that group of patients in which it was depressed prior to dialysis. Echocardiography provides a simple means for evaluating left ventricular function in patients on chronic haemodialysis and shows that cardiac performance improves with acute dialysis when it is depressed pre-dialysis.

In the Plymouth area, 95 patients with end-stage renal failure have undergone haemodialysis for 6 months or longer. Of the 47 patients beginning dialysis between 1967 and 1973, when water deionisers were not used routinely, a bone disease with multiple fractures, ‘fracturing osteodystrophy’, occurred in 18 patients and dialysis encephalopathy in 10. Of the 48 patients first dialysing between 1974 and 1979, when water deionisers were used commonly, fracturing osteodystrophy occurred in only one and dialysis encephalopathy also in only one. Duration of dialysis without a water deioniser appeared to be the mostimportant factor in the development of these two conditions. The use of water deionisers usually led to healing of fractures in patients with fracturing osteodystrophy and also led to improvement in 4 of the 11 patients with dialysis encephalopathy. Neither condition has occurred in any patient using a water deioniser from the first dialysis. The physiologic action of prazosin in man was studied in 23 patients including 10 treated with prazosin plus polythiazide. Responders to prazosin alone (blood pressure fell > 10 mm Hg) had a decrease in peripheral resistance (p < 0.02) and no significant change in plasma volume, while nonresponders’ plasma volume rose significantly (p < 0.02). 10 patients treated with prazosin and polythiazide (blood pressure not normalized on prazosin alone) were analyzed at baseline (Rx 0), after prazosin (Rx 1), and after prazosin plus polythiazide (Rx 2). Mean supine blood pressure fell from 184/118 ± 8/5 (SE) mm Hg atRx0 to 161/106 t7/4mmHgatRxl (p < 0.005) and 129/89 ± 4/4mmHgatRx 2 (p < 0.001). Supine renin increased in each group, but the increase with prazosin alone was not significant. Stimulated renin activity increased significantly after Rx 2 (p < 0.025). Circulating immune complex levels have been measured using four different assay systems in 2 patients with Wegeners granulomatosis during active and inactive disease and during different treatments. Changes in complex levels (particularly those containing IgG) have correlated well with disease activity and may be the first indication of relapse enabling treatment to be started before irreversible tissue damage has occurred. Cyclophosphamide, initially combined with plasmapheresis, has proved the most effective treatment available.

To examine the effect of T lymphocyte deficiency on the course of immune complex nephritis, we studied renal function and structure in Swissalbino nude and non-nude mice following injection of heterologous protein. Four groups of 22 mice each (two nude and two non-nude) received either apoferritin 2 mg or saline daily for 10 weeks. Nude mice were maintained in a gnotobiotic environment. Non-nude mice receiving apoferritin developed proteinuria and had increased cellularity within glomeruli compared to either nude mice receiving apoferritin or to control groups (p < 0.05). Of 22 non-nude mice receiving apoferritin, 16 had glomerular immune deposits by electron and immunofluorescent microscopy while 9 of 22 counterpart nude mice receiving apoferritin had such deposits. Non-nude mice more commonly showed membraneous deposits. Nude and non-nude mice receiving saline had no glomerular deposits. These preliminary data suggest that T lymphocytes may play a significant role in the development of immune complex nephritis.
