Identification of the various blood components that adhere to dialyzer membranes may further understanding of blood-membrane interactions during dialysis. We have examined coil membranes following 6–36 h of clinical dialysis plus 60 min of saline rinsing with light, scanning electron (SEM) and transmission electron (TEM) microscopy. Deposition of BC identified by Wright stain was generalized (excluding areas of mesh-membrane contact) and increased directly with blood perfusion time. The BC identified included leukocytes, platelets, fibrin-like strands, and many free cell organelles. Transcellular creatinine disequilibrium and its significance in hemodialysis. Creatinine and urea transcellular disequilibria were searched for on the venous side of a hemodialysis machine and in patients immediately following dialysis. Results indicated: (1) hemodialysis induces disequilibrium of creatinine but not for urea, between red cells and plasma on the venous side of a dialyzer; (2) much of the rapid rise in plasma creatinine concentration during the first hour following dialysis probably results from dissipation of a generalized transcellular creatinine disequilibrium, and (3) transerythrocytic creatinine disequilibrium necessitates incubation of dialyzer venous blood samples prior to plasma creatinine determination.

In the normal guinea pig, the rennin activity of renal cortical tissue was one tenth of that of the rat. Light microscopy did not reveal juxtaglomerular granules in epitheloid cells, while electron microscopy showed very rare small and pale granules. In adrenalectomized animals, cortical rennin activity increased fourfold 8–12 days after operation. Granules increased more than tenfold and were larger and darker; the rough-surfaced endoplasmic reticulum expanded. Data suggest that normal guinea pig kidney may contain extragranular cytoplasmatic renin, while larger amounts of renin occurring after adrenalectomy are deposited in newly formed granule

A family group of first and second degree relatives is presented. Members of the group have vesico-ureteric reflux and/or duplex collecting systems. Renal scars of chronic pyelonephritis are seen. It is considered that because histological features are not always specific and because the relationship of renal scars to infection uncertain, chronic pyelonephritis is a diagnosis made on radiological evidence. It is suggested that the renal scars of this condition may result from vesico-ureteric reflux in infancy. Vesico-ureteric reflux is a congenital abnormality related to the position of the junction on the bladder wall. When double ureters are present, reflux is more common. The possibility that the congenital abnormalities are inherited, is discussed. The abnormalities in the family group presented, suggest that a single gene inheritance is possible, with variable expression and occasional lack of penetrance, and no sex linkage. This genetic basis of the abnormality, suggests that anatomical and physiological disturbances underly the development of renal scarring and are worth looking for more widely in the families of those already presenting with symptoms.

Prompted by the observation of marked alkaline phosphatase (AP) elevation in a patient with severe, bilateral pyelonephritis, a retrospective survey was undertaken of the records of patients hospitalized during 1970–1972 under the headings of pyelonephritis, papillary necrosis, hydro- and pyonephrosis. Patients with liver and bone disease or chronic renal failure were excluded. Out of 95 consecutive patients studied, 14 (14.7%) had substantial elevations of serum AP. The common feature of most cases was pyelonephritis with severe systemic manifestations; four patients had gram-negative bacteremia; two patients died of sepsis and uremia and the autopsy findings included bilateral, widespread, nonobstructive pyelonephritis with papillary necrosis in one case and multiple renal abscesses in the other. Eight patients had associated uni- or bilateral hydronephrosis. It is postulated that inflammatory tubular damage and extra- or intrarenal obstruction can result in release of AP into the circulation. Rat kidney plasma membrane preparations and suspensions of intact glomeruli were used to study glycoprotein: glycosyl transferase activities present on cell surfaces. Collagen: glucosyl, fetuin: galactosyl, and fetuin: sialic acid (sialíc acid is the generic term for N-acetylneuraminic acid, NANA) transferase activities were increased from 1.9- to 5.6-fold in membrane preparations compared to whole tissue homogenates. Considerable activity was found with suspensions of glomeruli though in each assay specific activity was substantially less than for glomeruli homogenized and sonicated in buffer containing the nonionic detergent Triton X-100. For both membranes and glomeruli, collagen: glucosyl and fetuin: galactosyl transferase activities were enhanced by addition of folic acid whereas sialic acid transferase activity was reduced

An attempt was made to improve renal function in five patients with malignant hypertension and progressive renal disease by treatment with intravenous heparin. Oral dipyridamole, an antithrombic agent, was given in four patients. The renal disease was far advanced in four of the five, but only one patient had evidence of a microangiopathic hemolytic anemia. Although there was an apparent beneficial effect of heparin and dipyridamole on the microangiopathic hemolytic process, no beneficial effect of therapy on the renal function of any of the five patients was documented.
