Red blood cell (RBC) phosphate release was linear for more than 1 h and dependent on the intracellular hydrolysis of organic phosphate esters. In uremics on chronic hemodialysis total phosphate release was significantly increased suggesting an elevated RBC energy metabolism. Ouabain-sensitive phosphate release, however, was decreased. For RBCs of controls and uremic subjects approximately 80% of inorganic phosphate liberated within the cell was recycled. Thus, RBC phosphate release represents 20% of intracellular phosphate ester metabolism. In uremia active electrolyte transport was diminished, suggesting an impaired Na-K-ATPase activity. It resulted in an increased RBC sodium and a decreased potassium concentration. The positive correlation between ouabain-sensitive rate constant for sodium efflux and ouabain-sensitive RBC phosphate release indicates that ouabain inhibition of phosphate elimination might be related to Na-K-ATPase. In RBCs of uremic subjects almost 4% of the increased energy metabolism was needed for active electrolyte transport mechanisms, in control RBCs 12% was required.

We have used homologous platelet labelling with 111ln oxine in the diagnosis of thrombocytopenia occurring in a patient who had received two renal transplants. The technique enabled us to prove that platelet destruction was occurring in the right-sided, non-functioning transplant. In the presence of moderately impaired bone marrow function, this destructive process was sufficient to cause thrombocytopenia. Transplant nephrectomy relieved the thrombocytopenia.

Peritoneal dialysis was performed in 18 uremic patients using both room temperature (27–31°C) and pre-warmed (37°C) dialysate. There were no significant differences between the two periods of the following parameters: urea clearance, creatinine clearance, inulin clearance, ratio of inulin to urea clearance, ratio of inulin to creatinine clearance, drainage volume, dialysate flow rate, ultrafiltration rate and total protein loss. Since the peritoneal dialysis efficiency was not improved by warming dialysate in this range of temperature, the warming procedure is not necessary in the countries where the average room temperature is 29°C (27–31°C). Electron microscopic examination was carried out on 10 glomeruli from 4 infants with congenital nephrotic syndrome of the Finnish type (CNF) and 5 glomeruli from 1 control. The thickness of the peripheral glomerular basement membrane (GBM) and lamina densa were measured. The width of the GBM was thinner in CNF than in the control but still within normal limits for the age and the width of the lamina densa was thinner in CNF compared to the control. This study supports earlier findings that the basic defect in CNF could be a faulty synthesis of the lamina densa.

Chronic interstitial nephropathy with disproportionate hyperuricemia (serum uric acid 10.5–14.8 mg/dl [625–880 µmol/l] at a GFR of 40 ml/min/1.73 m2) was observed in 2 girls and their mother who suffered from gout since the age of 20 years. Urinary excretion of uric acid was normal. Renal biopsies in the 3 patients showed focal tubulointerstitial nephropathy. Absolute values of GFR remained stable in the 2 pediatric patients over a period of 10 years, whereas the older patient required dialysis at the age of 34 years. We speculate that this family suffers from a primary interstitial nephropathy which is accompanied by a subtle defect in tubular excretion of urate. A 45-year-old male was admitted to the hospital because of polyuria and polydipsia. After admission, proteinuria and hematuria were found. The kidney function deteriorated and necessitated the initiation of chronic hemodialysis. Examination of the bone marrow revealed multiple myeloma and kappa light chains were found in the urine. The kidney biopsy showed membranoproliferative glomerulonephritis with dense deposits in the glomerular basement membrane.

A successful pregnancy in a patient who presented with renal failure secondary to pelvi-ureteric obstruction is described. Haemodialysis was instituted during the pregnancy. An Anderson-Hynes pyeloplasty was performed, probably preventing deterioration of remaining renal function. Metabolic acidosis was managed using additional bicarbonate in the dialysate. An active, multidisciplinary approach is recommended in the management of such patients.

A 28-year-old patient receiving a kidney transplant from a 2-year-old cadaver kidney donor is described. The patient developed hypertension in the posttransplant period due to renal arterial stenosis. The hypertension was unresponsive to oral therapy after several unsuccessful attempts to correct the stenosis, nephrectomy of the transplanted kidney was performed resulting in the control of the hypertension.
