摘要:
The cellular pattern of skin exudate in untreated uremic patients differed from that in healthy persons. The skin exudate composition in patients on maintenance hemodialysis approached normal values. Such normalization was, however, static: after sensitization with dinitrochlorobenzene – contrary to the control – both treated and untreated patients exhibited no changes in the cellular pattern of exudate. The activity of acid phosphatase in the cells of exudate was highly positive in control, almost normal in dialyzed, but weak in untreated uremic patients. The influence of hemodialysis on oxygen consumption was studied in 15 patients on maintenance dialysis. Red cell 2,3-DPG, P50, an inverse measure of oxygen affinity of hemoglobin, arterial and central venous blood gases and cardiac index were measured. 2,3-DPG remained unchanged, whereas in vivo P50 fell significantly during dialysis due to a rise of pH (Bohr effect). Arterial PO2 was lower after than before dialysis, but arterial and central venous oxygen saturations did not change significantly. Cardiac index increased from 3.66 to 4.05 liter/min/m2. Oxygen consumption rose from 120.5 to 131.7 ml/min/m2 (p < 0.05), the rise being accounted for by an increase in cardiac index and by a slight post-dialysis hemoconcentration. However, even correcting for these parameters did not reveal a decrease in oxygen consumption. It is concluded that, contrary to previous assumptions, the hemodialysis-induced rise in pH with its consequent increase of oxygen hemoglobin affinity did not impair oxygen delivery in this group of patients on maintenance dialysis.

The renal glomerular and tubular transport rate of amylase was studied by measuring the urinary excretion of this protein before and during inhibition of tubular protein reabsorption by lysine. The excretion of amylase was compared with the excretion of albumin, β-2-microglobulin and free light chains of immunoglobulins. This investigation showed that amylase is reabsorbed by the tubular cells, but only to a very modest degree compared with the reabsorption of the other three proteins. In the case of amylase only about 45% of the filtered molecules are reabsorbed, whereas more than 90%o of the filtered amount of the other molecules is reabsorbed by the tubular cells. The excretion of amylase rose after lysine injection only by a factor 1.8, whereas excretion rose by a factor 28 for albumin, 1,500 for β-2-microglobulin, 16 for kappa chains and 8 for lambda chains. 摘要:
Quantitation of the thrombocytopathy of uraemics may be one useful way of evaluating forms of therapy. 24 patients treated by haemodialysis and peritoneal dialysis at two different times had platelet aggregation studies whose parameters were compared with those of 24 normal persons, 5 successful transplants or 13 untreated uraemics. Renal transplantation and peritoneal dialysis improved platelet function. The haemodialysis procedure itself impaired platelet function: this was not due to heparin.

The effect of changes in certain physicochemical parameters, analogous to those occurring in the kidney, on stimulation of normal human lymphocytes by phytohemagglutinin was studied. Increase in osmolality by sucrose or urea, and increases in concentration of sodium, potassium and calcium all significantly inhibited stimulation while magnesium did not. Lowering pH to 6.8 and 6.5 inhibited stimulation. Raising the pH to 8.0 had no effect but at pH 8.4 stimulation was decreased. The effect was largely but not entirely due to decrease in viability.

Percutaneous renal biopsy was performed on 5 patients with selective vitamin B12 malabsorption of whom 3 had proteinuria. Light microscopy showed slight prominence of the mesangial areas but otherwise the findings were normal. Electron microscopy showed increased mesangial matrix, thickening of the basement membrane at the mesangial areas and dark mesangial deposits. Light flocculent subendothelial material, moon craters, membranous convoluted structures, extracellular round particles and occasional intracellular microtubular inclusions were also seen in the glomeruli. The tubular basement membrane appeared thickened around a few tubules, showing membranous vesicular debris and convoluted structures. The capsular membrane also showed vesicular debris. On immunohistochemical examination the glomerular deposits contained immunoglobulins but not complement – an argument against their immunological nature. Deposits were seen only in patients who had been on inadequate treatment for years before the biopsy. Adequate treatment after correct diagnosis decreased the amount of deposits. 摘要:
Subacute sclerosing panencephalitis (SSPE) is a progressive neurologic disease associated with chronic measles infection. We have assessed renal function parameters and renal biopsy details in 14 patients with SSPE. All had normal renal function. However, 9 had microscopic hematuria. 9 patients had focal segmental mesangial proliferation and 5 had capillary wall thickening with deposits visible by light microscopy. 13 patients had glomerular C3 deposition and 4 had IgG deposits. SSPE is a disease caused by chronic viral infection, in which nephritogenic complex formation may occur.
